The spectrum of endocrine dysfunction in active pulmonary tuberculosis.

OBJECTIVE: Substantial variation in the prevalence (0-55%) of hypoadrenalism in tuberculosis (TB) has been reported. The aim of this study was to prospectively evaluate adrenal, thyroid and gonadal function in patients presenting with active pulmonary TB. DESIGN: A prospective study of 50 patients admitted to a single hospital. PATIENTS: Fifty hospitalized adults with newly diagnosed sputum positive pulmonary TB (mean age 38 years, mean body mass index 18 kg/m2, mean albumin 28 g/l) were studied. MEASUREMENTS: Adrenal reserve was assessed by intravenous tetracosactrin administration with me

[The reasons for the untimely detection of pulmonary tuberculosis and their characteristics].

In order to clear up causes of late detection of pulmonary tuberculosis 772 outpatients' case histories, materials of centralized new cases control and results of untimely case-finding analysis were studied in Vinnitsa region for the period from 1989 to 1992. Neglect of normative demands aimed at early tuberculosis case-finding, general practitioners' unwatchfulness and poor quality of fluorography service were established to be the main causes of untimely diagnosis of the disease.

Sarcoidosis accompanied by pulmonary tuberculosis and complicated by sacroiliitis.

We report the unique occurrence of a unilateral sacroiliitis in a patient with active sarcoidosis accompanied by pulmonary tuberculosis. Convincing (clinical) evidence of sarcoidosis as the extremely rare cause of this articular involvement is presented. Discussion is focused on comparison of sarcoidosis and tuberculosis, particularly with respect to their articular involvement, and the literature of previously reported cases of sarcoid sacroiliitis is briefly reviewed.

Pulmonary function and immunologic abnormalities in miliary tuberculosis.

In a group of 33 patients with miliary tuberculosis (MTB), pulmonary function testing and bronchoalveolar lavage (BAL) were each performed in 31. The arterial blood gas analysis was done in 13 patients. Mild restrictive ventilatory defect, hypoxemia, and hypocapnia were observed. BAL revealed lymphocytic alveolitis. The lymphocyte subsets were measured in the BAL fluid and peripheral blood in 10 patients. The helper lymphocytes were decreased in the peripheral blood and increased in the BAL fluid. The immunoglobulins (IgG, IgA, and IgM) were significantly increased in the peripheral blood and

Pulmonary tuberculosis in patients treated with inhaled beclomethasone.

Inhaled beclomethasone dipropionate (BDP) has been used with few side-effects in the treatment of bronchial asthma for 2 decades. Until now the manifestation of tuberculosis (TB) in patients on inhaled BDP has not been reported. Eight patients with allergic asthma, of a total of 548 asthmatics (1.46%) seen over a 2-year period, developed active TB following the use of inhaled BDP. All were sputum-positive for acid-fast bacilli (AFB) on smear and/or culture, all responded well to a combination of anti-TB drugs, and none showed evidence of immunological or pituitary-adrenal suppression. Two pati

The radiography, haematology and biochemistry of pulmonary tuberculosis in the aged.

Ninety-three elderly patients with pulmonary tuberculosis were surveyed prospectively to determine the extent and severity of radiological, haematological and biochemical abnormalities. Radiological features of inflammatory changes were present in all patients; 7 per cent of these were purely apical, 48 per cent were in mid and basal zones and in 46 per cent there was a mixed picture. Cavities were seen in 33 per cent of radiographs; half of these occurred in the apical and half in the mid and basal zones. A pleural reaction was present in 46 per cent of cases. Hilar gland enlargement was not

Radiological features of pulmonary tuberculosis in elderly patients.

We reviewed the radiological findings in 55 hospital inpatients aged 60 and over with bacteriologically proven tuberculosis over a 5-year period 1981-5. Since most of the patients were infected during childhood it was expected that the majority would show the 'typical' findings of post-primary disease with apicoposterior lung lesions. This was found in 43 of the patients (78%). Unusual features were found in 12 patients: solitary pleural effusion (1), miliary tuberculosis (2), tuberculoma (1), isolated mediastinal or hilar lymphadenopathy (1), solitary ventrobasal and middle-lobe or lingular l

The haematological and biochemical changes in severe pulmonary tuberculosis.

