Significance of detection of extra metacentric microchromosome in amniotic cell culture.

A metacentric bisatellited microchromosome was detected in all metaphases from an amniotic culture performed because of maternal age. A wide-ranging survey of the literature failed to disclose any consistent anomaly associated with such a marker, but did reveal that the clinical picture of patients manifesting it could range from complete normality through mental retardation to a variety of deformities. The parents elected for termination, and the only deformity detected in the abortus was fixed talipes equinovarus. The implications of the finding of this marker chromosome on amniocentesis, be…

Central core disease with family history of malignant hyperthermia: report of one case.

A 10-month-old boy presented with gross motor delay and muscle weakness, especially in both lower limbs. At age 5, he developed lordosis, talipes, and planovalgus. His grandmother died of malignant hyperthermia during surgery. On neurological examination, he had mild proximal muscle weakness and atrophy, decreased deep tendon reflexes and Gowers' sign, but his intelligence was normal. The electromyogram showed myopathic pattern. Muscle biopsy revealed type 1 fiber atrophy and central core abnormalities. We report this case of central core disease with a family history of malignant hyperthermia…

The universal, muscular chain reaction, muscle spasm, torsions, ruptures and extravasations. Chameleons of pathology and some manifestations of simple muscular disorders.

Most bodily functions require the coordinated actions of complementary and supplementary paired muscle groups. Where this essential muscular cooperation is lacking, hollow organs may burst and others become literally screwed up, giving rise to many similar spastic diseases such as Torticollis, Twisted ovarian cyst, Torsion of the Testis, Volvulus of the intestines, Varicose Veins, Megacolon, Aortamegaly, Scoliosis, Erb's Palsy, Peyronie's Disease, Main-en-Griffe, Undescended Foot (Pes Cavus), Talipes, Strabismus. Spasm is "panenepidemic" and unclassified examples of Torsion Dystonia and Dyskin…

Case report: human neonatus with spina bifida, clubfoot, situs inversus totalis and cerebral deformities: sequence or accident?

An unreported coincidence of malformations has been studied by anatomical dissection in a mature male fetus. Auxiliary, this fetus was examined by computed tomography and the skeletal malformations were three-dimensionally reconstructed. The organs of the thorax as well as of the abdomen showed a visceral inversion. In the lumbar region, a hyperkyphosis was detected in addition to a split-notochord. A hydrocephalus internus with enlarged lateral ventricles and an agenesis of the corpus callosum were apparent. The lower extremities were dystrophic with both showing a talipes equinovarus-deforma…

Sonographic assessment of clubfoot deformity in young children.

We report a new diagnostic imaging technique for talipes equinovarus. Sonographic assessment of the medial border of the foot offers insight into the center of tarsal malalignment at the talonavicular joint. Technique, observations, and interpretation of the findings are delineated and compared with current radiologic evaluation of talonavicular alignment according to the method of Simons in 38 cases. The TnCE-angle is introduced as a sonographic measure to quantify talonavicular malalignment in clubfeet. Sonographic results of clubfeet are compared with those of a group of 98 normal feet. The…

Fetal ascites and oligohydramnios: prenatal diagnosis of a sialic acid storage disease (index case).

In a 20-year-old primiparous patient, a routine ultrasound scan performed at 28 weeks revealed fetal ascites, bilateral talipes, and oligohydramnios. This woman, married to possibly her first cousin, was at risk for an autosomal recessive disease, a metabolic disorder. At 29 weeks, an amniotic fluid biochemical study revealed the presence of an abnormal band of free sialic acid, leading to a diagnosis of a congenital form of sialic acid storage disease. Termination of pregnancy was performed at 30 weeks. Measurement of free sialic acid in cultured fetal skin fibroblasts confirmed the diagnosis…

A 26-month-old child with Marden-Walker syndrome and pyloric stenosis.

We recently examined a 26-month-old boy with abnormal face, blepharophimosis, hypertelorism, apparently low-set ears, micrognathia, arachnodactyly, talipes equinovarus, and joint contractures. Subsequently he manifested failure to thrive, respiratory infections, and developmental delay. These congenital anomalies and associated findings are consistent with a diagnosis of the Marden-Walker syndrome. He also had mild pyloric stenosis and duodenal bands, not previously reported in this syndrome. This syndrome appears to be an autosomal recessive trait in some families. A summary of findings of th…

Congenital form of spinal muscular atrophy predominantly affecting the lower limbs: a clinical and muscle MRI study.

