Promoting the use of bicycle helmets during primary care visits.

PURPOSE: The purpose of this article is to communicate ways in which the nurse practitioner (NP) working with the pediatric population can incorporate bicycle helmet use education into the primary care setting. DATA SOURCES: Review of literature on bicycle helmets, safety, and head injury, retrieved from scientific journals and reliable Internet sources. CONCLUSIONS: Bicycling has become a popular sport for both children and adults. Despite excellent benefits, bicycling can pose hazards to riders when safety measures are not practiced. Head injury has been cited as the leading cause of death

Preemptive airway management planning: A retrospective evaluation of the pediatric difficult airway consultation service.

BACKGROUND/AIMS: The pediatric Difficult Airway Consultation Service (pDACS) was created in 2017 to identify patients with potentially difficult airways and create airway management plans prior to airway management. METHODS: Consults were either nurse-initiated, physician-initiated, or both nurse-and-physician-initiated and were examined for demographic and clinical factors. If a child had difficult airway risk factors, a consult note with airway management recommendations was completed. RESULTS: We included 419 consults from the 4-year study period for analysis. Sixty-one patients had chron

Critical Care Nurses' Traumatic Experiences and Compassion Fatigue: The Mediating Roles of Professional Self-Efficacy and Post-Traumatic Growth.

BACKGROUND: Critical care nurses (CCNs) are in the frontline of providing quality care to critically ill patients. Consequently, they are constantly exposed to stress, trauma and compassion fatigue. However, nursing literature is scarce in reporting the connections between vicarious trauma (VT), secondary traumatic stress (STS), post-traumatic stress disorder (PTSD), compassion fatigue (CF), professional self-efficacy (PSE) and post-traumatic growth (PTG) among CCNs. AIM: To examine the interrelationships of VT, STS, PTSD, CF, PSE and PTG among CCNs. STUDY DESIGN: A cross-sectional and corre

Assessment, management and treatment of acute fingertip injuries.

Fingertip injuries with nail bed trauma can require specialist hand surgery, depending on severity. However, most of these injuries can be managed in well-equipped emergency departments by emergency nurses with an in-depth knowledge and understanding of the anatomy and physiology of the fingernail and surrounding structures, assessment and examination, pain management and treatment. This article describes the surface and underlying anatomy and physiology of the nail, the most common mechanisms of injury, relevant diagnostic investigations, and initial assessment and management. It also discuss

Honoring loss in the neurosurgical intensive care unit: an exemplar of caring.

This article illustrates how the staff in a neurosurgical intensive care unit cared for a 22-year-old woman who sustained severe head and spine trauma after a motor vehicle crash. She was pregnant, and the fetus died. The staff were determined to bring love and compassion to the tragic event, honoring the death of the fetus, making arrangements for baptism and for the family to spend time with the fetus and mother after the birth. Witnessing this demonstration of love reminds us how blessed we are to be nurses and how human life is sacred at every stage.

Traumatic atlanto-occipital dislocation: a pediatric case report.

Atlanto-occipital dislocation, once considered rare and fatal, has become more common. Reasons for improved survival are training, efforts of prehospital care givers, improved diagnostics, and a collaborative healthcare approach to the patient. Advanced prehospital care strategies and diligence in educating prehospital providers are the core elements in preventing further injury and facilitating injury identification. Coupled with improved diagnostics, time to injury identification and treatment initiation is accelerated. This pediatric case study presents a coordinated plan of care demonstrat

Nurse Adaptability and Post-traumatic Stress Disorder Symptoms During the COVID-19 Pandemic: The Effects of Family and Perceived Organizational Support.

OBJECTIVE: To examine the effect of family and perceived organizational support on the relationship between nurse adaptability and their experience with COVID-related PTSD (post-traumatic stress disorder) symptoms in frontline nurses working on COVID-19 units. BACKGROUND: Proximity to and survival of life-threatening events contribute to a diagnosis of PTSD, which is characterized by avoidance of reminders of trauma, intrusive thoughts, flashbacks of events, sleep disturbances, and hypervigilance. Using the job-demands and resource model, we examined the effect of adaptability, family support

A Quality Improvement Project to Assess if Preoperative Trauma Patients Are Receiving Their Prescribed Medications Preoperatively.

Introduction All preoperative trauma patients should receive their preoperative medications regardless of being nil per os (NPO). Anticoagulants, angiotensin-converting enzyme inhibitors (ACE-I), and angiotensin II receptor blockers (ARBs) should be omitted. This is according to both local and national guidelines. We noticed that some preoperative patients have not been receiving their morning medications prior to having their operations. This has led to pre and perioperative complications. The aim of this study, therefore, was to conduct a quality improvement project to assess if preoperative

What's in a Name? Provider Perception of Injured John Doe Patients.

BACKGROUND: We previously demonstrated that unidentified aliased patients, John Doe's (DOEs), are one of the highest risk and most medically fragile populations of injured patients. Aliasing can result in misplaced information and confusion that must be overcome by health care professionals. DOE alias use is institutionally dependent and not uniform, which may lead to significant variation in perception of confusion and error. We sought to determine if health care practitioners experience confusion that may result in compromised care when caring for injured DOE patients. METHODS: After obtain

Beyond "true" and "false" memories: remembering and recovery in the survival of childhood sexual abuse.

