7 Current practice of healthcare professionals in hospice and hospital palliative care settings related to eye donationas part of end-of-life care: a national survey.

BACKGROUND: Around 53% of the world's population have no access to the benefits of sight saving and sight restoring transplantation surgery due to a short fall in the supply of ophthalmic tissue that is only available via eye donation (ED). In England the National Health Services Blood and Transplant (NHSBT) seeks to have a consistent and sustained supply of eye tissue to satisfy current demand, however, historically and currently there is gap between supply and demand. Data reports that between April 2020 - April 2021, 3,478 corneas were donated a 37% decrease on the previous year figure of 5

[End-stage skin failure: an evolving clinical paradigm between wound care and palliative care.].

Over the past decade, the concept of skin failure has received increasing international attention, including definitions such as terminal ulcers, Skin Changes at Life's End (SCALE), and unavoidable pressure injuries. In the absence of a shared Italian translation, in this paper we adopt the term "end-of-life skin lesions" to describe end-stage skin failure, keeping the term international for consistency with the literature. The emerging paradigm recognizes the skin as an organ that, under conditions of global physiological deterioration, can undergo terminal failure, especially in the last wee

Application of a regional, multiorganisational education programme to implement a digital advance care plan.

INTRODUCTION: Advance care planning (ACP) is key to effective end-of-life care. If digitalisation of ACP is to be achieved, targeted education addressing the challenges of system-level change with diversity across roles, environments and patient groups is required. Currently, little is known about how to design and deliver this education. METHODS: This report describes a multiorganisation education programme focused on increasing engagement with digital advance care plans (DACP). Education sessions were delivered by clinicians and evaluated by postsession participant feedback questionnaire. T

Policy priorities in palliative and end-of-life care: An exploration of issues reflected in public strategy.

OBJECTIVES: To map the issues and priorities reflected in palliative and end-of-life care policy in Australia. METHODS: A qualitative documentary policy analysis examined the content of national and state-based policies and strategies on palliative and end-of-life care that were published in Australia and identified during July-September 2020. Summative content analysis was used to quantify policy characteristics, key domains and depth of coverage across strategy documents and to identify emergent themes. RESULTS: Nine policies comprising a total of 304 pages published from 2005-2019 and cov

Gender-sensitive and diversity-sensitive end-of-life care in Switzerland (GiveCare): a mixed-methods study protocol.

INTRODUCTION: Palliative care has been identified as one of the most inequitable areas of healthcare. In Switzerland, as globally, disparities in end-of-life care (EOLC) exist along socio-demographic lines, shaping the access to and quality of care received across services by patients and their caregivers. Research has linked these disparities to binary gender differences and other aspects of a person's social position. The GiveCare project aims to provide a systemic understanding of how aspects of gender and diversity intersect to shape the provision of EOLC in Switzerland and to translate th

Policy pathways and historical insights: Canada's evolving approach to psychedelic access for end-of-life distress.

Canada's evolving attitudes toward psychedelic interventions in palliative and end-of-life care reflect a departure from historically prohibitionist policies and an emerging recognition of their therapeutic potential for individuals facing end-of-life distress. This shift parallels international regulatory developments in jurisdictions such as the United States, Australia and parts of Europe, where cautious policy liberalization has signaled growing acceptance of psychedelics within clinical contexts. Canada is also a relevant case because of its formative role in the development of modern pal

Aging and End-of-Life Care Planning Among Transgender and Nonbinary Individuals: A Systematic Review.

PURPOSE: The objective of this review was to identify perceptions of end-of-life (EOL) care needs among transgender and nonbinary (TNB) individuals. METHODS: A systematic review was conducted following the Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines. We used PubMed and Web of Science databases to identify EOL care needs, experiences, and preferences related to TNB individuals, with publication dates spanning January 1, 1988, through February 20, 2024. Articles were required to be written or translated in English, focus on EOL care factors, and include TNB ind

Sliding doors at the bedside: conditional outcomes and moral judgments in end-of-life care.

BACKGROUND: Intensive care medicine has increasingly embraced shared decision-making and advance care planning as core components of good clinical practice. Nonetheless, clinical reasoning is sometimes implicitly framed as a linear, biologically driven process. In contexts of prognostic uncertainty, this framing risks obscuring a structural feature of decision-making: the constitutive role of value-based judgments in shaping prognosis and outcomes. MAIN BODY: This paper introduces the concept of conditional outcomes to clarify a structural feature of certain clinical situations, in which survi

Growing palliative and end-of-life care research in the United Kingdom: Findings from a mixed methods survey.

BACKGROUND: Globally the number of people with an advanced progressive illness is increasing. Research is essential to inform policy, services and clinical practice to meet their needs. We investigated researcher perspectives on conducting palliative and end-of-life care research in the United Kingdom and what is needed to grow this field. METHODS: We conducted a mixed-methods online survey of individuals conducting palliative and end-of-life care research in an academic institution or any care setting. Quantitative data were analysed descriptively, while qualitative data from open-ended ques

Palliative care interventions and their early integration in the management of older adults with acute myeloid leukemia: a narrative review.

