Deep Learning-Based Multi-Class Segmentation of the Paranasal Sinuses of Sinusitis Patients Based on Computed Tomographic Images.

Accurate paranasal sinus segmentation is essential for reducing surgical complications through surgical guidance systems. This study introduces a multiclass Convolutional Neural Network (CNN) segmentation model by comparing four 3D U-Net variations-normal, residual, dense, and residual-dense. Data normalization and training were conducted on a 40-patient test set (20 normal, 20 abnormal) using 5-fold cross-validation. The normal 3D U-Net demonstrated superior performance with an F1 score of 84.29% on the normal test set and 79.32% on the abnormal set, exhibiting higher true positive rates for

Cavernous Sinus Thrombosis: Efficiently Recognizing and Treating a Life-Threatening Condition.

Cavernous sinus thrombosis (CST) can develop as a result of both infectious and noninfectious conditions. Infections in the middle part of the face caused by Staphylococcus aureus are the most common cause of septic thrombosis of the cavernous sinuses. Paranasal (typically sphenoid) sinusitis, dental abscess, and, less commonly, otitis media are other antecedent sources of infection. Fever is almost always present, but a headache may not be noticeable. In almost all cases, periorbital edema, chemosis, proptosis, and a restriction of extraocular movements (particularly lateral gazing) emerge. W

Chronic osteitic rhinosinusitis as a manifestation of cystic fibrosis: A case report.

A 28-year-old male patient with a history of cystic fibrosis (CF) was referred to the University of Connecticut School of Dental Medicine for an evaluation of a cystic lesion in the right maxilla using cone-beam computed tomography (CBCT). CF is an autosomal recessive disease characterized by an abnormal production of viscous mucus, affecting the mucociliary clearance. The CBCT scan revealed a large cystic lesion in the right maxilla extending from the right maxillary second molar to the midline in the region of the right central incisor with a significant buccal expansion. Further evaluation

[Solitary intrasellar plasmocytoma revealed by a diplopia: a case report].

INTRODUCTION: Plasmocytomas rarely invade the skull base. It can be solitary or multiple. The clinical presentation mainly consists in a progressive neuropathy involving many cranial nerves, or may simply present as an optochiasmatic syndrome. OBSERVATION: The authors report a case of a female patient aged 68 years presenting with an optochiasmatic syndrome with an anosmia and paralysis of the right sixth (VI) cranial nerve progressing over two years prior to her hospitalisation. Cerebral MRI showed a voluminous intrasellar lesion, isointense on T1 and hyperintense on T2, enhancing intensely

Pneumosinus dilatans of the maxillary sinus: a case report and review of the literature.

Pneumosinus dilatans (PSD) is an abnormal dilatation of one or more of the paranasal sinuses and must be included in the differential diagnosis of expansile lesions of the sinonasal tract. The frontal and sphenoid sinuses are the most frequently involved sites. Herein, we present the case of a 30-year-old woman with a 6-year history of right nasal obstruction and pain in the right maxillary region, elicited by rapid changes in atmospheric pressure. Computed tomography (CT) detailed an abnormal expansion of the right maxillary sinus resulting from a process of hyperpneumatization with no eviden

[Non-invasive fungal sinusitis: allergic fungal sinusitis and sinusal mycetomas].

We review non-invasive fungal sinusitis (FS) cases treated at our Unit following the recent SF classification, based on physiopathology, treatment and prognosis. We report 7 FS cases treated during 2 years and followed a minimum of 24 months. They are two allergic FS, one of them related to an allergic bronchopulmonary aspergillosis, and five mycetomas, two of them without a sinusal foreign body, sphenoidal and maxillary respectively, and three caused by a maxillary foreign body. All were treated by endoscopic sinus surgery (ESS). The two allergic cases were also treated with systemic corticoi

Orbital pneumosinus dilatans.

Pneumosinus dilatans consists of an abnormal dilatation of the paranasal air sinuses which contain air only and are lined by normal epithelium. The condition is most commonly observed when it affects the sphenoid sinus as a response to a local meningioma of the tuberculum sellae or planum sphenoidale. Pneumosinus dilatans affecting the walls of the paranasal sinuses which form the boundaries of the orbit is less common; six patients reported here presented with either unilateral or bilateral exophthalmos. The frontal sinus and ethmoid cells were affected unilaterally in four patients and the m

Imaging of mucormycosis skull base osteomyelitis.

Skull base osteomyelitis (SBO) is typically bacterial in origin and caused by Pseudomonas, although the fungus Aspergillus has also rarely been implicated. SBO generally arises from ear infections and infrequently complicates sinonasal infection. Rhinocerebral Mucor infection is characteristically an acute, fulminant, and deadly infection also affecting the orbits and deep face and is associated with intracranial complications. Bony involvement is uncommon because of the angioinvasive nature of the fungus. More recently, chronic invasive Mucor sinusitis has been described. We report the unusua

A Rare Case of Metastases to Paranasal Sinus From Colonic Adenocarcinoma.

Metastases of malignant tumors to the nasal cavity and paranasal sinuses are very rare. Metastases to these locations are usually solitary and produce similar symptoms to those of a primary sinonasal tumor. Pain, nasal obstruction, and epistaxis are the most common symptoms. Although any malignancy could potentially lead to metastasis to the paranasal sinuses, colo-rectal malignancy metastasizes to this site is rare. We report a case of metastatic adenocarcinoma of colorectal origin to the paranasal sinuses in a 55-year-old female who was initially diagnosed with adenocarcinoma of the colon wi

[Importance of computer tomography in preoperative diagnostics of polyposis nasi].

Computed tomography (CT) is a very important diagnostic tool prior to endoscopic nasal or paranasal sinus surgery. However, it is frequently notice during endonsal surgery of the paranasal sinuses, that the intraoperative results do not completely agree with the radiological ones. The objective of the present study was to compare clinical and operative findings with those of CT investigations. We collected 200 cases treated during the past 2 years and studied their CT reports. Paranasal sinuses were separated into six regions, and the pathological changes due to sinusitis found by CT were clas

Functional endoscopic sinus surgery in chronic sinusitis--a series of 237 consecutively operated patients.

From 1989 to 1999, 237 consecutive patients with chronic sinusitis and/or nasal polyposis entered a prospective study on the effect of functional endoscopic sinus surgery (FES). Nasal stenosis associated with massive nasal polyposis was the most frequent problem found in 61% of the patients. The rest had long-lasting symptoms of chronic sinusitis. Duration of symptoms averaged 9.3 years. Most frequent symptoms preoperatively were: nasal stenosis, chronic secretion, anosmia, frontal pain, headache and maxillary pain. All patients had the operation performed under general anaesthesia. 86% of the

Volumetric study in the development of paranasal sinuses by CT imaging in Asian: a pilot study.

BACKGROUND AND OBJECTIVES: The volume of the air cavities in the paranasal sinuses is not only the simplest, but also the most important index for paranasal sinus evaluation. However, few volumetric studies have been performed in all age groups. The purpose of the current study was to outline the normal development of paranasal sinuses in all age groups, and to determine normal adult volumetric values by means of computed tomographic (CT) scan of paranasal sinus using volumetric procedures. MATERIALS AND METHODS: A prospective volumetric CT study was conducted with 260 patients (520 sides) <2