As survivors of childhood sexual abuse (CSA) struggle to grasp and reclaim their selves, their stories, and their futures from the grip of aftereffects of trauma, the processes of recovery and rehabilitation are interwoven with remembering. Questions about women's delayed memories of CSA have stirred a controversy that places clients' credibility at stake. Nurses need to understand the historical and political roots of this controversy and to be familiar with the empirical knowledgebase that exists about traumatic memory. This article is a critical feminist analysis of the topic. Its purposes

The need for long-term support: Five-year outcomes after severe injury in older adults.

BACKGROUND: While the short-term risks of severe injury among older adults (age≥65) are well studied, little is known about long-term functional outcomes in this population. This knowledge gap impacts clinicians' ability to counsel patients and provide care aligned with their values. Our objective was to evaluate the association between severe injury and the likelihood of an older adult remaining alive and living in their own home five years later. METHODS: This was a retrospective, matched, population-based cohort study using administrative health data in a large regional trauma system (2006

Understanding kinesiophobia in older postoperative patients admitted to the ICU: A narrative review of conceptual models, assessment tools, and clinical implications.

BACKGROUND: Early mobilization is a key component of recovery for older patients in intensive care units (ICUs). However, psychological resistance to movement-particularly fear-based avoidance-remains poorly understood and frequently overlooked in critical care settings. OBJECTIVE: This narrative review aims to explore the conceptual relevance of kinesiophobia (fear of movement) in older postoperative patients admitted to the ICU, examine its potential assessment challenges, and outline nursing-sensitive strategies for identification and intervention. METHODS: A purposive literature search w

Medical Lessons from the 1972 Andes Plane Crash.

In 1972, a chartered plane carrying rugby players crashed in a remote region of the Andes, leaving 32 survivors exposed to extreme cold, high altitude, and near-total deprivation of food and water. After 72 days, only 16 survived. Two secured rescue after crossing a 16,000-foot peak and a 10-day trek despite severe malnutrition. Although widely known, the medical aspects of this event remain underexamined. We conducted a retrospective descriptive study of the 16 survivors. Data were obtained through interviews with survivors conducted by Roberto Canessa, MD-both a cardiologist and a survivor-a

Male rape--the silent victims.

This literature review examines the issue of male rape and the possible counselling strategies that may assist in recovery. There are several misconceptions surrounding male rape which can result in the under-reporting and secondary or sanctuary victimisation of the survivor. Men who have been raped may believe that it attacks the very essence of what it is to be masculine and male. Many may not seek help unless they perceive a need for immediate attention, such as physical trauma requiring medical assistance. The literature also reveals that when men do seek help they may be treated poorly. S

Designing and Implementing a Simulation-Based Pediatric Trauma Training Program in a Resource-Limited Setting: The PRACTICE Study.

BACKGROUND/OBJECTIVES: Road traffic accidents represent the leading cause of death in children and adolescents in Brazil. In the state of Piauí, non-specialized facilities frequently manage critically injured pediatric patients. Targeted training programs are considered a key strategy for improving outcomes. This study assessed the feasibility of adapting a pediatric trauma course developed in a high-income setting for medical first responders in both pre-hospital and in-hospital settings in a resource-limited environment in northeastern Brazil. METHODS: This prospective non-randomized mixed-

Effect of guided imagery on dyspnea in trauma ICU patients: a randomized controlled trial.

BACKGROUND: Dyspnea is a prevalent and distressing symptom among critically ill patients, particularly in trauma intensive care units, and is often poorly managed in semi-conscious individuals. While guided imagery has shown benefits in various chronic and palliative contexts, its efficacy in ICU trauma patients remains understudied. METHODS: In this randomized controlled trial, 102 trauma ICU patients with Glasgow Coma Scale scores of 9-13 were randomly allocated to an intervention group (n = 51) or a control group (n = 51). The intervention group received nature-based guided imagery session

Frailty Screening in the Emergency Department Enables Personalized Multidisciplinary Care for Geriatric Trauma Patients.

Frailty is a multidomain reduction in physiologic reserve that impacts recovery and can contribute to poor outcomes following trauma beyond what chronological age, comorbidities, or injury severity predicts. In geriatric trauma patients, a large proportion are frail or prefrail on initial encounter in the emergency department, and because there are opportunities for actionable management plans, major trauma guidelines endorse systematic screening integrated into coordinated geriatric trauma care. We reviewed the literature and identified practical instruments used in the acute trauma setting f

The Impact of Sensory Stimuli on Healthcare Workers and Outcomes in Trauma Rooms: A Focus Group Study.

OBJECTIVE: This study investigated issues related to noise, lighting, and temperature in trauma rooms that impact patient care and staff performance. BACKGROUND: Uncontrolled sensory stimuli can hinder healthcare delivery quality in trauma rooms. High noise and temperature levels can increase staff stress and discomfort as well as patient discomfort. Conversely, proper lighting can decrease staff stress levels and reduce burnout. Sensory overload in trauma rooms is a crucial concern, but no studies have been conducted on this issue. METHOD: Using a convenience sampling method, 65 trauma team

Livestock trauma in central Texas: cowboys, ranchers, and dudes.

INTRODUCTION: Emergency nurses routinely treat a wide variety of animal-related injuries, yet the trauma literature rarely addresses injuries associated with livestock. METHODS: The purpose of this retrospective, descriptive study was to examine the incidence, demographics, severity, and other factors associated with trauma related to livestock in persons admitted to one central texas trauma center during a 5-year period. Patients were classified by activity at the time of injury. The following 5 categories were identified: cowboy, rancher, dude, standing near, and unidentified. Groups were f
