Where Sepsis Meets Haemorrhage: A Case Report of Spontaneous Splenic Rupture Following Perforated Diverticulitis.

Atraumatic splenic rupture is an extremely rare cause of deterioration in the context of concurrent abdominal pathology. This case report describes rapid deterioration and shock in a mid-50s-year-old man with acute complicated sigmoid diverticulitis 3 days into admission without history of abdominal trauma. This was associated with a rapid haemoglobin (Hb) drop from 119 to 37 g/L, which necessitated activation of the massive transfusion protocol. An atraumatic splenic rupture was subsequently diagnosed on urgent exploratory laparotomy. The patient underwent splenectomy followed by a Hartmann's…

Spontaneous rupture of a mycotic splenic artery pseudoaneurysm secondary to histoplasmosis: a case report.

BACKGROUND: A splenic artery pseudoaneurysm is a rare pathology that occurs mainly secondary to pancreatitis, abdominal trauma, peptic ulcers, pancreatic and gastric cancers, and infections. It is best diagnosed using computed tomography angiography and typically treated using endovascular embolization and, in some cases, open or laparoscopic surgery. In this report, we present a case of a ruptured mycotic splenic artery pseudoaneurysm containing Histoplasma capsulatum, which to our knowledge is the first case to report a mycotic splenic artery pseudoaneurysm of this nature. CASE PRESENTATION

Atraumatic Splenic Rupture as an Unusual Presentation of Hairy Cell Leukemia: A Case Report.

Hairy cell leukemia (HCL) represents a rare B-cell malignancy with 2% of all leukemias and should be differentiated from HCL-like conditions, including HCL-variant (HCL-V) and splenic diffuse red pulp lymphoma (SDRPL). HCL gets its name from the short, thin projections that look like hair on its cells. It is associated with a specific immunophenotypic profile, cytopenia, and splenomegaly. Spontaneous splenic rupture can be a symptom of hematological malignancy such as HCL and is a life-threatening acute emergency. Here, we present a case of a 37-year-old man who presented to the hospital with …

COVID-19 With an Initial Presentation of Intraperitoneal Hemorrhage Secondary to Spontaneous Splenic Rupture.

The WHO declared coronavirus disease 2019 (COVID-19) a global pandemic in early 2020. As the pandemic has continued to evolve over a period of several months, many cases of unusual presentations are now emerging, which pose a greater challenge for physicians in terms of quickly identifying COVID-19 patients based on initial signs and symptoms. In this report, we present one such unusual presentation in a patient with sudden intraperitoneal hemorrhage and spontaneous splenic rupture with COVID-19 as the likely etiology and contributing factor. The patient was a 75-year-old Caucasian woman who p…

Splenic artery aneurysm rupture during pregnancy: A case report of maternal and fetal survival.

INTRODUCTION: Pregnancy has been demonstrated as a significant risk factor of splenic artery aneurysm (SAA) formation and rupture. However, prompt diagnosis of SAA rupture in a pregnant patient showing acute abdomen has been practically challenging in light of its rarity and vague initial presentation. PRESENTATION OF CASE: A 40-year-old woman (gravida 1, para 0) at 35 weeks' gestation presented to the emergency department with upper abdominal pain and nausea. Because of fetal dysfunction, emergency caesarian section was performed by a Pfannenstiel incision. Following delivery, 400 g of hemor

Spontaneous Splenic Rupture Complicating Severe P. falciparum Infection: A Case Report and Literature Review.

Spontaneous rupture of the spleen in malarial infection or malarial splenic rupture (MSR) is a rare but life-threatening condition complicating severe malarial infection in tropics and subtropics, and hence it deserves special attention. A high index of clinical suspicion is warranted for the early diagnosis as delayed or missed diagnosis can be potentially fatal. We report on a 32-year-old male who was diagnosed with severe Plasmodium falciparum infection and presented with an acute abdomen due to spontaneous splenic rupture. He was managed conservatively and had a successful outcome. The epi…

Management of Traumatic Splenic Rupture in Adults: A Single Center's Experience in Mainland China.

BACKGROUND/AIMS: We evaluated the effectiveness of various therapies to treat the traumatic splenic rupture by using data from our single center. METHODOLOGY: From July 2008 to Jan 2014, 125 patients who were diagnosis spleen rupture without other organ injury were included into our present study. We divided these patients into two groups according to the managements of the splenic rupture: the operative management (OM) group (48 patients) and the non-operative management (NOM) group (77 patients). We compared the baseline characteristics and long-term outcomes of two groups. the OM group was…

Non-operative management of malarial splenic rupture: the Khartoum experience and an international review.

Malarial splenic rupture (MSR) occurs in a subset of patients and can be an acute surgical emergency. MSR is a well-known entity for more than 100 years, yet there are no well-structured studies in the literature that systematically evaluate this complication. While it has become increasingly recognized that splenic salvage can be vital to the long term immunity and health of these patients, there are few data to guide a safe approach to non-operative management of these patients. Current knowledge of spontaneous rupture of the spleen has been gained largely though reported cases. We present 2…

Spontaneous splenic rupture in atypical (Philadelphia chromosome negative) chronic myeloid leukaemia following blastic crisis.

Philadelphia chromosome negative and bcr/abl negative chronic myeloid leukaemia (CML) is an uncommon atypical CML. We describe a patient with this disorder who experienced an acute blastic transformation that resulted in rapid splenic enlargement and subsequent atraumatic splenic rupture. Clinically, spontaneous splenic rupture may be a difficult diagnosis to make and this case highlights the importance of considering atraumatic splenic rupture as a cause for unexplained abdominal pain in a patient with a haematological malignancy.

[Autotransfusion: its use in a case of spontaneous splenic rupture in non-Hodgkin's lymphoma].

The article refers to autotransfusion experience in a patient affected by lymphoma (not Hodgkin type), who reached surgery for haemoperitoneum caused by the spontaneous rupture of the spleen. Considering the pathology, this method brought unpredictable variations in the hemochrome thus leading to a deep reflection on the limitations of the use of autotransfusion.

Uncommon Case of Uterine Rupture Associated With Splenic Artery Aneurysm and Rupture During Pregnancy: A Clinical Report and Review of the Literature.

We present a case of concomitant splenic artery aneurysmal rupture with uterine rupture in a 32 year-old pregnant woman at 34 weeks of gestation with a history of one previous Cesarean section. The patient presented to the Emergency Department (ED) due to a sudden abdominal pain, which resolved spontaneously. In the ED, she displayed tachycardia, sweating, and lethargy, with no detectable fetal heart rate, raising suspicion of uterine rupture. Surgical exploration revealed significant intra-abdominal hemorrhage from a prior uterine incision, resulting in intrauterine fetal demise (IUFD) and pl

[Splenic arterial aneurysm rupturing into the splenic vein. A rare cause of acute fortal hypertension in the post-partum period (author's transl)].

Following the sudden onset of a clinical syndrome of acute portal hypertension in a young woman in the course of childbirth, angiography revealed a voluminous aneurysm of the splenic artery rupture into the splenic vein. Surgical treatment was successful. The authors review the main characteristics of splenic arterio-venous fistulae and discuss the physiopathological problems of portal hypertension associated with such lesions.
