Development of clinical tools to estimate the breathing effort during high-flow oxygen therapy: A multicenter cohort study.

INTRODUCTION AND OBJECTIVES: Quantifying breathing effort in non-intubated patients is important but difficult. We aimed to develop two models to estimate it in patients treated with high-flow oxygen therapy. PATIENTS AND METHODS: We analyzed the data of 260 patients from previous studies who received high-flow oxygen therapy. Their breathing effort was measured as the maximal deflection of esophageal pressure (ΔPes). We developed a multivariable linear regression model to estimate ΔPes (in cmH2O) and a multivariable logistic regression model to predict the risk of ΔPes being >10 cmH2O. Candi

COVID-19: Pulse oximeters in the spotlight.

From home to intensive care units, innovations in pulse oximetry are susceptible to improve the monitoring and management of patients developing acute respiratory failure, and particularly those with the coronavirus disease 2019 (COVID-19). They include self-monitoring of oxygen saturation (SpO2) from home, continuous wireless SpO2 monitoring on hospital wards, and the integration of SpO2 as the input variable for closed-loop oxygen administration systems. The analysis of the pulse oximetry waveform may help to quantify respiratory efforts and prevent intubation delays. Tracking changes in the

Respiratory muscle failure.

The diseases which are commonly complicated by hypercapnic respiratory failure also compromise the respiratory muscles in several ways. Increased work of breathing, mechanical disadvantage, neuromuscular disease, impaired nutritional status, shock, hypoxemia, acidosis, and deficiency of potassium, magnesium, and inorganic phosphorus are the major non-neurologic factors which contribute to respiratory muscle fatigue and failure. Respiratory muscle fatigue has two components. High frequency fatigue occurs rapidly with intense contractile efforts but is usually not severe. It also recovers rapidl…

Aortic Dissection and Cardiac Tamponade in an Elderly Female With Cushing's Syndrome: A Fatal Case.

A 77-year-old female with multiple cardiovascular comorbidities presented with increased work of breathing and was found to have hypotension, bradycardia, and hypoxia. Despite initial stabilization, the patient's condition declined, leading to a sudden cardiac arrest. Postmortem examination revealed the cause of death as aortic dissection, with exsanguination into the pericardium and left pleural space. This case illustrates the value of considering aortic dissection in patients with a history of Cushing's syndrome, even when their presenting symptoms are nonspecific.

Noninvasive Options.

Noninvasive ventilation (NIV) has assumed a central role in the treatment of selected patients with acute respiratory failure due to exacerbated chronic obstructive pulmonary disease or acute cardiogenic pulmonary edema. Recent advances in the understanding of physiologic aspects of NIV application through different interfaces and ventilator settings have led to improved patient-machine interaction, enhancing favorable NIV outcome. In recent years, the growing role of NIV in the acute care setting has led to the development of technical innovations to overcome the problems related to gas leaka

Fog-induced cough with impaired respiratory sensation in congenital central hypoventilation syndrome.

RATIONALE: Congenital central hypoventilation syndrome (CCHS) is a genetic disorder mainly characterized by failure of automatic control of breathing, causing alveolar hypoventilation. Little is known regarding cough in CCHS. Parental reports indicate that patients cough normally during airway infections; however, previous studies have demonstrated no cough response to fog inhalation. OBJECTIVES: To evaluate the sensory and motor components of cough, respiratory sensations, and changes in ventilation evoked by fog inhalation in children with CCHS and in sex- and age-matched control subjects.

Prolonged neural expiratory time induced by mechanical ventilation in infants.

Mechanical ventilation may interfere with the spontaneous breathing pattern in infants because they have strong reflexes that play a large role in the control of breathing. This study aimed to answer the following questions: does a ventilator-assisted breath 1) reduce neural inspiratory time, 2) reduce the amplitude of the diaphragm electrical activity, and 3) prolong neural expiration, within the delivered breath? In 14 infants recovering from acute respiratory failure (mean age and weight were 2.3 +/- 1.3 mo and 3.95 +/- 0.82 kg, respectively), we measured 1) the electrical activity of the d

Congenital Diaphragmatic Hernia Presenting in a 7-Day-Old Infant.

A 7-day-old male infant presented to the emergency room after respiratory distress was noted at an outpatient well child check. On exam, he was observed to have tachypnea, increased work of breathing, and decreased breath sounds on the left side of the chest. On chest X-ray, he was found to have a left-sided congenital diaphragmatic hernia. The infant was transported to a tertiary care facility where the defect was repaired without complication. Interestingly, the mother had a history of a normal antenatal ultrasound, completed at 19 + 2 weeks of gestational age. This case report summarizes th…

Nitrogen tensions in brachial vein blood of Korean ama divers.

Intravascular bubble formation and symptoms of decompression sickness have been reported during repetitive deep breath-hold diving. Therefore we examined the pattern of blood N2 kinetics during and after repetitive breath-hold diving. To study muscle N2 uptake and release, we measured brachial venous N2 partial pressure (PN2) in nine professional Korean breath-hold divers (ama) during a 3-h diving shift at approximately 4 m seawater depth and up to 4 h after diving. PN2 was determined with the manometric Van Slyke method. Diving time and depth were recorded using a backpack computer-assisted d

Sex Influence on Trigeminal Nerve Stimulation and Breath-Hold Diving Performance: Examination of the Autonomic Regulation of Cardiovascular Responses to Facial Cooling and Apnoea Across Sex and Varied Factors.

