An anatomic comparison of cebocephaly and ethmocephaly.

Cebocephaly (hypotelorism, single-nostril nose) and ethmocephaly (hypotelorism, interorbital proboscis) lie in the middle of the spectrum of … th ethmocephaly. Both manifested dysplastic changes of the ethmoid bone and anterior portion of the sphenoid bone, with concomitant hypotelorism and defects of the medial orbital walls. Through these latter defects, the eyes were joined in the ethmocephalic fetus (synophthalmia). Other changes of bone (single optic foramen, approximated maxillae, choanal atresi …

Experimental unilateral coronal synostosis in rabbits.

Immobilization of the coronal suture was produced unilaterally in 9-day-old rabbits to determine its effect on subsequent craniofacial development. The suture was immobilized unilaterally by the topical application of methylcyanoacrylate adhesive. Subsequent growth effects on the cranial vault, base, and facial skeleton were assessed by serial radiographic cephalometry. Unilateral coronal suture immobilization resulted in significantly decreased bone growth at the coronal suture (mean 0.95 mm +/- 0.35 SE) when compared to sham-treated control animals (mean 5.06 mm +/- 0.20 SE). Frontonasal sut

Temporal double inversion method in reshaping the temporal bulging in a case of Apert's syndrome.

A case is reported of Apert's syndrome with severe bilateral balloonings of the temporal region in addition to other typical craniofacial deformities. He had had three previous strip craniectomies. His mentality and intelligence were normal. In order to improve his deformities, the following steps were carried out: 1. 1.5 cm frontal advancement with reshaping the horizontal arc of the supraorbital ridge by out-fracturing. 2. Bilateral temporal double turnover method with internal decompression of the temporal lobe by resection of the sphenoid wing abnormally roofing the temporal lobe. 3. Reduc

Maxillary sinus osteoma: two cases and review of the literature.

Osteomas are benign tumours characterized by proliferation of compact or cancellous bone. The most common site is the mandible, followed by the sinuses. These tumours are slow-growing, usually asymptomatic, and are generally discovered as incidental radiological findings. Osteomas occur commonly in frontal sinus, followed by the ethmoid and maxillary sinus, and very rarely in the sphenoid sinus. Symptoms arise when osteomas obstruct the ostium of the sinus or impinge on adjacent orbital or intracranial structures. Two cases of maxillary sinus osteoma are presented along with a review of the li

[The value of computer reconstruction of the frontal and sagittal planes of the facial structures].

The authors evaluate the merits of computer reconstructions in the coronal and sagittal planes to assess the anatomy of the facial structures. If the patient can be kept motionless, the coronal plane provides information similar to those obtained by a study with direct sections. The ethmoidofrontal recesses, the lamina cribrosa, the thin relief of the middle meatus, the floor of the orbit are perfectly examined. The sagittal plane provides a new approach of the anterior and posterior cortical bone, of the frontal sinus, of the nasofrontal canal, of the ethmoidal cells, of the sphenoid sinus an

Congenital/primary clival meningocele: A systematic review and surgical approach based on a new proposed classification.

Skull base meningoceles are extremely rare, most commonly resulting from congenital defects in sphenoid bone fusion. Inadequate embryonic fusion between the posterior sphenoid and the occipital bones may result in a transclival meningocele. Given the absence of a formal classification system for clival meningoceles and clear guidelines for the most appropriate endoscopic skull base surgical approach, we combined an anatomical study and a systematic review to characterize anatomical patterns, propose a preliminary classification, and discuss transoral and endonasal surgical corridors according …

Updating nasalisation: From concept to technique and results.

Nasalisation is a surgical technique proposed for the treatment of nasal polyposis in 1995. The technique as initially described was based on large opening of the maxillary, sphenoidal and frontal ostia and resection of the middle turbinates, with the aim of resecting the mucosa of the lateral masses of the ethmoid as completely and safely as possible. Recent findings on the evolution and development of the nose and sinuses and sinus physiology allow both the concept and the technique of nasalisation to be updated. According to evo-devo theory, the ethmoid is not a paranasal sinus but the skul

Obliterative sinusitis: An underreported clinical entity.

Obliterative paranasal sinusitis is a rare sequela and end stage of chronic rhinosinusitis, which is often overlooked on imaging. It is an exuberant form of neo-osteogenesis characterized by complete or partial obliteration of the sinus cavity by new bone formation. This article presents a series of cases of obliterative sinusitis involving the sphenoid and maxillary sinuses. It is important to comment on the presence of obliterative sinusitis in the radiology report and not confuse it for native bone, fibro-osseous lesion, or arrested pneumatization, as this may have clinical implications for

Fibrous dysplasia and aneurysmal bone cyst of the skull base presenting with blindness: a report of a rare locally aggressive example.

Fibrous dysplasia (FD) and aneurysmal bone cyst (ABC) are uncommon benign intraosseous lesions. Simultaneous occurrence of both lesions is extremely rare. We present an example of concomitant FD and ABC in a 7 year-old with left eye blindness and discharge of one month duration. Physical examination revealed a proptotic left eye and bulging of the hard palate. CT and MRI are consistent with FD and ABC that involved the sphenoid and ethmoidal bones bilaterally. Incomplete combined endonasalcranial resection was performed. The patient presented five months postoperatively with a large recurrence

Transalar sphenoidal meningoencephalocele associated with buccal angiomatosis. One case.

