Measurement of Spleen Stiffness by Shear-Wave Elastography for Prediction of Splenomegaly Etiology.

OBJECTIVE: The aim of this study was to evaluate the reproducibility of measurement of spleen stiffness at the time of the initial detection of splenomegaly, whether it is found incidentally or not, in determining the etiology of splenomegaly. METHODS: The pathologies that brought about the diffuse splenomegaly were evaluated in 3 main groups as follows: hepatoportal, myeloproliferative, and infectious causes. In addition, 17 healthy control patients were recruited. All patients were examined with acoustic radiation force impulse imaging with VTQ. RESULTS: The difference between the splenic …

The Best Single Measurement for Assessing Splenomegaly in Patients with Cirrhotic Liver Morphology.

RATIONALE AND OBJECTIVES: There is little agreement within the radiology literature as to the best single measurement for assessing splenomegaly. In this study, we evaluate the correlation of multiple unidirectional measurements of the spleen with splenic volume in patients with cirrhotic liver morphology on computed tomography (CT). MATERIALS AND METHODS: Splenic volume was retrospectively calculated from CT examinations of 179 adult patients, 47 of whom were approved as renal donors, and 132 of whom were referred for various other indications, and were found to have cirrhotic liver morpholo…

Safety and effectiveness of total splenic vessel ligations in paediatric patients with splenomegaly.

BACKGROUNDS: Splenomegaly may contribute to hypersplenism and can result in thrombocytopenia. Many approaches are used to treat splenomegaly; however, the current management of splenomegaly has intrinsic limitations or disadvantages. Now, we initiate a new approach, that of total splenic vessel (artery and vein) ligations (TSVLs) in paediatric patients with splenomegaly. The purpose of our study is to evaluate the results obtained with TVSLs procedure for paediatric patients. PATIENTS AND METHODS: Seventeen paediatric patients with splenomegaly were screened for enrolment into this retrospect…

In vivo study on splenomegaly inhibition by genistein in Plasmodium berghei-infected mice.

Spleen plays an important role in removing old and damaged red blood cells and malaria-infected erythrocytes. When malaria parasites invade the spleen and induce splenomegaly, splenic function tends to be impaired. Thus, the inhibition of splenomegaly is strongly required to protect the spleen. In this study, malaria-induced splenomegaly is inhibited by injecting genistein into a Plasmodium berghei-infected ICR mouse. To explain this phenomenon, the effect of genistein in spleen and liver of malaria-infected mice was evaluated by histological examination. Malaria parasites disrupted splenic ar…

"The Great Mimicker": An Unusual Etiology of Cytopenia, Diffuse Lymphadenopathy, and Massive Splenomegaly.

Sarcoidosis is an idiopathic multisystem disease characterized by the formation of noncaseating granulomas. It frequently presents with pulmonary infiltrates and bilateral hilar and mediastinal lymphadenopathy. Splenic involvement is common, but massive splenomegaly is a rare occurrence. Sarcoidosis is known as "the great mimicker" (or "the great imitator") since it exhibits a myriad of symptoms, mimicking other inflammatory, infectious, and neoplastic conditions, including lymphoma. Herein, we report the case of a 44-year-old male patient who was found to have bicytopenia, hypercalcemia, diff…

Treatment of suspected hyper-reactive malarial splenomegaly (HMS) in pregnancy with mefloquine.

Malaria infections in pregnancy are associated with adverse outcomes for both mother and child. There are few data on hyper-reactive malarial splenomegaly, an aberrant immunological response to chronic or recurrent malaria in pregnancy. This retrospective assessment reviewed the impact of mefloquine treatment on pregnant women with suspected hyper-reactive malarial splenomegaly in an area of low malaria transmission in the 1990s, showing significant reductions in spleen size and anemia and anti-malarial antibody titers without any notable negative effect on treated women or their newborns.

Hyper-reactive malarial splenomegaly (HMS) in a patient with β thalassaemia syndrome.

This report describes a case of hyper-reactive malarial splenomegaly in a patient with a thalassaemia syndrome. Increased haemoglobin A2 is valuable for the diagnosis of common forms of β-thalassemia, while haemoglobin F (HbF) helps in diagnosis of the rarer δβ- forms. Thalassemia is characterised by splenomegaly and is common in malaria endemic areas. Hyper-reactive malarial splenomegaly is also a common cause of massive splenomegaly in malaria endemic areas. Splenic enlargement regresses with prolonged antimalarial therapy.

The learning curve for laparoscopic splenectomy for massive splenomegaly: a single surgeon's experience.

BACKGROUND: Laparoscopic splenectomy (LS) for massive splenomegaly is more technically challenging than for a normal-sized spleen. The purpose of this study was to determine the effect of operative experience on perioperative outcomes of LS for massive splenomegaly. METHODS: Between January 2008 and December 2010, 36 consecutive patients who were diagnosed with massive splenomegaly underwent LS in our department. The perioperative outcomes were evaluated for evidence of a learning curve effect. Patients were divided into three groups (1, 2, and 3) of 12 consecutive patients, and outcomes of e…

Laparoscopic and open splenectomy for splenomegaly secondary to liver cirrhosis: an evaluation of immunity.

OBJECTIVE: We sought to investigate the perioperative inflammatory response and immunological function of patients with portal hypertension-induced splenomegaly who underwent laparoscopic (LS) or open splenectomy (OS). METHODS: This prospective study investigated a total of 34 patients with splenomegaly due to portal hypertension who underwent either LS (n = 18) or OS (n = 16) between May 2009 and September 2010. Peripheral venous blood samples were taken from these patients prior to surgery and on postoperative days (POD) 1, 3, and 7. The perioperative clinical outcomes and immunological fun…

Sarcoidosis presenting with massive splenomegaly in a child with a history of iridocyclitis and sensorineural deafness.

Childhood sarcoidosis is a rare multisystemic disorder that can have variable clinical presentations. A triad of skin, eye, and joint involvement is common in children younger than 5 years; however, pulmonary disease is more common in older children, similar to adults. The authors report the case of a 10-year-old girl who presented solely with massive splenomegaly. Her history was significant for iridocyclitis and unilateral sensorineural deafness at 6 and 7 years of age, respectively. A gallium scan showed diffuse splenic uptake, and the pathology of the spleen was consistent with a noncaseat…

A rare presentation of visceral leishmaniasis without fever or splenomegaly in an elderly person.

Visceral leishmaniasis (kala azar) is characterized by fever and splenomegaly in more than 80% of cases. We report a case of kala azar in an elderly person with severe anemia but no other typical features. This highlights the importance of being aware of rare presentations to avoid missing the diagnosis of this potentially fatal, but fully treatable, condition.

[Prophylactic splenectomy to prevent complications of splenomegaly in children with sickle cell anemia?].

OBJECTIVE: To assess the effects of splenectomy in children with sickle cell anemia and to propose a therapeutic approach to splenomegaly in sickle cell anemia. MATERIAL AND METHOD: This retrospective study, conducted in the pediatric surgery department of the Tokoin Teaching Hospital in Lomé, included 8 children followed for sickle cell anemia (hetero- and homozygous) and who were admitted from January 1987 through December 2004 for splenic rupture or referred for prophylactic splenectomy. RESULTS: The patients' mean age at splenectomy was 9 years 6 months. Five were homozygous and three he…
