Analysis of the causes of the unsatisfactory results of SPV in 39 patients has shown that the leading ones among them were the preservation of a high level of acid formation, antral stasis, chronic duodenal obstruction resulting from the extravasal compression of the celiac trunk and celiac ganglioneuritis. When choosing the corrective operation, it is necessary to consider the main factors causing the occurrence of an unsatisfactory result.

Changes in gastric motility were studied in the urethane-anaesthetized ferret following acute or chronic (3 weeks) vagotomy. The stomach was divided into the corpus and antrum and the effects of vagotomy on tone, frequency and contraction amplitude were investigated separately in the two gastric regions. Out of a total of 300 consecutive cases of duodenal ulcer undergoing surgery, 51 patients were treated by truncal vagotomy and a closed pyloroduodenal digital dilatation. To test the assumption that gastric decompression is beneficial after TV, 66 patients who underwent elective TV plus anterior pylorectomy were randomly allocated into three groups immediately after completing pyloric reconstruction. Changes in gallbladder motility and alteration of bile composition have been implicated in the formation of gallstones after truncal vagotomy. Hepatic bile composition was found to be unchanged after truncal vagotomy with Heineke-Mikulicz pyloroplasty in our long-term study. . The data suggest that the alleged increase in the incidence of cholelithiasis after vagotomy and pyloroplasty can be isolated from an effect on the liver. These results may have implications for the preferred course of surgical intervention in certain patients.

The authors report on the one-to-five year results of a prospective randomized trial of proximal gastric vagotomy (PGV) with and without pyloroplasty in 64 men operated upon electively for chronic duodenal ulcer. Old age and massive hemorrhage long have been recognized as ominous prognostic signs in the management of patients with peptic ulcer disease. The authors present the results of their long-term evaluation of 73 such patients. The group was unselected, critically ill, and treated surgically for the arrest of profuse hemorrhage. 426 subdiaphragmatic vagotomy operations were performed by the authors. On basis of the results obtained they give full details of the complications arising in the course and in the early postoperative period after vagotomy, and also of the possibilities preventing these complications. They call the attention to the more frequent injuries of the spleen and the oesophagus, and to the increasing number of the subphrenic abscess. Finally, they emphasize the low mortality rate of the vagotomy in comparison to the traditional gastric resections.
