Automatic temporal analysis of speech: A quick and objective pipeline for the assessment of overt stuttering.

Fluency disorders, such as developmental stuttering, have been characterized by behavior such as blocks, repetitions, and prolongations in speech. Accurate measurement of overt stuttering behavior can aid in diagnostic evaluation and the determination of optimal treatment for this disorder. This study proposes a method - Automatic Temporal Analysis of Speech (ATAS) - for the assessment of speech fluency based on the detection and quantification of discrete pauses and vocal events. Our ATAS metrics include speech rate, total pause time, pause count, mean pause duration, mean vocal duration, pau…

Speaking to a metronome reduces kinematic variability in typical speakers and people who stutter.

BACKGROUND: Several studies indicate that people who stutter show greater variability in speech movements than people who do not stutter, even when the speech produced is perceptibly fluent. Speaking to the beat of a metronome reliably increases fluency in people who stutter, regardless of the severity of stuttering. OBJECTIVES: Here, we aimed to test whether metronome-timed speech reduces articulatory variability. METHOD: We analysed vocal tract MRI data from 24 people who stutter and 16 controls. Participants repeated sentences with and without a metronome. Midsagittal images of the vocal …

T-PALS framework to assess children who stutter with coexisting disorders: A tutorial.

The purpose of this paper is to present a tutorial on a diagnostic framework developed to assess children who stutter and exhibit co-existing disorders. While we have guidelines for treating these children, there are no specific guidelines for assessing them. We provide a rationale for the development of T-PALS with support from the literature. The T-PALS framework assesses 5 foundational key elements for the child: Temperament (T), Pragmatics (P), Articulation/phonology (A), Language (L), and Stuttering (S). Both qualitative and quantitative measures are used within each dimension. This frame…

Multimodal explainable AI predicts upcoming speech behavior in adults who stutter.

A key goal of cognitive neuroscience is to better understand how dynamic brain activity relates to behavior. Such dynamics, in terms of spatial and temporal patterns of brain activity, are directly measured with neurophysiological methods such as EEG, but can also be indirectly expressed by the body. Autonomic nervous system activity is the best-known example, but, muscles in the eyes and face can also index brain activity. Mostly parallel lines of artificial intelligence research show that EEG and facial muscles both encode information about emotion, pain, attention, and social interactions,

Treatment for School-Age Children Who Stutter: A Systematic Review of Japanese Literature.

PURPOSE: This systematic review identified and synthesized published research articles, written in Japanese, on the clinical effectiveness of a broad range of nonpharmacological interventions for school-age children who stutter. METHOD: A systematic review of Japanese literature published between January 1, 1980, and July 7, 2020, reporting interventions for school-age children who stutter, was carried out through a search of two databases (CiNii Article database and Japan Medical Abstract Society database) using the key words "stuttering" and "school-age" or "child" or "primary school studen…

Effects of the presence of a dog on the psychic content expression of a stuttering person: case report.

Stuttering has negative impacts on an individual's quality of life and is associated with higher risk of development of social and psychological problems. From this perspective, despite the diversity of etiological hypotheses for and treatments of stuttering, the interface between psychology and speech therapy in the approach to the biopsychic aspects involved in this clinical scenario stands out. Recent research indicates that the presence, specifically of dogs, can assist patients in symbolic elaboration of psychic content. From this perspective, the aim of this study is evaluate the effects…

Influence of gestures on the intelligibility and comprehensibility of utterances with stuttering events in individuals with Down syndrome.

INTRODUCTION: Individuals with Down syndrome (DS) often stutter, which can affect their speech intelligibility. Previous research has shown that manual signs can enhance speech intelligibility and comprehensibility. It remains unclear to what extent spontaneous hand gestures, such as iconic and beat gestures, may enhance intelligibility and/or comprehensibility during utterances with stuttering events. METHODS: Eleven individuals with DS who stutter provided video-recorded speech samples. From these samples, 60 utterances containing a stuttering event were selected. In half of them, the stutt…

Stuttering and Word-Finding Difficulties in a Patient With COVID-19 Presenting to the Emergency Department.

