Assessment of spinal cord motion as a new diagnostic MRI-parameter in cervical spinal canal stenosis: study protocol on a prospective longitudinal trial.

BACKGROUND: Increased spinal cord motion has been proven to be a relevant finding within spinal canal stenosis disclosed by phase-contrast MRI (PC-MRI). Adapted PC-MRI is a suitable and reliable method within the well deliberated setting. As the decision between conservative and operative treatment can be challenging in some cases, further diagnostic marker would facilitate the diagnostic process. We hypothesize that increased spinal cord motion will correlate to clinical course and functional impairment and will contribute as a new diagnostic marker. METHODS: A monocentric, prospective longi…

Impact of spinal imbalance and BMI on lumbar spinal canal stenosis determined by a diagnostic support tool: cohort study in community‑living people.

INTRODUCTION: The Japanese Society for Spine Surgery and Related Research (JSSR) developed a diagnostic support tool (score range −2 to 16) for lumbar spinal canal stenosis (LSS). It is well known that patients with LSS have a forward-bending posture while walking. We could find only one study which examined sagittal alignment of LSS patients. The objectives of this study were to screen community-living people with LSS, and to investigate the association between LSS and spinal sagittal alignment, range of motion (ROM), and other spinal or physical factors. MATERIALS AND METHODS: We enrolled 2…

Early outcomes of 270-degree spinal canal decompression by using TESSYS-ISEE technique in patients with lumbar spinal stenosis combined with disk herniation.

PURPOSE: Traditional open approach is an efficient way to treat lumbar spinal stenosis (SS) combined with disk herniation (DH); however, risk factors such as advanced age, osteoporosis etc. are associated with the complications after the surgery. This study aims to analyze the early clinical and radiological outcomes of treatment on SS&DH by using newly developed minimal invasive TESSYS-ISEE technique. METHODS: Patients with limp and unilateral lower limb radiculopathy underwent minimal invasive surgery by using TESSYS-ISEE technique. The visual analogue scale score (VAS) and Oswestry Disabil

Two Cases of Lumbar Spinal Canal Stenosis with Dorsal Meningovertebral Ligaments: Potential Risk of Dural Laceration.

Dural laceration is a critical complication in full endoscopic spinal surgery. We need to recognize precise surgical anatomy to avoid it. Recently identified dorsal meningovertebral ligaments, connecting the dorsal spinal dura mater to the ligamentum flavum, may contribute to this risk, but their role remains unclear. We present 2 cases of lumbar spinal canal stenosis treated with full endoscopic laminectomy. In both cases, dorsal meningovertebral ligaments anchored the dura to the ligamentum flavum and were visible through high-resolution endoscopy. Attempts to remove ligamentum flavum fragme…

Full-Endoscopic Spinal Canal Decompression After Direct Midline Splitting of the Spinous Process: A Technical Note.

For lumbar spinal canal stenosis, endoscopic spine surgery typically employs a unilateral approach. While this approach has the advantage of early access to the lamina, it risks damage to the facet joint on the entry side. Additionally, decompression of the ipsilateral lateral recess can be challenging, sometimes resulting in inadequate decompression laterally, leading to incomplete symptom relief. To address these issues, a midline approach after midline splitting of the spinous process has been developed. However, this technique often uses a relatively large 16mm cylindrical retractor, resul…

Unilateral Approaches for Posterior Spinal Canal Decompression in Cervical Spondylotic Myelopathy-An Evaluation of Conceptual Feasibility.

BACKGROUND: Patients with cervical spondylotic myelopathy (CSM) can be treated with posterior approaches for spinal canal decompression. OBJECTIVE: We compared the patients' outcome after 2 different unilateral and a bilateral posterior approach for decompression to elucidate feasibility and potential procedure-related differences. METHODS: Medical records of 98 patients with CSM undergoing posterior decompression between 2012 and 2018 were assessed. Patients were divided into 3 groups: (1) unilateral interlaminar fenestration with over-the-top "undercutting" (laminotomy) for compression lim…

Lumbar Spinal Canal Stenosis from the Perspective of Locomotive Syndrome and Metabolic Syndrome: A Narrative Review.

Patients with lumbar spinal canal stenosis (LSS) have impaired activities of daily living because of pain or motor paralysis, but no effective preventive treatment is currently available. The number of patients with LSS is predicted to continually increase as the average age of the global population increases. To provide a conceptual framework for improving healthy life expectancy, the Japanese Orthopaedic Association introduced the concept of locomotive syndrome, to which LSS is related. Ours and other studies have shown that LSS exacerbates locomotive syndrome and that surgical treatment is …

Diagnostic Importance of Axial Loaded Magnetic Resonance Imaging in Patients with Suspected Lumbar Spinal Canal Stenosis.

OBJECTIVE AND BACKGROUND: To study the efficacy of lumbar (AL) magnetic resonance imaging (MRI) in patients with suspected lumbar spinal stenosis (LSS), with and without AL compression. Supine MRI is used in the assessment of patients with LSS. However, MRI findings may poorly correlate with neurologic findings because of the morphologic changes of the lumbar spinal canal between upright standing and supine positions. In patients without significant stenosis in routine lumbar MRI, by applying AL, MRI can show significant LSS. METHODS: This study included 103 consecutive patients (188 disc lev…

Percutaneous kyphoplasty for the treatment of very severe osteoporotic vertebral compression fractures with spinal canal compromise.

BACKGROUND: Very severe osteoporotic vertebral compression fractures (vsOVCFs) are osteoporotic vertebral compression fractures with vertebral body collapse to less than one third of their original height. Few data are available about the use of percutaneous kyphoplasty (PKP) in treating vsOVCFs with spinal canal compromise. The aim of this study was to evaluate the safety and efficacy of percutaneous kyphoplasty (PKP) for the treatment of vsOVCFs with spinal canal compromise. METHODS: Thirty-five patients who suffered vsOVCFs with spinal canal compromise but without neurological deficits wer…

Imaging symptomatic bone morphogenetic protein-2-induced heterotopic bone formation within the spinal canal: case report.

Heterotopic bone formation within the spinal canal is a known complication of bone morphogenetic protein-2 (BMP-2) and presents a clinical and surgical challenge. Imaging modalities are routinely used for operative planning in this setting. Here, the authors present the case of a 59-year-old woman with cauda equina syndrome following intraoperative BMP-2 administration. Plain film myelographic studies showed a region of severe stenosis that was underappreciated on CT myelography due to a heterotopic bony lesion mimicking the dorsal aspect of a circumferentially patent thecal sac. When evaluati…

Intrathecal volume changes in lumbar spinal canal stenosis following extension and flexion: An experimental cadaver study.

BACKGROUND: The spinal canal stenosis is a common disease in elderly. The thecal sac narrowing is considered as the anatomical cause for the disease. There is evidence that the anatomical proportions of the lumbar spinal canal are influenced by postural changes. The liquor volume shift during these postural changes is a valuable parameter to estimate the dynamic qualities of this disease. OBJECTIVE: The aim of this human cadaver study was the determination of intrathecal fluid volume changes during the lumbar flexion and the extension. A special measuring device was designed and built for the…