This study surveys the extent and severity of haematological and biochemical abnormalities which occurred in 265 patients with pulmonary tuberculosis, and records the haematological changes that occur with treatment. Anaemia was present in 60 per cent of patients, more frequently in males than in females. Leucocytosis with neutrophilia occurred in 40 per cent, lymphopenia in 17 per cent and monocytopenia in 50 per cent. Platelet count and erythrocyte sedimentation rate were elevated in 52 and 80 per cent respectively. Bone marrow aspiration and trephine biopsy were of limited diagnostic value.

A case of pulmonary tuberculosis presenting with a choroidal tuberculoma.

A 17-year-old woman was referred because of a choroidal mass, with marked decrease in vision, in her left eye. Her medical history was negative. However, preliminary examination revealed a positive skin tuberculin test (PPD) and a chest x-ray picture of pulmonary tuberculosis. Bronchoscopy documented the diagnosis. Findings of other systemic investigations were negative. Fluorescein angiographic findings were compatible with the clinical diagnosis of a choroidal tuberculoma. A regimen of antituberculous medications was started, and both the choroidal and the pulmonary lesions gradually subside

[Surgical treatment of tuberculosis of non-pulmonary localization].

Experiments in 86 rabbits and observation of 360 patients have shown the antibacterial therapy to be of limited use for the treatment of non-pulmonary tuberculosis. It was found that the antibacterial therapy failed to result in healing the caseous-destructive foci and to cure turebculosis of kidneys, bones, joints, female genitals and lymph nodes. It is considered to be a reason for surgical procedures according to principles of the focal surgery on non-pulmonary tuberculosis elaborated by P. G. Kornev and his school.

The accuracy of diagnosing pulmonary tuberculosis at a teaching hospital.

Accuracy of the clinical diagnosis of tuberculosis and of the mycobacteriology laboratory test results was assessed in a teaching hospital by reviewing clinical and microbiologic data on patients from whom Mycobacterium tuberculosis had been recovered. Mycobacteria were isolated in 230 of 6,550 specimens (3.5%). Clinical data were available for 42 patients with tuberculosis, 20 of whom had significant underlying nonmycobacterial disease. Positive tuberculin skin tests wee recorded for 90% of the patients with no underlying disease and for 29% of the patients with underlying disease. Tuberculos

Study of the immunological status of patients with pulmonary tuberculosis.

Most patients with pulmonary tuberculosis have impaired T-lymphocyte function as measured by dinitrochlorobenzene (DNCB) sensitization and response to tetanus toxoid. The humoral immune response as gauged by the ability to respond to typhoid vaccine was not affected. Tuberculous patients had higher immunoglobulin levels than normal controls. The relative proportions of T lymphocytes in patients were lower than in controls while those of B lymphocytes were normal. By these criteria, rifampin treatment did not seem to cause immunosuppression.

Clinical and roentgenographic spectrum of pulmonary tuberculosis in the adult.

In order to define the roentgenographic manifestations of pulmonary tuberculosis in the adult, we reviewed a 12 month experience with newly diagnosed patients with active pulmonary tuberculosis in two Boston hospitals. Of 88 patients, 30 (34 per cent) presented with roentgenographic manifestations other than those associated with "usual" postprimary disease. At least 12 patients (13.5 per cent) had primary tuberculosis, including two subjects over 60 years of age. Six patients (6.8 per cent) had disease confined to the lower lung fields, eight patients (9 per cent) had tuberculomas and four (4

T and B lymphocytes in patients with pulmonary tuberculosis.

Counts of T and B lymphocytes were made in the peripheral blood from patients with active and inactive pulmonary tuberculosis and in healthy subjects. In patients with active pulmonary tuberculosis the numbers of T cells were significantly lower than in the controls. After treatment for two months, if the patient's condition improved the percentage of T cells returned to nearly normal levels. If the patient failed to improve, the numbers of T cells remained low. The mechanism of these phenomena is discussed.
