OBJECTIVES: An annual screening cystoscopy protocol was begun at our institution in an attempt to minimize the morbidity and mortality of bladder cancer in the chronically catheterized spinal cord injured (SCI) population. An 11-year old girl suffered a diffuse axonal injury after car accident. On admission, she was unconscious (GCS 4) and presented with brain contusion, pulmonary contusion, severe tetraparesis, spasticity and seizures. She did not respond to sedatives and anti-epileptic drugs. After 18 days at Intensive Care Unit, she was selected to baclofen screening. She received 100 microg of baclofen and her spasticity decreased and motor and sensor aphasia resolved.

We describe the management of a 23-yr-old woman with extreme needle and mask phobia, presenting for an emergency Caesarean section for fetal distress. She also suffered from spina bifida cystica with no sensation from mid thigh. Regional anaesthesia, rapid sequence induction, and gaseous induction were not possible.