Basal encephaloceles are uncommon congenital malformations occurring with an estimated frequency of one case in 35,000 births. They are frequently occult since they protrude into the orbit, nasal cavity, nasopharynx or paranasal sinuses. Basal encephaloceles are divided into five anatomical types. We report a case of transsphenoidal encephalocele which differed from the classical type in that it protruded through a congenital defect in the greater wing of the sphenoid bone. This case had an unusual clinical presentation, being responsible for trigeminal neuralgia and associated with an angioma…

Transsphenoidal injection of silicone for the production of communicating or obstructive hydrocephalus in dogs.

A technique for the insertion of a spinal needle through the soft palate and sphenoid bone into the suprasellar cistern of dogs is described. Instillation of room-temperature-curing silastic elastomer via the needle causes an increased resistance to the flow of cerebrospinal fluid in the basal subarachnoid spaces above the tentorium, for the production of communicating hydrocephalus. If the needle is advanced farther, the same material can be injected directly into the third ventricle for the production of obstructive hydrocephalus. Twenty-five of 34 dogs (74%) developed hydrocephalus and surv…

Morphological and morphometric study on sphenoid and basioccipital ossification in normal human fetuses.

Congenital anomalies of the brain frequently correspond to cranial base anomalies, and a detailed description of morphology and individual variations in the developing cranial base is of clinical importance for diagnosing anomalies. Development of the human cranial base has been studied using dissection, computed tomography, and magnetic resonance imaging, each of which has advantages and disadvantages. We here examined development of the normal human fetal cranial base using bone staining, which allows for direct observation of the ossification centers and precise three-dimensional measuremen

Restrictive Strabismus Following Frontotemporal-orbitozygomatic Craniotomy.

The frontotempotal-orbitozygomatic craniotomy (FTOZ) is a standard approach for large sphenoid wing meningiomas (SWMs). Nevertheless, resection of these tumors is not without ophthalmologic risks. This series presents two patients with acute postoperative restrictive strabismus following tumor resection and orbital wall reconstruction. Forced duction testing and postoperative imaging revealed impingement of the lateral rectus muscle caused by an alloplastic implant and/or residual bone, prompting immediate orbitotomy and restoration of normal extraocular muscle function. This report highlights

Bilateral abducens paralysis revealing bipartite spheno-occipital synchondrosis fracture in a child: case based update.

BACKGROUND: After reviewing the literature about sella turcica and spheno-occipital synchondrosis fracture, we present the case of a 6-year-old girl who suffered complex sphenoid bone fracture produced by an indirect mechanism. METHODS: The girl fell from her height after a blunt head injury due to a falling lamppost. RESULTS: Computer tomography (CT) scan showed occipital, sellar and clival fractures with pneumatocephalus. The probable fracture mechanism is contre-coup injury linked to osseous and brain oscillations after head trauma. CONCLUSION: Two complications, abducens nerve palsy and…

Pituitary adenoma composed of two compartments each secreting prolactin or growth hormone.

We report a case of pituitary adenoma with two compartments, i.e. a part within the sella turcica and a part infiltrating the sphenoid bone beneath the sella, producing growth hormone (GH) and prolactin. The patient had the amenorrhea-galactorrhea syndrome, but not acromegalic symptoms. Although the two compartments were united through a small dural perforation, immunohistochemical studies demonstrated that GH and prolactin were separately secreted from the intrasellar and extrasellar components, respectively. Somatotropic adenomas frequently produce prolactin, but it is unusual for these horm…

A Giant Maxillary Mucocele Presenting Left Cheek Swelling.

A paranasal sinus mucocele is an epithelial-lined, mucus-containing sac that completely fills the sinus and forms an expandable cystic structure. It most commonly affects the frontal and ethmoidal sinuses, and rarely the maxillary and sphenoid sinuses. Orbital displacement or external disfigurement resulting from the expansion of the frontal or ethmoid sinuses is common; however, facial asymmetry caused by maxillary bone remodeling is rare. We describe a case of large maxillary sinus mucocele that destroyed the maxillary sinus bony wall, resulting in notable left cheek swelling and disfigureme

A case of presumed monostotic sphenoid fibrous dysplasia and abducens nerve palsy.

Monostotic fibrous dysplasia is a rare osteodystrophy that typically affects adolescents, manifests later in life, and may be associated with subclinical hormonal imbalances. It can lead to serious complications, including visual compromise. We report a case of presumed monostotic sphenoid fibrous dysplasia presenting with transient abducens nerve palsy and esotropia in an otherwise healthy infant. The mechanism of transient, noncompressive abducens nerve palsy is not clearly understood but has been theorized to be secondary to localized inflammation during a cycle of florid bone remodeling th

Primary sphenoid sinus neoplasms: a report of four cases with common clinical presentation treated with transsphenoidal surgery and adjuvant therapies.

BACKGROUND: Primary neoplasms of the sphenoid sinus are a rare occurrence, accounting for approximately 1-2% of all paranasal sinus tumors. Here we report a series of four such patients. METHODS: Four patients with sphenoid sinus neoplasms were identified (1%), all treated during the year 2003. RESULTS: The four patients included two women and two men (mean age 61 years, range 44-70 years). Two patients presented with unilateral abducens cranial nerve (CN) palsies; one had trigeminal facial numbness and dizziness; another had headache, epistaxis, and partial third and fourth CN palsies.
