HYPOTHESIS: The Disabilities of Arm, Shoulder, Hand (DASH), EuroQol-5D (EQ-5D), Health Utilities Index Mark 3 (HUI3), and Short Form (SF)-6D questionnaires are reliable and valid measures of functional outcome and health state values in patients with proximal humeral fractures.
MATERIALS AND METHODS: Patients aged 55 and older treated for a proximal humeral fracture during a 5-year period completed the DASH, EQ-5D, HUI3, and SF-12 questionnaires. Test-retest reliability was quantified using intraclass correlation (ICC 2,1) and Bland-Altman agreement statistics during a second administration o

BACKGROUND: The incidence of closed proximal humerus fractures (CPHF) is steadily increasing worldwide. Open reduction internal fixation (ORIF) remains the most common surgical technique while reverse shoulder arthroplasty (RSA) is becoming a noteworthy approach. The optimal management of CPHF (ORIF vs. RSA) remains complex and continually evolving.
METHODS: This retrospective cohort study utilized data from the National Inpatient Sample (2016-2019). Patients with proximal humerus fractures (PHF) treated with ORIF or RSA were included. Demographics, fracture characteristics, comorbidities, an

BACKGROUND: Posttraumatic shoulder stiffness remains a problem after proximal humerus fracture (PHF) despite good healing rates. The aim of this pilot study was to determine whether the implant material and overlying soft tissue have an effect on shoulder range of motion (ROM) before and after implant removal (IR).
METHODS: 16 patients (mean age 55.2 ± 15.3 (SD) years; 62.5% female) were included who underwent operative treatment with locking plates of either carbon fiber reinforced Polyetheretherketone (PEEK) (PEEKPower® humeral fracture plate, Arthrex, Naples, Florida, USA, n = 8) or titani

HYPOTHESIS AND BACKGROUND: Recently, the indication of reverse total shoulder arthroplasty (RTSA) has expanded beyond rotator cuff arthropathy to include treatment of complex acute proximal humeral fracture (PHF). Limited previous studies have compared the long-term clinical and functional outcomes of patients undergoing RTSA for PHF vs. elective indications for degenerative conditions. The purpose of this study was to compare implant survivorship, reasons for revision and functional outcomes in patients undergoing RTSA for acute PHF with those undergoing elective RTSA in a population-based co

BACKGROUND: Treating seizure-related shoulder injuries is challenging, and an evidence-based consensus to guide clinicians is lacking. The aim of this prospective single-center observational clinical trial was to evaluate the clinical results of a cohort of patients undergoing treatment of seizure-related shoulder injuries, to categorize them according to the lesion's characteristics, with special focus on patients with proximal humerus fracture-dislocations (PHFDs), and to define groups at risk of obtaining unsatisfactory results. We hypothesized that patients with a PHFD, considered the wors

BACKGROUND: The diagnosis of bone mineral density (BMD) abnormalities involves dual-energy x-ray absorptiometry (DXA), but few patients complete this after a fragility fracture. The assessment of BMD using Hounsfield unit (HU) measurements from computed tomography (CT) scans has been correlated with DXA results in previous studies. We aimed to evaluate the correlation between shoulder CT HU and DXA scores.
METHODS: Billing databases of 3 academic institutions were queried for patients who underwent both DXA and CT scan of the upper extremity within 1 year of each other. DXA T-scores for spine

BACKGROUND: Reverse shoulder arthroplasty (rTSA) is increasingly being used to treat complex proximal humerus fractures, especially in the senior population. The question of whether it is worthwhile to repair the greater tuberosity (GT) after rTSA for proximal humerus fractures still needs to be answered.
METHODS: A systematic review was performed on PubMed, Scopus, Cochrane library, and Embase for studies available up to February 2024 focused on tuberosity repair after rTSA for proximal humerus fracture and reporting clinical outcomes and range of motion (ROM) between patients with anatomic

We evaluated the responsiveness of patient questionnaires and physical testing in the assessment of recovery after distal radius fracture. Patients (n = 59) were assessed at their baseline clinic visit and again 3 and 6 months after injury. At each visit patients completed a short form-36, Disability of the Arm, Shoulder, and Hand questionnaire, and patient-rated wrist evaluation (PRWE). At 3 and 6 months grip strength, range of motion, and dexterity were analyzed. Standardized response means (SRM) and effects sizes were calculated to indicate responsiveness. The PRWE was the most responsive.

