Catheter-related urinary tract infection in patients suffering from spinal cord injuries.

Urinary tract infection is commoner in patients with spinal cord injuries because of incomplete bladder emptying and the use of catheters that can result in the introduction of bacteria into the bladder. 145 patients suffering from spinal cord injuries, admitted to the Institute for physical medicine and rehabilitation, Centre for paraplegia of the Clinical Centre of the University of Sarajevo, were included. The patients were divided in three groups according to the method of bladder drainage: Group A (n=61) consisted of patients on clean intermittent catheterization; Group B (n=54) consisted…

Urolithiasis in patients with spinal cord injuries: risk factors, management, and outcomes.

PURPOSE OF REVIEW: Despite major advances in the urological care of spinal cord injury patients, the incidence of urolithiasis has not changed. Although the incidence of kidneys lost to staghorn calculus disease has decreased considerably, stone disease is still a significant cause of morbidity and concern. The purpose of this review is to summarize the risk factors for the development of stone diseases in the spinal cord injury patient population and present the outcomes of current endourologic treatment modalities. RECENT FINDINGS: Recurrent urinary tract infections, indwelling catheters, v

Pitfall in insertion of suprapubic catheter in patients with spinal cord injuries.

OBJECTIVE: To report an unusual presentation of a misplaced suprapubic catheter (SPC) in a spinal cord injury (SCI) patient. DESIGN: A case report of a SCI patient in whom a SPC was 'partially misplaced' in an emergency. SETTING: London Spinal Injuries Unit, Stanmore, UK. SUBJECT: A 33-year-old man who sustained a C5 SCI in a road traffic accident 6 months ago. He had an indwelling urethral catheter, which blocked off and repeated attempts to reinsert another one per urethra were unsuccessful. MAIN OUTCOME MEASURE: A SPC was inserted in an emergency at the bedside, as he developed autonomi

[Birth injuries and lesions of the spinal cord in the perinatal period].

The paper presents a clinical and pathological analysis of 40 deliveries, autopsy findings in fetuses and newborns with combined birth injury of the brain and spinal cord, perinatal disease entities associated with intrapartum spinal lesions and disease entities which entail secondary spinal lesions. A subgroup of newborns with intraventricular hemorrhage showed at autopsy diffuse subarachnoidal hemorrhage of the cervical and thoracic spinal segments as a function of its extension into the cerebrospinal fluid system. Guidelines for pathological diagnosis in the pediatric practice are offered.

Is testicular endocrine function abnormal in young men with spinal cord injuries?

Plasma concentrations of FSH, LH, testosterone and prolactin have been studied in seventeen men, aged 19 to 38 years, with traumatic paraplegia. Plasma testosterone, FSH and LH values were normal in all patients. The median LH concentration in patients was significantly higher (P less than 0.05) than in controls but there was no difference for FSH or testosterone. Plasma prolactin was high in five patients and the median prolactin concentration in patients was significantly greater (P less than 0.001) than in controls. It is concluded that there is no evidence of primary testicular failure in

A review of long-bone fractures in patients with spinal cord injuries.

In a retrospective investigation of 38 long-bone fractures in 30 spinal cord injured patients (mean follow-up, 22 months), fractures were classified acute (occurring at the time of spinal cord injury) or pathologic (occurring in the chronically injured patient). The methods of treatment and complications were recorded and evaluated. The incidence of long-bone fractures was 6.7%. Complication rates were high, ranging from 20% to 40%, with open or closed treatment of extremity fractures. Strict adherence to established operative fracture management principles in paraplegic patients irrespective

The risk of delayed spinal cord injury in pediatric spinal deformity surgery.

Delayed spinal cord injury (SCI) hours or days after surgery, with uneventful monitoring and initial normal postoperative neurological examination, is a rare complication. Based on anecdotal evidence, the risk of delayed spinal cord injury might be higher than previously assumed. Therefore the aim of this study was to determine the risk of delayed SCI after pediatric spinal deformity surgery between 2013-2019 in the Netherlands. The total number of pediatric spinal deformity surgeries performed for scoliosis or kyphosis between 2013-2019 was obtained from the Dutch National Registration of Hos

A multilevel noncontiguous spinal fracture with cervical and thoracic spinal cord injury.

INTRODUCTION AND IMPORTANCE: A multi-level non-contiguous spinal fracture (MNSF) caused by a high-energy impact is a type of complex traumatic injury that is been frequently initially missed, and resulting in delayed diagnosis which adversely affects can result in spinal deformity and neurological deficit. This report describes the operative management of a patient with MNSF with spinal cord injury involving the cervical and thoracic vertebrae by cervical orthosis and posterior thoracic decompression and fusion. CASE PRESENTATION: An 18-year-old male presented with extensive neck pain and par

The Functional Role of Spinal Interneurons Following Traumatic Spinal Cord Injury.

Traumatic spinal cord injury (SCI) impedes signal transmission by disrupting both the local neurons and their surrounding synaptic connections. Although the majority of SCI patients retain spared neural tissue at the injury site, they predominantly suffer from complete autonomic and sensorimotor dysfunction. While there have been significant advances in the characterization of the spared neural tissue following SCI, the functional role of injury-induced interneuronal plasticity remains elusive. In healthy individuals, spinal interneurons are responsible for relaying signals to coordinate both

Risk factors for spinal cord injury during surgery for spinal deformity.

BACKGROUND: Spinal cord monitoring is now considered standard care during surgery for spinal deformity. Combined somatosensory and motor evoked potential monitoring allows the detection of early spinal cord dysfunction in most patients. The purpose of the current study was to identify clinical factors that increase the risk of intraoperative electrophysical changes and to provide management recommendations. METHODS: The records of 162 consecutive patients who underwent surgery for the treatment of spinal deformity at a tertiary referral center were reviewed. Electrophysical monitoring of thes
