Sleep Management Among Patients with Substance Use Disorders.

Sleep and substance use disorders commonly co-occur. Insomnia is commonly associated with use and withdrawal from substances. Circadian rhythm abnormalities are being increasingly linked with psychoactive substance use. Other sleep disorders, such as sleep-related breathing disorder, should be considered in the differential diagnosis of insomnia, especially in those with opioid use or alcohol use disorder. Insomnia that is brief or occurs in the context of active substance use is best treated by promoting abstinence. A referral to a sleep medicine clinic should be considered for those with chr

Psychosocial Interventions for Individuals With Comorbid Psychosis and Substance Use Disorders: Systematic Review and Meta-analysis of Randomized Studies.

BACKGROUND AND HYPOTHESIS: Substance use is highly prevalent among people with schizophrenia (SCZ) and related disorders, however, there is no broad-spectrum pharmacotherapy that concurrently addresses both addiction and psychotic symptoms. Psychosocial (PS) interventions, which have yielded promising results in treating psychosis and substance dependence separately, demonstrate potential but have not been systematically evaluated when combined. STUDY DESIGN: Systematic review and random-effects meta-analyses of randomized controlled trials (RCTs) investigating PS interventions for individual

Examining the shared and unique relationships among substance use and mental disorders.

BACKGROUND: Co-morbidity among use of different substances can be explained by a shared underlying dimensional factor. What remains unknown is whether the relationship between substance use and various co-morbid mental disorders can be explained solely by the general factor or whether there remain unique contributions of specific substances. METHOD: Data were from the 2007 Australian National Survey of Mental Health and Wellbeing (NSMHWB). A unidimensional latent factor was constructed that represented general substance use. The shared and specific relationships between lifetime substance use

Z-drug abuse and dependence: clinical guideline of the Brazilian Academy of Neurology for diagnosis and management.

Benzodiazepine (BZD) receptor agonists, commonly known as Z-drugs, are non-BZD hypnotics primarily prescribed for the treatment of insomnia. Their use is recommended for no longer than four weeks to minimize the risk of adverse effects, including dependence and withdrawal. However, these guidelines are frequently disregarded, and the abuse of and dependence on Z-drugs has emerged as a growing public health concern in Brazil. The present article reviews the current evidence on Z-drug use disorder-including dependence and withdrawal-and proposes clinical guidelines for the management of disconti

Does tobacco dependence worsen cannabis withdrawal in people with and without schizophrenia-spectrum disorders?

BACKGROUND AND OBJECTIVES: Rates of cannabis use disorder (CUD) are higher in people with schizophrenia than in the general population. Irrespective of psychiatric diagnosis, tobacco co-use is prevalent in those with CUD and leads to poor cannabis cessation outcomes. The cannabis withdrawal syndrome is well-established and increases cannabis relapse risk. We investigated whether cannabis withdrawal severity differed as a function of high versus no/low tobacco dependence and psychiatric diagnosis in individuals with CUD. METHOD: Men with CUD (N = 55) were parsed into four groups according to s

Introduction to the Special Issue on "Anxiety Sensitivity and Addictive Behaviors".

Anxiety sensitivity (AS) is a cognitive, individual difference variable characterized by a fear of arousal-related bodily sensations due to beliefs that such sensations are signs of impending catastrophic physical, psychological, or social outcomes. AS has been linked to increased risk for the development and maintenance of panic attacks and anxiety disorders, and more recently has been related to risk for other psychopathological conditions including those related to substance misuse. This article introduces a special issue of Addictive Behaviors focusing on cutting edge findings on the relat

Benzodiazepines--current safety issues.

Benzodiazepines are relatively safe compounds. They do, however, have an adverse effect on psychomotor and cognitive functions. A variety of behavioural and mood disturbances have been reported, including increased hostility, depression, antisocial behaviour, paranoid ideation, and suicidal tendencies. Benzodiazepines may produce pharmacological dependence in therapeutic dosage. The typical pattern of a drug dependence syndrome is rare. Dependence is primarily manifested by an abstinence syndrome occurring after the dose of the drug is reduced or treatment is stopped. The problems of distingui

Relationship Between ABO Blood Groups and Mental Disorders.

Previous research has unveiled an intriguing positive association between the AB blood group and mental disorders in general. In this study, we compared ABO blood groups with five major groups of mental disorders to attain a higher level of specificity. The analyses were conducted using data from the CoLaus|PsyCoLaus study (N=5111). They revealed that the AB blood group exhibited a positive association with both neurodevelopmental disorders (RR 2.29, CI 1.38-3.82) and substance use disorders (RR 2.25, CI 1.38-3.65) after adjusting for sex and childhood adversities. These associations could be

A Prospective Comparison of Bipolar I and II Subjects with and without Comorbid Cannabis Use Disorders from the COGA Dataset.