BACKGROUND: Proximal humerus fractures (PHF) are common and painful injuries, with the majority resulting from falls from a standing height. As with other fragility fractures, its age-specific incidence is increasing. Surgical treatment with hemiarthroplasty (HA) and reverse shoulder arthroplasty (RSA) have been increasingly used for displaced 3- and 4-part fractures despite a lack of good quality evidence as to whether one type of arthroplasty is superior to the other, and whether surgery is better than non-surgical management. The PROFHER-2 trial has been designed as a pragmatic, multicentre

BACKGROUND: The proximal humerus is an easily overlooked site for metastatic bone tumors, and pathological fractures here can cause severe pain and functional impairment. The selection of surgical regimens should take into account the location of bone metastases, the patient's physical status, and other relevant factors. Therefore, the optimal surgical strategy remains controversial. This study aimed to investigate and compare the clinical efficacy of two surgical approaches-tumor resection combined with prosthesis implantation versus plate fixation combined with bone cement augmentation-in pa

INTRODUCTION: The proximal humerus fracture (PHF) is one of the most common fractures in the elderly. The majority of PHFs are treated non-operatively, while 15%-33% of patients undergo surgical treatment. Recent randomised controlled trial (RCT) and meta-analyses have shown that there is no difference in outcome between non-operative treatment and locking plate or hemi-arthroplasty. During the past decade, reverse total shoulder arthroplasty (RTSA) has gained popularity in the treatment of PHF, although there is a lack of RCTs comparing RTSA to non-operative treatment.
METHODS: This is a pro

BACKGROUND: Symptomatic proximal humeral fracture sequelae (PHFS) represent a surgical challenge due to the altered bone and soft tissue morphology. The purpose of this study was to report the outcome of Multiplanar Corrective Humeral Osteotomies (MCHOs) in combination with reverse total shoulder arthroplasty (rTSA) performed following a three-dimensional (3D) preoperative planning and using a 3D-printed patient-specific surgical instrumentation (PSI) in type 1C, 1D, and 4 PHFS.
METHODS: In this prospective monocentric study, we enrolled patients affected by symptomatic PHFS type 1C, 1D, or 4

We retrospectively reviewed alpine skiing injuries at a destination ski resort during three seasons to characterize the incidence and types of shoulder injuries. A total of 3451 injuries in 3247 patients were reviewed. The overall injury rate was 4.44 injuries per 1000 skier-days. Injuries to the upper extremity represented 29.1% (N = 1004) of all alpine ski injuries. Injuries involving the shoulder complex (393 injuries in 350 patients) accounted for 39.1% of upper extremity injuries and 11.4% of all alpine skiing injuries. The rate of shoulder injury was 0.51 injuries per 1000 skier-days. Pa

PURPOSE: Shoulder dystocia is the strongest known risk factor for brachial plexus birth palsy (BPBP). Fractures of the clavicle are known to occur in the setting of shoulder dystocia. It remains unknown whether a clavicle fracture that occurs during a birth delivery with shoulder dystocia increases the risk of BPBP or, alternatively, is protective. The purpose of this study was to use a large, national database to determine whether a clavicle fracture in the setting of shoulder dystocia is associated with an increased or decreased risk of BPBP.

There has been a decrease in the overall injury rate and the rate of lower extremity injuries for alpine skiing, with a resultant increase in the ratio of upper extremity to lower extremity injuries. Upper extremity injuries account for 20 to 35% of all injuries during alpine skiing and nearly 50% of all injuries during snowboarding. The most common upper extremity injuries during skiing are sprain of the thumb metacarpal-phalangeal joint ulnar collateral ligament, and the most common in snowboarding is wrist fracture. Shoulder injuries from skiing and snowboarding have been less well characte