BACKGROUND AND OBJECTIVE: Palliative care (PC) interventions have become essential in treating patients with acute myeloid leukemia (AML). Studies indicate that the early PC (EPC) integration during the entire disease course, from the diagnosis and the start of treatment until the end-of-life (EOL), especially regarding the decision-making process and the relief of symptom burden, can significantly impact the patient's quality of life (QoL) of AML patients and comprehensively address their physical and psychosocial needs. This narrative review synthesizes quantitative and qualitative research,

Parenteral Hydration in Dying Patients With Cancer: A National Registry Study.

CONTEXT: Clinically assisted hydration during end-of-life care among patients with cancer is controversial; practice varies between clinical settings and countries, and there is a lack of evidence. OBJECTIVES: To examine whether breathlessness, respiratory secretion, or confusion correlates with receiving parenteral hydration during end of life, adjusted for sex, age, and place of death. METHODS: The Swedish Register of Palliative Care database was used to collect data about the usage of parenteral hydration during the last day of life, and the occurrence of three symptoms during the last we

Palliative sedation - revised recommendations.

Palliative sedation is defined as the monitored use of medications intended to induce a state of decreased or absent awareness (unconsciousness) to relieve the burden of otherwise intractable suffering in a manner ethically acceptable to the patient, their family, and healthcare providers. In Switzerland, the prevalence of continuous deep sedation until death increased from 4.7% in 2001 to 17.5% of all deceased in 2013, depending on the research method used and on regional variations. Yet, these numbers may be overestimated due to a lack of understanding of the term "continuous deep sedation"

Evaluating the perceived added value of a threefold intervention to improve palliative care for persons experiencing homelessness: a mixed-method study among social service and palliative care professionals.

BACKGROUND: Palliative care for persons experiencing homelessness who reside in social service facilities is often late or lacking. A threefold intervention was implemented to improve palliative care for this population by increasing knowledge and collaboration between social service and palliative care professionals. This consultation service comprised: 1) consultations between social service professionals and palliative care professionals; 2) multidisciplinary meetings involving these professionals; and 3) training of these professionals. This study aims to evaluate the perceived added value

Palliative Sedation at Home: A Scoping Review.

Palliative Sedation (PS) is an effective measure for the relief of refractory symptoms in end-of-life patients. This intervention can be performed at home, respecting the patients' and their families' decisions. A scoping review was performed to map the available evidence in the literature on the performance of PS at home. This review included 23 studies. Most were conducted in European countries with adult cancer patients. Patients, family members and healthcare providers participated in the decision making regarding the use of PS at home. PS was used primarily to manage refractory symptoms (

"Palliative Syringe Driver"? A Mixed-Methods Study in Different Hospital Departments on Continuous Infusions of Sedatives and/or Opioids in End-of-Life Care.

OBJECTIVES: Continuous infusions of sedatives and/or opioids (continuous infusions) are frequently used in end-of-life care. Available data indicate challenges in nonspecialist palliative care settings. We aimed to assess the use of continuous infusions during the last week of life in different hospital departments. METHODS: In a sequential mixed-methods design, a retrospective cohort study was followed by consecutive qualitative interviews in 5 German hospital departments. Medical records of 517 patients who died from January 2015 to December 2017 were used, and 25 interviews with physicians

Are cancer patients living alone more or less likely to achieve a good death? Two cross-sectional surveys of bereaved families.

This study examined differences in sociodemographic characteristics and the achievement of a good death between cancer patients who live alone and those who do not live alone prior to death in different settings. Secondary analysis of data collected across two cross-sectional self-reported questionnaire surveys was undertaken. The participants were bereaved family members of cancer patients who had died in palliative care units (PCUs), acute hospitals or homes. We stratified the data by the place of death and examined the differences in sociodemographic characteristics to determine the relatio

Transitioning to Comfort-Focused Care at the End of Life.

Shared decision making requires the exchange of information from the patient and the surgeon (and ideally involves the expertise of the entire multidisciplinary team) to determine the medical and/or surgical treatment that best aligns with the patient's goals and values. Should the surgical patient wish to transition to end-of-life care, the transition to comfort-focused care is within the scope of practice for surgeons. Incorporating the expertise of other health care professionals is an important consideration for whole-patient care. Integrating primary palliative care into surgical practice

Biting the Hands that Feed "the Alligators": A Case Study in Morbid Obesity Extremes, End-of-Life Care, and Prohibitions on Harming and Accelerating the End of Life.

Obesity, recognized as a disease in the U.S. and at times as a terminal illness due to associated medical complications, is an American epidemic according to the Centers for Disease Control and Prevention ("CDC"), American Heart Association ("AHA"), and other authorities. More than one third of Americans (39.8% of adults and 18.5% of children) are medically obese. This article focuses on cases of "extreme morbid obesity" ("EMO")-situations in which death is imminent without aggressive medical interventions, and bariatric surgery is the only treatment option with a realistic possibility of succ
