Cyclosporine in Dermatology.

Cyclosporine, a member of the calcineurin inhibitor family, has immunomodulatory effects on T cells. It blocks the earliest stages of the immune response by binding to cyclophilin, thus inhibiting calcineurin. It was initially used for transplant patients to prevent transplant rejections. The main indication for cyclosporine in dermatology is severe psoriasis. Other dermatologic indications for the use of cyclosporine include atopic dermatitis, chronic idiopathic urticaria, pyoderma gangrenosum, and Behçet's disease.

Eczema herpeticum in pregnancy.

Eczema herpeticum (EH), or Kaposi's varicelliform eruption, is a skin infection with herpes simplex type I virus (HSV-1) that occurs in patients with compromised skin integrity, such as atopic dermatitis (AD). Unrecognized, it may be fatal and viremia in pregnancy may lead to fetal demise and miscarriage. We describe a rare case of EH in pregnancy, eczema herpeticum gravidarum (EHG), which is the third published report in the literature to date.

Mycophenolate mofetil and skin diseases.

Mycophenolate mofetil (MMF) is an immunosuppressive drug the efficiency of which has been established in renal transplantation. Recent studies suggest that it may also be effective in the treatment of variant skin diseases especially if the skin lesions are triggered by lymphocytes. Studies have shown efficacy in autoimmune bullous dermatoses, atopic dermatitis and psoriasis. However, there are no placebo-controlled trials that support the use of MMF as first line therapy in these skin diseases.

Common facial skin lesions in children.

A variety of facial skin problems are encountered in a busy pediatric dental practice. The dentist should recognize and be able to identify the more common important skin lesions. The etiology, presentation, and prognosis of acne vulgaris, impetigo, warts, molluscum contagiosum, ringworm, tinea versicolor, spider nevus, atopic dermatitis, seborrhea, herpes simplex, and lesions of child abuse are discussed. Several of these conditions require prompt referral to a pediatrician or dermatologist.

Is the skin affected by work at visual display terminals?

According to some reports, mainly from Norway and Sweden, visual display terminal work is suspected of causing skin rashes. In three different studies, we have tried to elucidate the question, and the results indicate that there might be a relation between VDT work and aggravation of some common skin diseases such as rosacea, seborrheic and atopic dermatitis, and acne. Whether this depends on physical, chemical, or psychological factors is still unknown.

Hand dermatitis in haridressers.

Most junior hairdressers, while shampooing, develop a dry irritant dermatitis over the metacarpophalangeal joints. It responds to simple treatment and disappears when the shampooing stage is passed. A few hairdressers develop eczema of the fingers: the response to treatment is less predictable and the prognosis is worse. In those who give up hairdressing the eczema generally improves or heals, in those who continue hairdressing the eczema is likely to persist, especially in atopics.

Indoor aeroallergens from American cockroaches and mites initiate atopic march via cutaneous contact in a murine model.

The progression of allergic diseases from atopic dermatitis in childhood to other allergic conditions such as asthma in later life is often referred to as the atopic march. In order to study the relationship between cutaneous sensitization by aeroallergen and atopic march, we established a mouse model to test the hypothesis using American cockroaches and house dust mites as the model allergens. Mice were sensitized via skin with native cockroach extract (CraA) or recombinant Per a 2 and Der p 2 proteins without adjuvant. Each mouse was subjected to a total of three 1-week patching sensitizatio

Prevalence of house dust mites in the homes of atopic dogs in Finland.

BACKGROUND: The house dust mites (HDM) Dermatophagoides farinae and D. pteronyssinus are important environmental allergens implicated in the pathogenesis of human and canine atopic dermatitis. Sensitization to HDM measured by allergen-specific IgE is common in Finnish atopic dogs. Studies on HDM prevalence in Finland are few but suggest that HDM are scarce. OBJECTIVE: The aim of the present study was to evaluate the prevalence of HDM in the home environments of atopic dogs in Finland. METHODS: Dust samples were obtained from the homes of 50 atopic dogs. Samples were collected by vacuuming th

Vitamin d, the cutaneous barrier, antimicrobial peptides and allergies: is there a link?

Atopic diseases such as atopic dermatitis (AD) are very common in industrialized countries. Up to 15%-30% of all children and 2%-10% of all adults suffer from AD. Already in early disease stages, a defective epidermal barrier is known to contribute to the pathogenesis of AD. Central elements in the epidermal barrier are antimicrobial peptides (AMPs), which are secreted by keratinocytes, sweat gland cells but also infiltrating immune cells. AMPs function as endogenous antibiotics and are able to kill bacteria, viruses, and fungi. Furthermore AMPs act as immune modulators with effects on the inn

Is there a role for fatty acids in early life programming of the immune system?

There may be a causal relationship between n-6 PUFA intake and allergic disease and there are biologically plausible mechanisms, involving eicosanoid mediators of the n-6 PUFA arachidonic acid, that could explain this. There is some evidence that high linoleic acid intake is linked with increased risk of atopic sensitisation and allergic manifestations. Fish and fish oils are sources of long-chain n-3 PUFA and these fatty acids act to oppose the actions of n-6 PUFA. It is considered that n-3 PUFA will protect against atopic sensitisation and against the clinical manifestations of atopy. All fi

Wood stove heating, asthma and allergies.

