The role of pepsin in the pathogenesis of peptic ulcer has been the subject of intense study and debate for many years. Two difficulties inherent in distinguishing between the role of acid alone vs acid and pepsin are that a) acid-containing gastric juice always contains pepsin, and, b) that hydrogen ion concentration (pH) is a major determinant of the activity of pepsin. A healthy 28-year-old man developed a slowly progressive corneal ulcer 21 months after an episode of corneal trauma. Acid-fast bacilli were identified in corneal scrapings, and the causative organism was identified as Mycobacterium avium-intracellulare. The charts of 139 patients operated on for benign gastric ulcer between 1976 and 1980 were reviewed. Indications for surgery included failure of medical management, bleeding, perforation and inability to differentiate benign from malignant disease. Surgical management included hemigastrectomy 29%, vagotomy with antrectomy 27%, vagotomy with pyloroplasty 13%, wedge resection 7% and highly selective vagotomy 4%. The healing rate of duodenal ulcers may be accelerated by secretory inhibitors such as histamine H2-receptor antagonists and pirenzepine, by antacids, by protective drugs such as sucralfate and colloidal bismuth, and by antidepressant drugs such as trimipramine. The effect of these drugs on the rate of healing is comparable; they differ with respect to practicability of treatment, incidence and types of side effects, and suitability for long-term administration. Currently, the most versatile and most thoroughly investigated drugs are the histamine H2-receptor antagonists.

It is a follow up of 242 patients with gastric ulcer disease. Sixty two per cent of them were observed for more than 3 years. They were controlled by X-ray examinations (41%) or endoscopically (59%). Full recovery was obtained in 127 patients (53%). A total of 115 were not healed until the end of the survey. The risk of malignisation is estimated as very low. Only 5 patients developed cancer of the stomach (2,06%). Much more important is the early differential diagnosis between gastric ulcer and the primary ulcerative form of gastric cancer. Ectopic gastric mucosa can be found in 40-80% of Meckel's diverticula; parietal cells producing acid are not inhibited by regulatory factors, giving rise in some cases to ulcers, which may perforate. A case report of a patient with a perforated ulcer in a Meckel's diverticulum is given. Meckel's diverticula should be removed surgically when they are diagnosed or when they are found by chance during surgery because of the risk of complications.

The mechanism of action of ulcer drugs without acid inhibition appears to involve improvement of defensive factors of the gastroduodenal mucosa. The concept of ulcer healing without acid inhibition is attractive for theoretical reasons, because doubts have arisen about the safety of elevation of intragastric pH, especially in long-term treatment of peptic ulcer disease. Skin cancers originating in areas of chronic injury (Marjolin's ulcers) are thought to behave in a more aggressive fashion than those due to other causes. The initial surgical treatment, especially the management of the regional lymph nodes, remains controversial. The histologic studies of the decubitus ulcer spectrum, which include blanchable erythema, nonblanchable erythema, decubitus dermatitis, decubitus ulcer, and the black eschar/gangrene reveal a dynamic process. The initial change occurs in the vessels of the papillary dermis. This is followed by necrosis of skin structures. The eschar/gangrene represents a full-thickness defect due either to prolonged ischemia and anoxemia or a sudden large vessel occlusion caused by shearing injury.
