RATIONALE: Information on referred pain can be helpful for diagnosing diseases of the visceral organs. Here, the authors report a patient with cervical spinal cord injury (SCI) who had referred pain at the right side from the neck to shoulder, as a presentation of pulmonary embolism (PE).
PATIENT CONCERNS: A 55-year-old man with complete tetraplegia, due to cervical SCI after C5 and C6 vertebral body fracture, complained of right neck-to-shoulder pain (numerical scale rating: 6). We retrospectively reviewed 26 patients who underwent reconstruction of the shoulder for a medial rotation contracture after birth injury of the brachial plexus. Of these, 13 patients with a mean age of 5.8 years (2.8 to 12.9) received an injection of botulinum toxin type A into the pectoralis major as a surgical adjunct. Brachial plexus palsy with upper segment C5 and C6 root avulsion can be effectively treated using a part of the radial nerve transferred to the axillary nerve. Shoulder stability and abduction was obtained with good deltoid muscle contraction. When combined with spinal accessory nerve transfer to the suprascapular nerve, the overall functional result was enhanced. Elbow flexion can be reconstructed by partial median nerve transfer to the musculocutaneous nerve. There were no neurologic deficits after nerve surgery.

BACKGROUND: Spinal accessory nerve (SAN) transfer is commonly used to reanimate the suprascapular nerve (SSN) in traumatic brachial plexus injuries (BPI). Despite its technical simplicity and satisfactory results, this transfer has two disadvantages: the inadequacy of the number of nerve fibers between donor and recipient and the weakening of the trapezius muscle.
