[Nontuberculous Mycobacterium pulmonary infection with pleural effusion caused by Mycobacterium kansasii].

A 28 year-old man was admitted to our hospital because of fever, cough and chest pain. A chest X-ray film taken on admission showed infiltrate in the left upper lung field with ipsilateral pleural effusion. Microscopical examinations of stained specimens of sputa and pleural effusions disclosed no acid-fast bacilli. The level of adenosine deaminase (ADA) in pleural effusion was 46.4 IU/l. A tuberculin skin test was moderately positive. The most probable diagnosis was pulmonary tuberculosis with pleural effusion. Isoniazid (INH) and rifampicin (RFP) were administered on the 5th hospital day and

[A comparison of the result of aspiration, needle-biopsy, pleuroscopy and thoracotomy in the diagnosis of chronic pleurisy (author's transl)].

A series of 60 diagnostic pleuroscopies was carried out in cases of isolated chronic pleurisy, or pleurisy accompanied with pleural and parenchymatous lesions. The following diagnoses were obtained : 23 non-specific chronic inflammations, 1 purulent pleurisy, 1 rheumatoid pleurisy, 25 neoplasms and 10 tuberculosis. In all cases, pleural aspiration, with bacteriology and cytotology of the fluid, as well as pleural needle-biopsy with the Abrams needle, were carried out before, with negative results in 53 cases and doubtful results in 7 cases. Later, 14 thoracotomies were carried out, either for

Radiologic manifestations of adult tuberculosis.

Adult pulmonary tuberculosis commonly presents with radiographic findings of upper lobe cavitary opacities, in which case the correct diagnosis is usually suspected. However, other radiographic presentations of this disease in the adult include intrathoracic adenopathy, pleural effusion, lower lobe consolidation, a miliary pattern, and a solitary nodule. In these latter cases, tuberculosis may not be considered in the differential diagnosis. This article discusses the protean manifestations of adult pulmonary tuberculosis.

Mesothelial cells in pleural fluid: TB or not TB?

Eighty-five samples of pleural fluid obtained from 76 patients with biopsy-proven tuberculous pleurisy were examined cytologically. Numerous reactive mesothelial cells were present in only 1,2% of specimens examined. In contrast, 65,3% of pleural fluid aspirates obtained from a control group of patients in congestive cardiac failure contained marked mesothelial exfoliation. The suggestion that the presence of numerous, often highly reactive mesothelial cells in a pleural aspirate makes the diagnosis of tuberculosis unlikely is confirmed.

The diagnostic value of T cell spot test and adenosine deaminase in pleural effusion for tuberculous pleurisy: A systematic review and meta-analysis.

BACKGROUND: Tuberculous infection of T cell spot test (T-SPOT.TB) and adenosine deaminase (ADA) have a high diagnostic value in pleural effusion for tuberculous pleurisy. However, there were major differences in existing research in regard to the clinical application of the two trials. Therefore, we conducted a meta-analysis to systematically evaluate the diagnostic value of T-SPOT.TB and ADA. METHODS: Pubmed, Web of Science and Embase databases were searched to compare diagnosis of tuberculous pleurisy by T-SPOT.TB and ADA. The search period was from inception to August 31, 2021. Statistical

[A case of tuberculous pleural effusion diagnosed by polymerase chain reaction (PCR)--with reference to tuberculous pleurisy using polymerase chain reaction].

A 38-year-old man was admitted to hospital because of chest pain and for the evaluation of a right pleural effusion on his chest X-ray film. Pleural effusion was characterized as exudative lymphocyte-predominant fluid with elevation of adenosine deaminase (ADA). Bacteriologic examination of pleural fluid was negative in both smear and culture of the fluid. PPD was positive and ESR was elevated. Mycobacterial DNA was detected in the pleural effusion using polymerase chain reaction (PCR) with primers which amplified a fragment of Is6110. Following treatment (INH, RFP and EB), the right pleural e

Penile tuberculosis associated with monoclonal gammopathy of undetermined significance.

Mycobacterium tuberculosis (TB) infection of the penis is a rare but serious problem. We report a case of penile TB in a 75-year-old man who presented with fever and dyspnea. No active lung lesions except pleural and pericardial effusion were found on chest X-ray. Monoclonal gammopathy of undetermined significance was diagnosed after serum and urine electrophoresis studies, and repeated bone marrow studies. Genital examination showed diffuse papulonecrotic skin ulcers involving the whole penile shaft, extending ventrally to the median raphe of the scrotum. Pus smear showed positive acid-fast s

Immunoassay with Novel Paired Antibodies for Detection of Lipoarabinomannan in the Pleural Fluid and Plasma of Patients with Tuberculous Pleurisy.

Tuberculous pleurisy (TP) is one of the most common forms of extrapulmonary tuberculosis, but its diagnosis is challenging. Lipoarabinomannan (LAM) antigen is a biomarker for Mycobacterium tuberculosis (Mtb) infection. LAM detection has potential as an auxiliary diagnostic method for TP. We have successfully generated five rabbit anti-LAM monoclonal antibodies (BJRbL01, BJRbL03, BJRbL20, BJRbL52, and BJRbL76). Here, anti-LAM antibodies were tested to detect LAM in the pleural fluid and plasma of patients with TP by sandwich enzyme-linked immunosorbent assays (ELISAs). The results revealed that
