Temporary Use of Skeletal Traction in Management of Pediatric Supracondylar Humeral Fractures.

BACKGROUND: The paper presents a treatment algorithm for supracondylar humeral fractures in children, involving temporary use of skeletal traction following failure of primary closed reduction and percutaneous fixation (CRPF) and introducing closed reduction after management by skeletal traction. The aim of this study was to assess the outcomes of supracondylar humerus fracture treatment performed according to the algorithm. MATERIAL AND METHODS: The treatment was performed in 149 children (70 girls, 79 boys) with extension-type supracondylar humeral fractures. The study group was assessed wi…

Intraoperative Shoulder Traction as Cause of C5 Palsy: Magnetic Resonance Imaging Study.

OBJECTIVE: During surgery, shoulder traction is often used for better fluoroscopic imaging of the lower cervical spine. Traction on the C5 root has been implicated as a potential cause of C5 palsy after cervical spine surgery. Using magnetic resonance imaging, this study was undertaken to determine the impact of upper extremity traction on the C5 root orientation. METHODS: In this study, 5 subjects underwent coronal magnetic resonance imaging of the cervical spine and left brachial plexus. Using a wrist restraint, sequential traction on the left arm with 10, 20, and 30 lb. was applied. Measur…

Rigid External Distraction with Intranasal Bone-borne Traction Hooks for Midfacial Hypoplasia.

Rigid external distraction is currently performed to correct cases of severe maxillary hypoplasia. As an improvement of this technique, we propose the use of an intranasal bone-borne traction hook. This study is a retrospective chart review of the intranasal bone-borne traction hooks used in the treatment of severe maxillary hypoplasia. There were 110 patients treated with the hooks from 2005 to 2017. The maximum traction force was 7.75 kg, and there were few complications encountered during distraction. There were 76 patients who had the hooks removed under local anaesthesia. A cephalometric …

Distal radial traction radiographs: interobserver and intraobserver reliability compared with computed tomography.

BACKGROUND: Distal radial fracture characterization with standard radiographs has been consistently poor, leading to a widespread use of computed tomography (CT) with this injury. Traction radiographs have recently been shown to improve interobserver reliability for fracture characterization and treatment. Our goal was to compare five traction radiographs and CT images to evaluate the interobserver and intraobserver reliability of individual fracture fragment identification, the correct identification of fracture fragments on CT imaging compared with traction images, and the consistency of tre…

Spontaneous improvement of macular traction retinal detachment associated with myopic macular schisis.

High myopia can be associated with a range of pathologic changes within the macula that are now easily appreciated with optical coherence tomography. In the setting of high myopia and a macular traction retinal detachment, the expectation is for progressive worsening over time, and surgical intervention is often undertaken early. The authors present a case of spontaneous improvement of myopic macular detachment, which illustrates the potential value of an initial period of observation in this clinical setting.

Outcome of traction tenolysis in open trigger finger release--a retrospective review.

Surgical treatment for trigger finger involves division of the A1 pulley. Some surgeons perform an additional step of traction tenolysis by sequentially bringing the flexor digitorum superficialis and flexor digitorum profundus tendons out of the wound gently with a Ragnell retractor. There is currently no study which states whether flexor tendon traction tenolysis should be routinely performed or not. The objective of this study is to compare the outcome in patients who have traction tenolysis performed (A group) versus those who did not have traction tenolysis (B group) performed. It was not…

Promising results after vacuum-assisted wound closure and mesh-mediated fascial traction.

INTRODUCTION: Patients with an open abdomen (OA) present a major challenge to the surgeon. High mortality and associated complication rates have been reported depending on the specific method of temporary abdominal closure, the primary disorder and any co-morbidity. Vacuum-assisted wound closure and mesh-mediated fascial traction (VAWCM) is a novel technique recently introduced for late fascial closure of the OA. In previous studies, the disease aetiologies were mainly vascular and visceral surgical disease and trauma. We report our results using VAWCM in a non-trauma patient population treate…

Do traction radiographs of distal radial fractures influence fracture characterization and treatment?

BACKGROUND: Our center evaluates all distal radial fractures with traction radiographs before splinting. Although investigations of various imaging modalities to evaluate distal radial fractures have been presented in the literature, to our knowledge the use of traction radiographs has not been well described. We hypothesized that the addition of traction radiographs to standard radiographs increases interobserver and intraobserver reliability for injury descriptions, affects the choice of treatment plan, and decreases the perceived need for computed tomography. METHODS: Radiographs for fifty…

A convenient dynamic wire traction-fixation method for hand and forearm fractures.

We report on a convenient intraoperative distraction technique, called the modified Dynamic Wire-framed Traction (mDWT) method, that assists in fracture reduction and/or is used as an external fixator in patients with unstable distal radius fractures and in other hand and forearm fractures. The mDWT device is easily assembled with wires at the time of surgery, and its configuration produces sufficient longitudinal distraction force that makes reduction easier and facilitates additional fixation procedures. Furthermore, the device can be converted into a simple unilateral external fixator that …

Surgical treatment of brachial plexus traction injuries in children, excluding obstetric palsy.

Traumatic brachial plexus injuries in children, excluding birth palsy, are seldom reported. In this study, we report on 11 cases operated upon between 1995-1998, and followed for at least 30 months. All patients were males with an average age of 11 years (range, 3-16 years). The denervation time averaged 3.8 months (range, 1-8 months). Eight patients had two or more root avulsions; two had additional severe infraclavicular injuries. In total, 6 grafting and 25 extraplexal neurotization procedures were used. Donor nerves included the intercostal nerves, phrenic nerve, spinal accessory nerve, an
