Innumerable descriptions about variations in the pattern of extensor tendons are recorded in the literature. The dorsum of the hand in an adult male cadaver revealed an unusual pattern of extensor arrangement during a gross anatomical practical session. The in vivo forces in human digital flexor tendons were determined in 33 patients undergoing a carpal tunnel decompression surgery using a newly developed device. The tendons were tested in passive and active flexion, and flexing against resistance of up to 500 g. Forces in the range of 0.2 to 50 N were noted. Whilst the mean of these forces remained below the force normally required to dehisce a fresh modified Kessler repair, the range of values was such that we would only recommend early movement with great caution and under supervision.

Two patients with combined rupture of the anterior tibial tendon and posterior tibial tendon are described. Both were elderly women with a gradually progressive valgus deformity of the ankle/hindfoot and severe pain. Arthrodesis procedures were performed in both patients; however, postoperative complications prevented significant clinical improvement. This combined tendon rupture is presented to document a new clinical entity.

Rupture of the patellar tendon is uncommon. In most cases the rupture occurs at the inferior border of the patella rather than through the midsubstance of the tendon. Bilateral ruptures are particularly rare and may occur sequentially or, as in the case reported here, simultaneously. Surgical repair is necessary to re-establish the extensor mechanism and produces good results.

In 2 sisters with congenital camptodactyly and joint effusions, abnormalities in tendons were restricted to the portion within synovial sheaths. This implied a disease of the tenosynovium, rather than one of the tendon itself. In areas of chronic involvement, some tendons were replaced by fibrous tissue. Significant portions of the tendons in fingers with camptodactyly were replaced by hard scars. Congenital camptodactyly is probably the result of an intrauterine tenosynovitis, rather than an isolated congenital anomaly.