In two German studies household wood or coal stove use was negatively associated with atopic sensitization and allergic rhinitis in childhood. Wood stove heating is strongly related to 'traditional lifestyle and therefore subjected to confounding factors possibly yet not known. The study was conducted to study these factors and the independent impact of early exposure to wood stove heating on subsequent asthma and atopic disease. In a questionnaire survey among 10667 Finnish university students aged 18-25 years, we investigated the association between wood stove heating at age 0-6 years and as

Clinical and epidemiological features of textile contact dermatitis: an Italian multicentre study.

BACKGROUND: The prevalence of occupational and non-occupational textile dermatitis seems to be increasing, probably because of changed textile manufacturing techniques. OBJECTIVE: Evaluation of the clinical features and epidemiology of textile contact dermatitis (TCD) in Italy. METHODS: Demographic data, clinical history aspects, atopy and positive patch test reactions to occupational and non-occupational allergens were investigated in 277 textile dermatitis patients. RESULTS: Contact dermatitis was the most frequent clinical presentation (95.3%). TCD was more common in females, in the four

Why am I so itchy?

BACKGROUND: Virtually all of us experience an itch at some time. However, for some people, an intense or persistent itch can cause considerable morbidity that often affects the entire family. OBJECTIVE: This article gives an overview of the problem of itch, and discusses the assessment and management of atopic eczema, scabies, lichen planus and dermatitis herpetiformis. DISCUSSION: A good history will nearly always provide the diagnosis. Examination can then be targeted at finding specific signs. The distribution of rash is important, as is examination of the nails and mouth. Simple but help

[Kaposi-Juliusberg's syndrome complicating Darier's disease"].

INTRODUCTION: Kaposi-Juliusberg's syndrome is a severe herpes simplex virus cutaneous infection, accompanied by general signs. Darier's disease is a rare genodermatosis characterised by keratinisation disorders. OBSERVATION: In a 51 year-old patient suffering from Darier's disease since the age of 20, a diffuse vesicular eruption occurred following a herpes eruption on the lower lip. Culture of a vesicle revealed a type 1 Herpes simplex virus. The diagnosis of Kaposi-Juliusberg's syndrome was made. COMMENTS: Kaposi-Juliusberg's syndrome is a "classical" herpes complication of atopic dermatit

Pediatric dermatology.

Dr. Baruch presents some of the most common pediatric lower extremity skin conditions seen in clinical practice. The conditions that he discusses appear to be relatively easy to diagnose in the early stages; however, some of them, such as contact dermatitis and atopic dermatitis, may be hard to distinguish from one another. I have to admit that I have never seen lichen planus in the pediatric age group; I will need to keep looking. I agree with Dr. Baruch that classification of lesions into either the primary and secondary groups will aid in diagnosis and treatment. It also seems that the ever

Postbiotics in Dermatology: A Literature Review of Emerging Topical Therapies for Acne, Rosacea, and Eczema.

Postbiotics, defined as non-viable microbial cells and their metabolites, have emerged as topical therapies for inflammatory dermatoses and provide more clinical benefits than live postbiotics by eliminating infection risk and stability. This literature review aims to generalise existing knowledge about the effectiveness and mechanisms of action of first-line topical postbiotic treatments for acne, rosacea, and eczema. Search terms included "postbiotics" and keywords that represented the dermatological conditions of interest. Thematic synthesis was performed on the systematically extracted dat

Where the Future Lies: GLP-1 Receptor Agonists as Therapeutics for Dermatologic Comorbidities.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have transformed the management of obesity, type 2 diabetes mellitus, and cardiometabolic disease through effects on weight reduction, glycemic control, and systemic inflammation. Their varied immunometabolic actions have generated increasing interest in dermatology, where chronic inflammatory skin diseases frequently coexist with obesity, insulin resistance, metabolic syndrome, and cardiovascular disease. Emerging evidence from clinical trials, observational studies, and real-world analyses suggests that GLP-1RAs may improve outcomes in pso

The Role of Tissue Resident Memory Cells in Immunodermatological Disorders.

Tissue-resident memory T cells (TRMs) are a distinct memory T-cell subset that permanently reside in tissues such as the skin. In chronic immune-mediated dermatoses, including psoriasis, atopic dermatitis, and vitiligo, TRMs are implicated in site-specific "disease memory," contributing to persistence and relapse at previously affected sites. This review synthesizes emerging concepts on TRMs in chronic inflammatory skin diseases and discusses clinical and translational implications, including TRM-targeted therapies. Original human, animal, or in vitro studies evaluating TRMs in the pathogenesi

Eczema Severity Scoring in Skin of Color: A Review of Current Best Practice and Need for Future Improvement.

Patient- and investigator-reported outcome measures are used in research and clinical practice to assess the severity and impact of atopic dermatitis (AD). Initial validation studies of the most commonly used outcome measures for AD underrepresent patients with skin of color or fail to report race, ethnicity, and skin color altogether. Various adaptations have been proposed. Upgrading the erythema score by 1 when using the Eczema Area and Severity Index in patients with skin of color has been suggested but has not yet been validated. However, the use of a "grey scale" in place of the erythema
