The pathophysiology of benign paroxysmal positional vertigo (BPPV) is not completely understood. Although the concept of degenerated otoconia transforming the posterior canal (PC) crista into a gravity-sensitive sense organ has gained popular support, several temporal bone (TB) series have revealed similar deposits in normal TBs, suggesting they are a normal change in the aging labyrinth. Furthermore, some TBs from patients with BPPV do not contain particles in the posterior canal. Five TBs from patients with BPPV were studied quantitatively and qualitatively. A small PC cupular deposit was fo…

The etiology of many diseases involving vertigo is still unknown although the same etiologic factors have been suggested for several diseases. In most cases the diseases are diagnosed on the basis of exclusion-other possible causes being ruled out before the diagnosis can be confirmed. Benign recurrent vertigo (BRV) has been defined as spells of vertigo characteristic of Menieres disease without auditory or clinical neurologic symptoms and signs. The etiology of this condition is also unknown. BRV has been linked to migraine and viral diseases. In a prospective study we collected the clinical …

If medical therapy fails (6-12 months or more), careful consideration of surgical therapy should be followed with; the patient's hearing, severity of symptoms, age, and occupation. Cochleosacculotomy is reserved for elderly Meniere's patients with poor health, poor hearing and good vestibular function. Endolymphatic sac shunt should be considered as the first procedure for disabled Meniere's patients with aidable hearing and may also be used in those patients with bilateral Meniere's disease. Retrolabyrinthine vestibular nerve section (RLVNS) is indicated for patients with disabling vertigo an…

Neuro-otological and posturography findings in 15 patients with visually induced vertiginous symptoms (visual vertigo) are reported. Thirteen patients were considered to have a peripheral vestibular disorder; seven had abnormal caloric or rotational test results. Two patients had CNS disorder--a cerebellar degeneration and a brainstem stroke. Posturography testing showed that five patients showed abnormally large body sway induced by full field visual motion stimulation. This group included the two patients with CNS disease and four with strabismic symptoms (diplopia, squint surgery, and ocula…

One hundred sixty-two otosclerotic patients who underwent a stapedotomy between October 1991 and April 1994 were examined after experiencing postoperative vertigo. Twenty-eight complained of dizziness. In sixteen patients, we observed vertigo and nystagmus only within the first six postoperative days; in these cases, special treatment was not necessary. However, in twelve patients in whom vertigo persisted even after their stay in the hospital, the problem seriously affected their quality of life and their return to work. In these cases, an individualized diagnostic and therapeutic procedure w…

BACKGROUND: Unilateral benign paroxysmal positioning vertigo (BPPV) may mimic bilateral BPPV if the diagnostic positioning maneuver is executed without appropriate alignment of the head and neck with the trunk.
OBSERVATIONS: Appropriate alignment is achieved by adjusting the plane of the affected posterior semicircular canal so that it is parallel to the plane of diagnostic head tilt, ie, 45 degrees horizontal head rotation to the left before a head tilt to the right in suspected right-sided BPPV. With inappropriate head alignment and head tilt to the side of the unaffected ear, a free-floati…

Seventy-eight patients with phobic postural vertigo (PPV) and 17 patients with psychogenic disorder of stance and gait (PSG) were asked to evaluate their condition 6 months to 5.5 years after their original referral and short-term psychotherapy. Two results seem most important: (1) PPV had a favorable course with a 72% improvement rate (22% of patients becoming symptom free), whereas the majority of patients with PSG (52%) remained unchanged; (2) the majority of patients with PPV experienced complete remission or considerable improvement even if their condition had lasted between 1 and 20 year…

Brainstem hematomas carry a poor prognosis. There are rare reports of excellent outcome with hemorrhages in the midbrain. We report a 43-year-old woman with vertigo and minimal neurological symptoms, whose symptoms were initially mistaken for "inner ear disease," but subsequent investigations revealed a medullary hemorrhage. Recovery was complete within two weeks. While rare, medullary hemorrhage should be considered in the differential diagnosis of vertigo.

The clinical symptoms of otitic syphilis, either congenital or acquired, are indistinguishable from Meniere's disease. In most cases the symptoms can be controlled by long-term and intensive medical treatment with penicillin and steroids. However, in some cases, luetic vertigo is refractory to such medical regimens, and surgical intervention may become necessary. We report on endolymphatic sac procedures performed on 37 patients with presumed luetic vertigo refractory to medical treatment, between March 1980 and December 1987, with follow-up ranging from 2 years to 9 years and 9 months. Result…

Main features of the benign paroxysmal positional vertigo (B.P.P.V.) are: latency before the onset of vertigo, nystagmus of the rotatory type beating toward the lower ear, nystagmus tires out, nystagmus gets inverted when the head is brought back to orthostatism, nystagmus is reproducible as many times as the position is taken. B.P.P.V. is due to a deposit of heavy material on the cupula of the posterior semi circular canal. In this unusual condition, the cupula moves under the effect of gravity acceleration. The goal of the treatment is to free the cupula. The manoeuvre consists after determi…

Posturographic recording with the patient's head in retroflexion has been proposed as a reliable test for diagnosing cervical vertigo. The clinical experience suggests that a positive result in this test-condition could be accepted as suggestive for cervical involvement, when other data corroborate it, but that it is not per se pathognomonic. In fact, this test is negative in most cases with other cervical indications, whereas it has been found positive, in cases with no cervical indications. A positive test, however, is significantly more frequent in "cervical" cases. On the other hand, otoli…

Episodic vertigo, similar to that of Meniere's disease, can occasionally develop years to decades after an ear has suffered a profound sensorineural hearing loss. Although the pathogenesis of this process is unknown, it is theorized that delayed endolymphatic hydrops develops. An inflammatory reaction could cause obstruction of the endolymphatic duct or interfere with venous drainage from the region of the endolymphatic sac, leading to hydrops. This condition is reviewed, and two adult patients with severe episodic vertigo and a unilateral profound sensorineural hearing loss which occurred in …

Acute vertigo occurring 48 hours after stapedectomy is assumed to be related to inner ear trauma. Similarly, acute vertigo occurring weeks after stapedectomy could be related to a fistula of the oval window. No one has tested the hypothesis that some of these cases could represent concomitant cranial polyneuritis. We report the development after stapedectomy of five cases of acute cranial polyneuritis with vertigo diagnosed by physical examination of the cranial nerves. In another case we determined that vertigo occurring after stapedectomy was not related to concomitant cranial polyneuritis. …

Two patients complained of severe vertigo following eructation or hiccup. One patient was poststapedectomy; the other had idiopathic cochlear degeneration. The symptoms were reproduced by Hennebert's test in the pathologic ear. The tensor tympani was transected in both patients. An endolymphatic sac procedure was performed concomitantly in the second patient. Both patients had immediate relief of symptoms. It is hypothesized that these reflex phenomena, which elicited vertigo, were accompanied by tensor tympani contraction, which resulted in medial movement of the stapes or prosthesis, and tha…

An entity of episodic true vertigo of delayed onset following sudden and profound sensorineural hearing loss is described. Data on 12 patients and three case reports are presented. The latency between sudden deafness and the onset of the vertigo varied from 1 to 68 years. The vestibular symptoms are identical to the vestibular symptoms of Ménière's disease, and there is some evidence that endolymphatic hydrops in the previously deafened ear represents at least part of the labyrinthine pathology. Labyrinthectomy in the deaf ear was curative. Tentatively, this entity is best considered a variant…