OBJECTIVE: The comorbidity of alcohol and substance use disorders among persons with bipolar disorder is elevated, as indicated by epidemiological and clinical studies. Following alcohol use, cannabis is the most frequently used and abused illicit substance among bipolar individuals, and such use may lead to comorbid cannabis use disorders (CUD). Previous research indicated that CUDs were related to a more severe course of bipolar disorder and higher rates of other comorbid alcohol and substance use disorders. Few studies, however, have conducted longitudinal research on this comorbidity. The

Sleep abnormalities associated with alcohol, cannabis, cocaine, and opiate use: a comprehensive review.

Sleep abnormalities are associated with acute and chronic use of addictive substances. Although sleep complaints associated with use and abstinence from addictive substances are widely recognized, familiarity with the underlying sleep abnormalities is often lacking, despite evidence that these sleep abnormalities may be recalcitrant and impede good outcomes. Substantial research has now characterized the abnormalities associated with acute and chronic use of alcohol, cannabis, cocaine, and opiates. This review summarizes this research and discusses the clinical implications of sleep abnormalit

Neurologic Complications of Drug and Alcohol Use.

OBJECTIVE: This article reviews the neurologic syndromes associated with substance use and suggests an approach for identifying and managing substance use disorders. LATEST DEVELOPMENTS: Substance use and overdose mortality, largely associated with fentanyl, rose sharply during the COVID-19 pandemic. Although recent data indicate modest decreases, current rates of overdose remain higher than before the pandemic. A wide variety of opioid-related toxic encephalopathies have been identified recently. Many novel psychoactive substances are unregulated and easily obtained online or in stores; seve

Office-Based Addiction Treatment in Primary Care: Approaches That Work.

Primary care is an important setting for delivering evidence-based treatment to address substance use disorders. To date, effective approaches to treat, care largely incorporate pharmacotherapy with counseling-based interventions and rely on multidisciplinary teams. There is strong support for primary care-based approaches to address alcohol and opioid use disorder with growing data focused on people living with human immunodeficiency virus and those experiencing incarceration. Future work should focus on the implementation of these effective approaches to decrease health disparities among peo

Emerging Threat of Xylazine.

BACKGROUND: The increasing reports of xylazine in clinical and postmortem toxicology necessitate a survey of its clinical impact. This review examines reports of xylazine across the peer-reviewed literature and grey literature (government and public health reports). CONTENT: Before 2005, reports of xylazine abuse primarily involved veterinary-associated accidental and intentional exposures. A notable shift occurred in 2006 with increasing xylazine exposure documented among persons with illicit drug use, including fatal and nonfatal overdoses. Xylazine prevalence continued to increase in clini

Patterns, harms and responses to licit and illicit substance use in Zimbabwe: A scoping review.

Anecdotal reports highlight increased methamphetamine, cocaine, and codeine-cough syrup use in Zimbabwe, with no clear empirical basis. Therefore, the scoping review aimed to identify primary evidence of patterns, harms and responses to substance use (SU) within Zimbabwe. Arksey and O'Malley's (2005) framework and the PRISMA Extension for Scoping Reviews (Tricco et al., 2018) were followed. Medline (Pub Med), Scopus, Academic Search Premier, Cumulative Index to Nursing and Allied Health Literature (CINAHL Plus), African Index Medicus, Africa-Wide Information, Web of Science, PsycInfo, PsycArti

Identifying Benzodiazepine Abuse: A Report of 3 Cases.

Benzodiazepines are a class of tranquilizers that, like prescription opiates, are commonly subject to abuse. Abusers of benzodiazepines commonly malinger to sustain access to these prescriptions from their health care providers by creating and enhancing medically necessary conditions requiring treatment with benzodiazepines. Such behavior may involve displays of unrelenting extreme anxiety that are resolved only by the administration of benzodiazepines. Typical substance abuse behaviors also include noncompliance with treatment, lack of self-care, and lack/failure of counseling. Three cases un

Shared Medical Appointments for Substance Use Disorders: A Systematic Review.

BACKGROUND: Shared medical appointments (SMAs) integrate group-based clinical care with individual medical management and are increasingly used in the treatment of substance use disorders (SUDs) to improve access, engagement, and care coordination. However, their use, outcomes, and implementation in SUD care have not been systematically synthesized. METHODS: We conducted a systematic review registered with PROSPERO (CRD42025639504). MEDLINE, Embase, PsycINFO, Scopus, and Cochrane Central were searched from database inception through September 2025. Randomized controlled trials, observational
