Interleukin 6 (IL-6) levels in the monitoring of surgical trauma. A comparison of serum IL-6 concentrations in patients treated by cholecystectomy via laparotomy or laparoscopy.

Interleukin 6 (IL-6) is involved in the processes of inflammation and tissue repair. We have been looking for the correlation between IL-6 concentration in patient's serum and the other factors related to the gravity of surgical trauma. Out of 25 patients with acute cholecystitis, 11 were randomly selected for laparoscopic cholecystectomy and the other 14 for open (laparotomic) cholecystectomy. The diagnostic and prognostic factors, age, and duration of disease did not differ significantly in either group. IL-6 serum levels were monitored by using commercially available ELISA tests throughout

Severe blunt assault injuries at Auckland Hospital.

The incidence and nature of severe blunt assault injury in the Auckland region was assessed by a retrospective review over a 2-year period. Approximately 14,000 charges of assault (excluding paediatric and sexual assault) were laid during this period. A total of 1035 people were admitted to hospital. Of 331 patients admitted to one hospital, 35 patients were admitted via the resuscitation room after severe assault; of these, two died. Of these patients, 33 were male and 19 of 21 ethanol levels measured were greater than 17 mmol/l. Head and facial injuries were the most frequent.

Accuracy of the history of injury obtained from the caregiver in infantile head trauma.

OBJECTIVE: We investigated the accuracy of parental reports of infant trauma history. METHODS: We retrospectively reviewed 166 infants from 3 tertiary institutions in Japan that had suffered a traumatic intracranial hemorrhage between 2002 and 2013. The infants were classified into 3 groups based on the parents' report of the mechanism of injury: the low-risk group consisted of injuries sustained from falling off a height below 3 ft. (n=84); the middle-to-high-risk group comprised other mechanisms of injury (n=54); and the unknown-risk group consisted of unknown mechanisms of injury (n=28). T

The article describes the clinical case and analyzes the literature concerning endoprosthesis replacement in the case of traumatic thoracic aortic injury in polytrauma. Current strategy of diagnostics and treatment of patient in critical condition with rupture of isthmic aortic part is based on the principle of multistage surgical treatment (damage control orthopedic). The external fixation of multiple pelvis and extremities fractures and on-time implantation of stent-graft in thoracic aortic part were performed at first hours after the admission. A condition of the patient was stabilized. The

Disparities in trauma center access of older injured motor vehicular crash occupants.

BACKGROUND: To evaluate whether older injured motor vehicular crash (MVC) occupants' access to trauma centers (TC) reflects the lower threshold suggested in triaging recommendations. METHODS: Adult front seat occupants of MVCs transported to a hospital from 1999 through 2006 included in the National Automotive Sampling System (NASS) were studied. Cases were classified by their age in years (≤60 years or >60 years). Younger and older injured MVC occupants were compared in relation to their likelihood of being transported to a TC. Traumatic bitemporal hemianopia.

A 45-year-old male pedestrian was struck by a motor vehicle moving at high speed. Upon initial assessment, the patient scored a 3T on the Glasgow Coma Scale. The patient suffered multiple facial and sinus fractures, a right orbital wall fracture, and a depressed open frontal skull fracture with visible brain parenchyma. Due to the nature of the brain injury, the patient was taken to the operating room emergently for a right frontal craniectomy. The patient required prolonged hospitalization followed by transfer to a rehabilitation facility. Six weeks after the accident, the patient underwent a

Complications of emergency tracheal intubation in severely head-injured children.

A high incidence of unsuccessful attempts and complications has been reported when emergency tracheal intubation (ETI) is performed outside the hospital in severely injured children. The aim of this prospective series was to analyse the incidence and related risk factors of complications of emergency tracheal intubation. The time to complete successful ETI and occurrence of incidents, e.g. cough reflex, hypoxia or spasm were related to the experience of the physician performing intubation and the use of drugs to facilitate ETI. The incidence of hypoxia, hypercarbia, postintubation complication

Torture rhabdomyorhexis--a pseudo-crush syndrome.

Two patients who were systematically tortured and deprived of any oral intake presented with acute renal failure several days later. Unlike the classical crush syndrome, we describe a clinical entity wherein repeated direct muscle injury from blunt trauma, in addition to forced dehydration, led to myoglobinuria and renal failure. The literature is reviewed, and biochemical indices predicting severity of injury, pathophysiology, and management protocol are described. This pseudo-crush syndrome caused by rhabdomyorhexis in addition to rhabdomyolysis is an unusual entity, in part related to extre

Racial Discrimination and Resting-State Functional Connectivity of Salience Network Nodes in Trauma-Exposed Black Adults in the United States.

IMPORTANCE: For Black US residents, experiences of racial discrimination are still pervasive and frequent. Recent empirical work has amplified the lived experiences and narratives of Black people and further documented the detrimental effects of racial discrimination on both mental and physical health; however, there is still a need for further research to uncover the mechanisms connecting experiences of racial discrimination with adverse health outcomes. OBJECTIVE: To examine neurobiological mechanisms that may offer novel insight into the association of racial discrimination with adverse he

Different flames, different fates : A comparative study of surgical interventions and critical care burden in electrical vs. thermal burn injuries.

BACKGROUND: High-voltage electrical injuries (HVEI) and thermal burns are both classified as severe burn trauma, yet their clinical impact differs markedly. The HVEIs cause deep tissue destruction and systemic complications that are often underestimated in severity by conventional burn indices such as TBSA (Total Body Surface Area) and ABSI (Abbreviated Burn Severity Index). METHODS: A retrospective cohort analysis was performed on 1515 burn patients treated between 1994 and 2024. After exclusions 1489 patients were evaluated, including 92 with HVEI and 1397 with thermal burns. The demographi

Clearing the cervical spine in critically injured patients: a comprehensive C-spine protocol to avoid unnecessary delays in diagnosis.

Clearing the cervical spine in polytrauma patients still presents a challenge to the trauma team. The risk of an overlooked cervical spine injury is substantial since these patients show painful and life-threatening injuries to one or more organ systems so that clinical examination is usually not reliable. A generally approved guideline to assess the cervical spine in polytrauma patients might significantly reduce delays in diagnosis, but a consistent protocol for evaluating the cervical spine has not been uniformly accepted or followed by clinicians. One purpose of this study was to analyse t

Predictive performance of ISS and NISS for clinical outcomes in severely injured trauma patients: a retrospective registry study.

BACKGROUND: The Injury Severity Score (ISS) and New Injury Severity Score (NISS) are widely used in evaluation of injury severity in trauma patients. The aim of this study is to assess the predictive accuracy of these scoring systems on clinical outcomes. METHODS: We conducted a retrospective registry study of 1,112 severely injured trauma patients (NISS ≥ 16) treated at Tampere University Hospital (TAUH) between 2015 and 2024. Outcomes included in-hospital mortality, prolonged hospital and ICU stay (≥ 75th percentile), in-hospital intubation, and blood transfusions. Predictive performance wa

The search for a simple injury score to reliably discriminate the risk of in-hospital mortality in South Africa.

BACKGROUND: The Injury Severity Score and Trauma and Injury Severity Score are used commonly to quantify the severity of injury, but they require comprehensive data collection that is impractical in many low- and middle-income countries . We sought to develop an injury score that is more feasible to implement in low- and middle-income countries with discrimination similar to the Injury Severity Score and the Trauma and Injury Severity Score. METHODS: Clinical data from KwaZulu-Natal, South Africa were used to compare the discrimination of the Injury Severity Score and the Trauma and Injury Se
