Total gastrectomy via thoracotomy for cancer of the cardia or fundus of the stomach (report of 90 cases).

From 1978 through 1990, 90 total gastrectomies with esophagojejunostomy via thoracotomy were performed for the treatment of cancer of the cardia or fundus of the stomach in our hospital. Eighty-five patients were in TNM-stage III and five in stage IV. The 30-day postresectional mortality was 1.1%, and the 5-year survival rate was 13.8%. 14CO2 breathing test and clinical evaluation of 34 postoperative patients (not included in the 90 patients) showed that total gastrectomy may decrease the incidence of positive residual cancer along the incision lines, and may help to avoid small stomach syndro…

Correlations between findings at computed tomography (CT) and at thoracoscopy/thoracotomy/autopsy in pleural mesothelioma.

Thirty five computed tomography (CT) scans of the thorax and upper abdomen of thirty three patients with malignant pleural mesothelioma were correlated with the findings at thoracotomy (28 patients), thoracoscopy (2 patients) or autopsy (5 patients). Pleural thickening with contrast medium enhancement was detected on the CT scans of all patients. This finding was a valuable diagnostic tool, especially in cases of large amounts of pleural effusion and/or only thin layers of tumour barely or not at all visible on plain film chest X-rays. Difficulties in defining the exact extent of the diseases …

[Total esophagectomy by stripping without thoracotomy in cancer of the pharynx, esophagus and cardia. 34 cases].

Total oesophagectomy by stripping without thoracotomy was performed in 34 cancer patients to reduce operative mortality and morbidity in those with impaired respiratory function. The carcinoma was located in the pharynx in 20 cases, on the cardia in 9 cases and on the lower oesophagus in 5 cases. Reconstruction in the posterior mediastinum was effected with the descending colon in 32 patients. The operative mortality rate was 11.8% and fistulization developed in 20.6% of the cases. Eight patients (26.5%) required post-operative endoscopic dilatation. Ninety per 100 of the patients who survived…

Thoracotomy as a staging procedure after chemotherapy in the treatment of Stage III nonseminomatous carcinoma of the testis.

Of 108 patients with a nonseminomatous testicular carcinoma, 28 with lung metastases were studied. After combination chemotherapy with cisplatin, vinblastine, and bleomycin (PVB), 11 patients underwent exploratory thoracotomy. Viable carcinomatous tissue, along with fibrosis, necrosis, and mature teratoma, was found in 4 patients. Three of these patients were successfully retreated with VP 16-213, cisplatin, and actinomycin D. In patients with residual pulmonary or mediastinal masses after chemotherapy, resection of the lesions is mandatory to demonstrate viable carcinoma so that treatment can…

Paravertebral Block for Postoperative Pain Management After Open Thoracotomy: A Review of Efficacy and Risks.

PURPOSE OF REVIEW: Paravertebral block (PVB) is an increasingly utilized regional anesthesia technique for managing postoperative pain in patients undergoing open thoracotomy. Effective pain control following thoracic surgery is crucial for optimizing patient recovery, reducing opioid consumption, and minimizing respiratory complications. RECENT FINDINGS: PVB provides unilateral analgesia by delivering local anesthetic to the paravertebral space, effectively blocking somatic and sympathetic nerve fibers. PVB has gained popularity as an alternative to epidural analgesia due to its efficacy and…

Comparison of the lateral extracavitary and lateral thoracotomy surgical approaches for the treatment of thoracic spinal schwannomas.

BACKGROUND: Thoracic spinal schwannomas are benign nervesheath tumors that can cause significant neurological impairment. Surgical resection remains the treatment of choice and gold standard for achieving gross total resection and preventing recurrence. The lateral extracavitary approach (LECA) and lateral thoracotomy (LT) are commonly used techniques; however, the relative advantages of these two approaches remain uncertain, and comparative data in the Mexican population are lacking. METHODS: We performed a retrospective cohort study of adults undergoing LECA or LT for thoracic spinal schwan…

Posterior left pericardiotomy for prevention of re-thoracotomy and postoperative atrial fibrillation in aortic surgery.