This review emphasises the importance of the cardiovascular response to facial cooling (FC) and breath holding in both sexes. The trigemino-cardiac reflex, triggered by FC, reduces heart rate (HR) and constricts blood vessels. When combined with breath holding, this effect intensifies, enhancing the cardiodepressive impact. The cardiovascular reaction to this combination, known as the cold-water face immersion or simulated diving test, varies among individuals and depends on their cardiovascular regulatory profiles, which differ between men and women. Despite extensive research on the cardiova

The application of statistical methods using VOCs to identify patients with lung cancer.

In this work, an attempt was made to determine a group of lung cancer biomarkers. For this study, breath samples collected from 137 patients with confirmed lung cancer were analyzed by the SPME-GC/MS method. As a reference group, exhaled air from 143 healthy volunteers with different smoking habits (active smokers, passive smokers and nonsmokers) was applied. Statistical methods such as discriminant analysis (DA) and the CHAID model tree were used for data processing and evaluation. In the breath of patients with lung cancer, increased concentration of ethanol, acetone, butane, dimethyl sulfid

ADAM phantom to test 4D medical imaging and dose delivery devices.

Radiotherapy treatment of moving lesions is a challenging task in which different strategies can be used. The clinical implementation of the variety of complex technical solutions pursued to treat cancer, while sparing healthy tissues, requires accurate quality assurance tests and adequate phantoms. The aim of this work is to report on the anthropomorphic dynamic breathing model (ADAM) and on its applications to test image quality and dose delivery for four-dimensional (4D) techniques used to compensate for breathing-induced tumor motion. ADAM reproduces a male torso containing realistic ribs

Extreme altitude-induced central sleep apneas lasting more than 100 seconds in a healthy 23-year-old man.

Central sleep apneas (CSA) can occur de novo at high-altitude in individuals without sleep-disordered breathing at low altitude. These apneas are usually brief, lasting only 5-15 s. This report presents the first documented case of a man experiencing extreme altitude-induced CSA lasting more than 100 s in the absence of any sleep breathing disorder in normoxia. A 23-year-old male with no pre-existing health conditions was recruited for a study examining the work of breathing during sleep at a simulated altitude of 3500 m (FiO2:13%). A lowland polysomnography was first conducted to exclude mode…

Ventilatory control of the 'isocapnic buffering' region in rapidly-incremental exercise.

During incremental exercise PCO2 does not fall for several work rates ('isocapnic buffering') above the anaerobic threshold (theta an). We explored this apparent lack of compensatory hyperventilation in 24 normal subjects who underwent incremental cycling (15 W/min) to exhaustion. Ventilation, pulmonary gas exchange, and end-tidal gas tensions were computed breath-by-breath. In 10 subjects, arterial blood was sampled every 2 min throughout the test. Our findings confirmed the 'isocapnic' supra-theta an region, but it consistently followed a progressive increase of PETCO2 in the sub-theta an re

Tracheal pressure regulated volume assist ventilation in acute respiratory failure.

PURPOSE: Proportional assist ventilation (PAV) uses volume assist (VAV) and flow assist ventilation (FAV) to reduce elastic and resistive effort, respectively. Proportional assist ventilation may be difficult to apply clinically, particularly due to FAV related considerations. It was hypothesized that regulating tracheal (Ptr) rather than airway opening pressure (Pao), to overcome endotracheal tube related resistive effort, during VAV would provide an effective alternative method of ventilation. We therefore compared the effects of Pao and Ptr regulated VAV on breathing pattern and inspiratory

Management of respiratory failure in infants with acute viral bronchiolitis.

Fifteen infants with acute viral bronchiolitis required mechanical ventilation. Infants were all aged less than 12 weeks and all had evidence of atelectasis or pneumonia on chest x-ray films. Respiratory syncytial virus was identified by immunofluorescence in 14 of 15 patients. Intubation and mechanical ventilation were initiated to reduce the work of breathing rather than to treat hypoxemia alone. Additional therapy included continuous positive airway pressure or positive end-expiratory pressure, sedation, aminophylline, diuretics, fluid restriction, and early feeding.

Neural Respiratory Drive During Different Dyspnea Relief Positions and Breathing Exercises in Individuals With COPD.

BACKGROUND: When the work load of the respiratory muscles increases and/or their capacity decreases in individuals with COPD, respiratory muscle activation increases to maintain gas exchange and respiratory mechanics, and perception of dyspnea occurs. The present study aimed to compare diaphragm and accessory respiratory muscle activation during normal breathing, pursed-lip breathing, and breathing control in different dyspnea relief positions, supine and side lying. METHODS: A cross-sectional study design was used. Sixteen individuals with COPD age between 40-75 y were included. Pulmonary fu

Synthesis of Nano-Praseodymium Oxide for Cataluminescence Sensing of Acetophenone in Exhaled Breath.

In this work, we successfully developed a novel and sensitive gas sensor for the determination of trace acetophenone based on its cataluminescence (CTL) emission on the surface of nano-praseodymium oxide (nano-Pr6O11). The effects of working conditions such as temperature, flow rate, and detecting wavelength on the CTL sensing were investigated in detail. Under the optimized conditions, the sensor exhibited linear response to the acetophenone in the range of 15-280 mg/m3 (2.8-52 ppm), with a correlation coefficient (R2) of 0.9968 and a limit of detection (S/N = 3) of 4 mg/m3 (0.7 ppm).
