Urachal catheter provides new choice for long-term urinary diversion in prune belly syndrome.

Prune belly syndrome has been identified as a clinical triad of abdominal muscle deficiency, bilateral cryptorchidism, and urologic abnormalities. We present the case of a discordant monozygotic twin with prune belly syndrome and voiding dysfunction that was relieved by long-term urinary catheterization by way of the urachus. To the best of our knowledge, this alternative method has not been previously reported. We suggest that for newborn infants with long-term voiding dysfunction, if the urachus retains patency, urinary catheterization through the urachus could be a choice for urine drainage…

Use and misuse of indwelling urethral catheters.

Because indwelling urethral catheterization is potentially detrimental, especially when it leads to urinary tract infection, indwelling urinary catheters must not be used indiscriminately. Appropriate use of indwelling catheters in recently obstructed cats is accomplished by identifying which cats are likely to experience repeated obstruction if such catheterization is not performed. When indwelling catheters are used, precautions must be taken to minimize the occurrence and severity of catheter-associated complications. Following catheter removal, which should be done as soon as possible, uri

[Postpartum urinary retention].

Postpartum urinary retention is an uncommon event that occurs in 0.7 to 0.9% of vaginal deliveries. An ignorance of this situation can lead to delayed diagnosis worsening the prognosis and to inadequate treatments. This complication is defined as the absence of spontaneous micturition within 6hours of vaginal delivery with a bladder volume above 400mL. The etiology depends on multiple factors. Because of physiological changes during pregnancy, the bladder is hypotonic with an increased post-void residual volume. The occurrence of a perineal neuropathy during delivery may cause a urinary retent

Residual urinary volume and urinary tract infection, a life threatening illness: a case report.

Multiple Sclerosis (MS) is an inflammatory disease that demyelinates the central nervous system causing progressive disability. The urodynamics of MS patients is an important consideration because these patients are susceptible to acute urinary bladder retention associated with a high fever. Treatment should include irrigation using a Foley catheter and an irrigation syringe to remove residual urine and sediment from the neurogenic bladder, intravenous fluid resuscitation, bacteriological analysis of the residual urine, and admission to an Intensive Care Unit for further evaluation by a urolog

Asymptomatic bacteriuria and urinary tract infection in geriatric inpatients after indwelling urinary catheter removal: a descriptive two-centre study.

BACKGROUND: Patients with indwelling urinary catheters (IUC) are common in geriatric care. Catheterization increases the risk of asymptomatic bacteriuria (ASB) and urinary tract infection (UTI). The prevalence of ASB after IUC-removal is only sparsely studied. This study aimed to compare the occurrence of ASB and UTI in geriatric patients with and without a history of catheterization and to explore factors associated with ASB. METHODS: Patients were included at two geriatric rehabilitation wards in Stockholm, Sweden. Data were collected about history of catheterization, antibiotic treatment,

Hospital-acquired urinary tract infections in the pediatric patient: a prospective study.

To determine through a prospective study the characteristics of hospital-acquired urinary tract infections (HAUTI) in children, 525 children subjected to bladder catheterization during a hospital admission were identified through surveillance of 12,316 admissions during a 24-month period. Urine culture results were available for 296 (56.4%) of the catheterized patients. In addition 12 noncatheterized children with a documented HAUTI were identified. The clinical courses of all patients with a HAUTI were followed for at least 6 months after their last HAUTI during the study period. Forty-four p

Clean and sterile intermittent catheterization methods in hospitalized patients with spinal cord injury.

The purpose of this study was to compare the incidence of urinary infection using clean intermittent catheterization with the incidence of infection using sterile intermittent catheterization in patients hospitalized with spinal cord injury who were not receiving prophylactic antibiotics. Forty-six patients were assigned randomly to a clean (n = 23) or sterile (n = 23) study group. Catheterizations were done at least every six hours. Infection was defined as bacteriuria greater than or equal to 100,000 organisms/mL or greater than or equal to 10,000 organisms per mL with fever of 100 degrees F

Bipolar radiofrequency thermotherapy treatment of the prostate in urinary catheter-dependent men.

OBJECTIVES: In this study, for the first time in the literature, we evaluated patients with benign prostatic hyperplasia (BPH) who had undergone permanent urinary catheterization for urinary retention and who were treated with radiofrequency (RF) thermotherapy as an alternative method due to their inoperability based on the high risk associated with anesthesia. We aimed to investigate these patients' posttreatment parameters concerning the catheter-free rates, quality of life (QoL) changes, and retrospective treatment efficacy. METHODS: RF thermotherapy was applied to 62 permanent patients ca…

Vesicostomy in adult meningomyelocele patients. Reappraisal of an old technique.

Intermittent catheterization is the standard treatment of neurogenic bladder dysfunction. In adult meningomyelocele patients, self-catheterization is not always possible or is not accepted. In these patients, urinary diversion is frequently hampered by anatomical abnormalities, preventing the use of bowel. Vesicostomy as a safe procedure in selected adult patients was employed at our department.

Recurrent purple urine bag syndrome presenting with full spectrum of disease severity: case report and review of literature.

Purple urine bag syndrome (PUBS) is a unique phenomenon characterized by purple discoloration of the urinary catheter bag and tubing following urinary catheterization lasting for hours to days. The purple discoloration is a mixture of indirubin dissolved in plastic with indigo on its surface. PUBS is most commonly associated with urinary tract infection (UTI) caused by bacteria with indoxyl phosphatase/sulfatase activity. It occurs predominantly in chronically catheterized, constipated elderly female patients. It usually appears to be asymptomatic and harmless, but rarely it can present as a s…

A clever technique for placement of a urinary catheter over a wire.

OBJECTIVE: The objective was to present a straightforward, step-by-step reproducible technique for placement of a guide-wire into any type of urethral catheter, thereby offering a means of access similar to that of a council-tip in a situation that may require a different type of catheter guided over a wire. MATERIALS AND METHODS: Using a shielded intravenous catheter inserted into the eyelet of a urinary catheter and through the distal tip, a "counsel-tip" can be created in any size or type of catheter. Once transurethral bladder access has been achieved with a hydrophilic guide-wire, this t

[Open or closed drainage systems: their impact on urinary infections].

Starting on the premise that the risks of infection due to vesical catheterism lasting more than 48 hours fundamentally depends on the type of drainage system employed, the authors make a study to see if it is possible to achieve a decrease in the rate of urinary tract infections by utilizing closed systems. This study follows a quasi-experimental design having two groups of patients who require this form of catheterism and analyzes its results. This study lasted five months and concluded that "the utilization of a closed drainage system having an anti-backup flow valve significantly reduces t