OBJECTIVES: Re-thoracotomy due to pericardial effusion is a frequent complication after aortic surgery, leading to prolonged intensive care unit (ICU) and hospital stays and adverse outcomes. This study aims to evaluate the frequency of re-thoracotomy and postoperative atrial fibrillation in patients undergoing ascending aorta replacement with or without posterior left pericardiotomy. METHODS: We retrospectively analysed clinical data from patients who underwent elective ascending aorta replacement with or without aortic root between January 2014 and June 2024. Patients were divided into two …

Outcomes and guideline adherence for emergency department thoracotomy: a single-center experience in Saudi Arabia.

BACKGROUND: Emergency department thoracotomy (EDT) is a high-risk, salvage procedure performed during the resuscitation of critically injured patients who are in extremis or experience cardiac arrest immediately before or after arrival at the emergency department (ED) following trauma. Despite its potential to improve outcomes in selected patients, its indications and ideal clinical applications remain controversial due to significant risks of morbidity and mortality. In Saudi Arabia, trauma is responsible for approximately 20% of fatalities and is the leading cause of disability-adjusted life…

Preoperative susceptibility to developing secondary hyperalgesia is associated with post-thoracotomy pain at 2 months.

BACKGROUND: Identifying the subset of patients at risk for developing persistent pain after surgery is clinically important as they could benefit from targeted prevention measures. In this prospective study, we investigated if the preoperative assessment of the individual susceptibility to developing experimentally induced secondary hyperalgesia is associated with post-thoracotomy pain at 2 months. METHODS: Forty-one patients scheduled to undergo a posterolateral thoracotomy were recruited before surgery and followed prospectively for 2 months. The day before surgery, we experimentally induce…

Management of adverse effects after inadvertent intrathecal injection of morphine in a dog undergoing thoracotomy.

A 33-kg adult Labrador Retriever dog presented for investigation of pneumothorax. A computed tomography scan confirmed the diagnosis, and right lateral thoracotomy and lung lobectomy were performed under general anaesthesia. During surgery, anaesthesia was maintained with a propofol constant-rate intravenous (IV) infusion (premedication with methadone and dexmedetomidine) and IV atracurium was given to facilitate surgery. A lumbosacral epidural injection of preservative-free morphine (0.3 mg kg-1 diluted to 6.6 mL with sterile saline) was performed. Using a spinal needle, the 'hanging drop' me…

Thoracotomy patch repair of large diaphragmatic herniae in a porcine model: a tale of two patches.

PURPOSE: Congenital diaphragmatic herniae (CDH) may require patch closure in 50% of the cases. We assessed a biologic and composite mesh in a porcine CDH model. METHODS: Left sided thoracotomy was performed in 20 pigs. Approximately, 30% of the diaphragm was excised and the patch (Surgisis® or Parietex®) inserted to close this defect. The pigs were killed at 6 months and the diaphragm was harvested for biomechanical and histological assessment. RESULTS: The mean weight of the pigs at surgery and killing were 6.1 kg (4.2-8.4 kg) and 94.1 kg (80-131 kg), respectively. There were two recurrence…

Thoracotomy vs. Thoracoscopy for Esophageal Atresia with Tracheoesophageal Fistula Repair: Is There a Difference in Quality of Life?

AIM: Esophageal atresia (EA) is the most common congenital esophageal malformation. Thoracoscopic repair is gaining popularity, but controversy persists regarding the ideal surgical approach for this challenging anomaly. This study's aim is to analyze outcomes in terms of quality of life (QoL) of children undergoing thoracotomy and thoracoscopy for type III EA repair. METHODS: Perinatal characteristics, malformations, surgical procedures, complications, gastrointestinal, and respiratory current symptoms were collected. QoL was investigated through Esophageal-Atresia-QoL (EAQoL) and Pediatric-…

Closure of Ventricular Septal Rupture through a Left Thoracotomy in a Patient with a History of Esophageal Reconstruction.