Coronavirus disease 2019 (COVID-19) was designated as a global pandemic by the World Health Organization (WHO) on March 11, 2020. The Cochrane Database of Systematic Reviews documents that COVID-19 has a wide range of common symptoms, which have made it difficult to characterize the disease. To date, the neurological symptoms of stuttering and word-finding difficulties have not been reported in confirmed COVID-19 cases. This case report describes the clinical course of a 53-year-old female that presented to the emergency department (ED) twice with varying symptoms consistent with COVID-19. At …

Effects of Social Stress on Autonomic, Behavioral, and Acoustic Parameters in Adults Who Stutter.

Purpose The purpose of this study was to assess changes in autonomic, behavioral, and acoustic measures in response to social stress in adults who stutter (AWS) compared to adults who do not stutter (ANS). Method Participants completed the State-Trait Anxiety Inventory ( Speilberger, Gorsuch, Luschene, Vagg, & Jacobs, 1983 ). In order to provoke social stress, participants were required to complete a modified version of the Trier Social Stress Test (TSST-M, Kirschbaum, Pirke, & Hellhammer, 1993 ), which included completing a nonword reading task and then preparing and delivering a speech to wh

Interestingly, speaking with a consistent 100-ms delay added to the auditory feedback signal-normalized PSAM in speakers who stutter, and there no longer was a between-group difference in this condition. Conclusions Combining our own data wit …

Qualitative investigation of the speech-language therapy experiences of individuals who covertly stutter.

PURPOSE: Individuals who covertly stutter have a unique experience of stuttering that involves concealing the core behaviors of stuttering (e.g., repetitions, prolongations, and blocks). From the listener's perspective, covert stuttering results in minimum typical, overt stuttering behaviors. However, from the speaker's perspective, covert stuttering often increases the cognitive and emotional impact of stuttering. This study explores the speech-language therapy experiences of individuals who covertly stutter in order to improve treatment recommendations and best practice. METHODS: This inves…

Idiopathic Normal Pressure Hydrocephalus With Stuttering: Report of Two Cases and Review of the Literature.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a disorder of aging that is characterized by enlarged cerebral ventricles, gait apraxia, dementia, and urinary incontinence. iNPH is frequently misdiagnosed, in part because the symptoms resemble other neurological disorders, and because other associated symptoms have not been fully characterized. Importantly, iNPH has not previously been associated with stuttering, and shunting has not been shown to alleviate the symptom of stuttering. CASE DESCRIPTIONS: Here, we report 2 cases of patients with iNPH presenting with stuttering tha…

Anomalous morphology in left hemisphere motor and premotor cortex of children who stutter.

Stuttering is a neurodevelopmental disorder that affects the smooth flow of speech production. Stuttering onset occurs during a dynamic period of development when children first start learning to formulate sentences. Although most children grow out of stuttering naturally, ∼1% of all children develop persistent stuttering that can lead to significant psychosocial consequences throughout one's life. To date, few studies have examined neural bases of stuttering in children who stutter, and even fewer have examined the basis for natural recovery versus persistence of stuttering. Here we report th…

Stuttering candidate genes DRD2 but not SLC6A3 is associated with developmental dyslexia in Chinese population.

BACKGROUND: Dyslexia is a polygenic developmental disorder characterized by difficulties in reading and spelling despite normal intelligence, educational backgrounds and perception. Increasing evidences indicated that dyslexia may share similar genetic mechanisms with other speech and language disorders. We proposed that stuttering candidate genes, DRD2 and SLC6A3, might be associated with dyslexia. METHODS AND RESULTS: The study was conducted in an unrelated Chinese cohort with 502 dyslexic cases and 522 healthy controls. In total, 23 Tag SNPs covering the two genes were selected for genotyp…

Clozapine-induced stuttering: a case report and analysis of similar case reports in the literature.

There is limited literature reporting clozapine-associated stuttering. In this case report, we present a case of a young male who developed stuttering with clozapine, which improved with dose reduction. Computer-assisted searches on clozapine-induced stuttering yielded 16 cases, and analysis of these case reports suggests that stuttering may be linked to seizures or movement disorders, but other putative mechanisms may be at work, which need further research.
