Results from studies evaluating needle-knife papillotomy are conflicting. The aim of this prospective trial was to assess the safety and efficacy of needle-knife papillotomy and compare the results with those of standard endoscopic sphincterotomy. The hypertrophic aspect of tissue after endoscopic papillotomy may suggest the diagnosis of a small periampullary tumor. In our experience of 55 periampullary carcinomas, the diagnosis was confirmed histologically in 50% of the cases before endoscopic papillotomy and in 100% of the cases when the biopsies were performed immediately after papillotomy. With this in mind, biopsies were performed in 22 cases of benign biliary tract disease after endoscopic papillotomy. Biopsies within the first 48 hours after endoscopic papillotomy showed cellular atypism.

Between January 1975 and December 1979, 71 patients over the age of 70 underwent attempted duodenoscopic sphincterotomy for stones in the common bile duct. Fifteen patients still had gall bladders in situ. Sphincterotomy was possible in 69 of the patients and in 65 of these duct clearance was achieved, giving an overall success rate of 92%. Failure to achieve sphincterotomy in two cases was due to substantial peripapillary diverticula. Duct clearance failed in four patients, mostly due to the size of the retained stones. A comparison of the outcome of the treatment of fissure in ano by a lateral subcutaneous sphincterotomy (LSS) and by an anal dilatation (AD) has been made retrospectively through a questionnaire sent to patients who were operated upon between three months and five years beforehand.