We describe clinical and muscle magnetic resonance imaging (MRI) findings in 11 cases (three familial and eight sporadic) with the form of spinal muscular atrophy characterised by predominant involvement of the lower limbs with weakness of the proximal and distal muscles and marked atrophy of the distal leg and foot muscles. All patients presented at birth with talipes, which were in extension in seven of the 11. Arm muscle and function were preserved and lower limbs appeared to be disproportionately shorter compared to trunk and upper limbs. Functional abilities were markedly affected and onl…

Foot deformity in myotonic dystrophy.

Myotonic dystrophy is a progressive multisystem disorder that may be inherited from either parent, although only children with affected mothers usually show the more severe congenital form of the condition; others are classified as having adult myotonic dystrophy. Since 1940, 29 patients with myotonic dystrophy (10 adult, 17 congenital, two unclassified) have been followed at the Alfred I. duPont Institute. Treatment protocols have been established for the treatment of talipes equinovarus deformity commonly seen in the congenital form of myotonic dystrophy, as well as the progressive dropfoot …

Clubfoot deformity and tibiofibular diastasis.

Four children with distal tibiofibular diastasis associated to talipes equinovarus deformity are described. Tibial hypoplasia resulting in tibiofibular diastasis is believed to be the underlying cause of the deformity. This complex deformity is usually associated with other congenital malformations including congenital cardiac defects and other limb anomalies. Surgical therapy has centered on establishment of an ankle mortise; however, several authors believe that the best treatment is early ankle disarticulation rather than reconstructive surgery. Radiographs should be made of all apparent cl…

Hindfoot flexibility assessment of cavovarus and planovalgus feet by modified Shriners Hospitals for Children - Greenville (mSHCG) foot model.

BACKGROUND: Multi-segment foot models have been used to quantify foot kinematics during walking. However, walking kinematics is not sufficient to assess hindfoot flexibility (available range of hindfoot varus-valgus motion). The modified Shriners Hospitals for Children - Greenville (mSHCG) foot model has been used to quantify hindfoot flexibility with Coleman block test (peak hindfoot valgus) and Root test (peak hindfoot varus). Sensitivity of mSHCG foot model to detect clinically relevant difference in hindfoot flexibility measures for planovalgus (PV) and cavovarus (CV) feet has not been dem

Effect of Foot Posture on Limits of Stability in Children With Spastic Diplegic Cerebral Palsy.

Cerebral palsy (CP) is a neurological condition that affects musculoskeletal system causing altered balance control, which is governed by constant adjustments by muscular activity and joint positioning. Since the foot is the platform upon which we stand and balance, it is important to study and characterize foot posture abnormalities in CP to better understand their possible effect on multidirectional limits of stability (MDLS) in standing. Our aim was to find relation between foot posture and MDLS in diplegic children. Thirty diplegics (13 pes-planus and 17 pes-cavus) between the age of 6 to

Foot Deformity in Patients With Ankylosing Spondylitis: Is It Associated With Functionality and Disease Activity?

Foot involvement affects mobility and functionality in patients with ankylosing spondylitis but it remains unknown if foot deformities in ankylosing spondylitis patients affect functionality, disease activity, and quality of life. The aim of this study was to evaluate in detail the presence of a relationship between radiologically detected foot deformities in ankylosing spondylitis patients and both clinical and electrophysiological findings. The cross-sectional study included 110 patients with ankylosing spondylitis who were diagnosed according to the Assessment in Spondyloarthritis Internati

Ankle and Pantalar Arthrodesis: End-Stage Salvage in Cavus Foot.

Bony alignment is the primary goal in foot and ankle reconstruction of the cavovarus foot. This condition presents as a malalignment causing a medial overload of the ankle articular surface and lateral overload of the hindfoot, midfoot, and forefoot. A painful gait associated with articular degeneration of the numerous joints can lead to a chronic and rigid arthrosis of joints, warranting arthrodesis of the affected joints accordingly.