A 73-year-old man who had undergone esophagectomy and retrosternal gastric tube reconstruction for esophageal cancer 8 years prior was transferred to our hospital for the treatment of an acute myocardial infarction. Emergent percutaneous coronary intervention for the left anterior descending artery (#7) was successfully performed. However, echocardiography revealed a ventricular septal rupture (25×27 mm). Seventeen days after admission, the rupture was successfully treated with a double-patch closure via a left anterolateral thoracotomy to avoid a surgical injury to his retrosternal gastric tu…

Anesthetic management of thoracotomy for massive intrathoracic solitary fibrous tumor of the pleura: a case report.

BACKGROUNDS: Solitary fibrous tumor of the pleura (SFTP) is a rare thoracic tumor and usually asymptomatic. Massive SFTP may affect adjacent organs and tissues including pulmonary vasculature, bronchus and heart. A thoracotomy for massive SFTP is necessary in severe case. Therefore, it is important for anesthesiologists to understand the condition of patients with massive SFTP and develop an appropriate anesthetic management strategy. A 76-year-old woman with massive SFTP presented to our clinical center and was evaluated as requiring thoracotomy. She received multidisciplinary cooperation tre…

Effect of thoracoscopic and thoracotomy on postoperative wound complications in patients with lung cancer: A meta-analysis.

Источник пометил публикацию как отозванную

Because of the difficult surgical procedures, patients with lung cancer who have received thoracic surgery tend to have postoperative complications. It may lead to postoperative complications like wound infection, wound haematoma and pneumothorax. A lot of research has assessed the effect of various surgery methods on postoperative complications in pulmonary cancer. The purpose of this meta-analysis is to establish if thoracoscopic is superior to that of thoracotomy in the rate of post-operative complications. From the beginning to the end of June 2023, we performed an exhaustive search on fou…

Midterm outcomes of isolated tricuspid valve surgery with a mini-thoracotomy and beating heart strategy.

BACKGROUND: Isolated tricuspid valve (TV) operation is considered high-risk surgery; thus, early surgical referral is frequently discouraged. Our study aims to evaluate the outcomes of isolated TV surgery with a mini-thoracotomy and beating heart strategy. METHODS: We retrospectively reviewed 25 patients [median age, 65.0 years (Q1-Q3, 59.0-72.0 years)] who had undergone mini-thoracotomy beating heart isolated TV surgery from January 2017 to May 2021. TV repair was performed in 16 patients (64.0%), and TV replacement in 9 patients (36.0%). Among them, 18 patients (72.0%) had previous cardiac …

Factors Associated with Successful Video-Assisted Thoracoscopic Surgery and Thoracotomy in the Management of Traumatic Hemothorax.

BACKGROUND: Few studies have identified factors associated with successful VATS or thoracotomy as the initial operative strategy among patients with traumatic hemothorax. MATERIAL AND METHODS: We performed an exploratory analysis using the 2008 to 2017 TQP database. We identified all patients aged 18 to 89 years with traumatic hemothorax who were treated with tube thoracostomy alone in the first 24-hours of admission, followed by VATS or thoracotomy. Logistic regression was used to identify factors associated with successful VATS (no conversion or reoperation) or thoracotomy (no reoperation) …

Performance of Focused Assessment with Sonography for Trauma Following Resuscitative Thoracotomy for Traumatic Cardiac Arrest.

BACKGROUND: In patients undergoing resuscitative thoracotomy (RT) for traumatic cardiac arrest, focused assessment with sonography for trauma (FAST) is often used to look for intraperitoneal fluid. These findings can help determine whether abdominal exploration is warranted once return of spontaneous circulation is achieved; however, the diagnostic accuracy of FAST in this clinical scenario has yet to be evaluated. The purpose of this study was to assess the performance of FAST in identifying intra-abdominal hemorrhage following RT. METHODS: We performed a 3-year retrospective study at a high…
