Long-term follow-up of general immune competence in breast cancer. II. Sequential pre- and post-treatment levels: a 10 year study.

Pre-treatment and sequential post-treatment (at 3 months, 6 months, 1, 2, 3, 4 and 5 years) examination of general immune competence was performed in 185 consecutive breast cancer patients. The patients were followed for 5 to 11 years to monitor the dynamic relationship between host immunity and cancer and to examine the effect of the treatment method. The tests of immune competence used were immunoglobulins IgG, IgA, IgM, leucocyte counts, percentage and total lymphocyte counts and Mantoux and DNCB skin hypersensitivity tests. Serum IgG and IgA showed no change relating to treatment method in

Maxillomandibular advancement surgery for obstructive sleep apnoea patients: the current workflow from referral to recovery.

Obstructive sleep apnoea (OSA) is a common medical condition with significant morbidity and mortality, but frequently undiagnosed. Clinical signs and symptoms may be identified by general dental practitioners during routine dental examinations and patient history taking. This article explores the clinical characteristics, aetiology, and prevalence of OSA, and outlines when a referral should be considered, including the essential details to include in the referral. Additionally, it examines the current clinical pathway following referral, detailing the assessment and planning process for maxill

Use of a Superficial Abdominal Wall Vein in a Gravida Patient for Emergency Vascular Access.

Obtaining vascular access through a superficial vein of the abdominal wall of a gravida patient is an option in an emergency Cesarean surgery when other means fail. Such superficial veins may be mistaken for striae gravidarum on physical exam. A small intravenous (IV) cannula is not ideal but could save valuable time and avoid delaying induction of general anesthesia. Once the airway is secured, a larger bore IV can then be inserted while surgical exposure is undergoing. Analysis of the risks and benefits of inducing general anesthesia with a small gauge IV for a gravida patient should take in

Erector spinae plane block for thoracoscopy in a paediatric patient.

We present the case of a 9 year old patient with a clinical history of epilepsy and various hospitalizations due to aspirative broncopneumonia among others, who was admitted to our hospital because of septic shock secondary to pneumonia of the lower left lobule associated with a parapneumonic pleural effusion and a complicated clinical course. During her stay, the patient undergoes surgical debridement using video assisted thoracoscopic surgery (VATS) under general anaesthesia and the hydropneumothorax is drained. For the procedure an ecoguided erector spinae plane block is performed and combi

Intensive care and anesthesia management for HARPOON beating heart mitral valve repair.

Patients with severe mitral valve regurgitation secondary to degenerative disease are known to benefit from mitral valve repair surgery. Novel techniques for achieving mitral valve repair on the beating heart have been developed and are being introduced into clinical practice. The HARPOON Beating Heart Mitral Valve Repair System (MVRS) in recent studies has demonstrated efficacy and safety for the repair of degenerative mitral valve disease on the beating heart. The device uses transoesophageal echocardiographic guidance to implant artificial expanded polytetrafluoroethylene (ePTFE) cords on p

Tips and Tricks for Anterior Cranial Base Reconstruction.

Reconstruction procedures come last in skull base surgery, but they are not the least important phase-rather, reconstruction is one of the most important steps required to prevent complications. In our opinion, there are three general principles upon which a good reconstruction of the skull base stand: (1) anatomo-surgical knowledge; (2) approach/route selection; and (3) the cooperation of the skull base surgical team. In general, three major complications may occur when a good skull base reconstruction has not been achieved, i.e., cerebrospinal fluid (CSF) leak, pneumoencephalus, and infectio

Life threatening bleeding from duodenal ulcer after Roux-en-Y gastric bypass: Case report and review of the literature.

Acute upper gastrointestinal bleeding is a rare, but serious complication of gastric bypass surgery. The inaccessibility of the excluded stomach restrains postoperative examination and treatment of the gastric remnant and duodenum, and represents a major challenge, especially in the emergency setting. A 59-year-old patient with previous history of peptic ulcer disease had an upper gastrointestinal bleeding from a duodenal ulcer two years after having a gastric bypass procedure for morbid obesity. After negative upper endoscopy finding, he was urgently evaluated for gastrointestinal bleeding. A

Early release of interalveolar synechiae under general anesthesia through fiberscopic nasal intubation.

This article presents a treatment strategy for early release of interalveolar synechiae, aiming to facilitate early oral feeding and prevent temporomandibular joint ankylosis. The treatment results of 2 patients with van der Woude syndrome were retrospectively studied. Both patients underwent early surgical release of interalveolar synechiae under general anesthesia through fiberscopic nasal intubation. The 2 patients were treated at the ages of 6 and 14 days, respectively. The interincisival distances increased from 5 and 6 mm preoperatively to 11 and 10 mm immediately after surgery. This was

[Cystoscopy in a heifer with rupture of a patent urachus].

This case report describes the clinical, ultrasonographic and cystoscopic findings and treatment in a two-year-old Swiss Braunvieh heifer with rupture of a patent urachus. The lead signs in the seven-month-pregnant heifer were markedly abnormal general condition and demeanour and a pear-shaped abdomen. The heifer had severe azotaemia, and abdominal ultrasonography revealed ascites, which was diagnosed as uroperitoneum based on an elevated creatinine level in the fluid. A patent urachus was identified during cystoscopy; the endoscope could be advanced beyond the apex of the urinary bladder into

[Problems posed by spinal anesthesia in a patient with Gélineau disease].

A 44-year-old patient, with narcolepsy-cataplexy, underwent surgery for lumbar disk hernia under spinal anaesthesia. Our purpose was to prevent an interaction between the patient's disease and general anaesthetic agents with the risk of postoperative hypersomnia. During surgical procedure, two narcolepsy fits occurred, without clinical consequences. The postoperative course was uneventful. However, spinal anaesthesia cannot be considered as a technique of choice because of the risk of narcolepsy-cataplexy fits with loss of consciousness and atonia, during regional anaesthesia. General anaesthe

A 27-year-old female patient with partial Alagille's syndrome (hypoplasia of the intrahepatic bile ducts, typical facial dysmorphism and skeletal anomalies) developed a marked cholestasis (total bilirubin 59 mg/dl), decrease of liver synthesis tests and ascites four weeks after gynecological surgery with general anesthesia involving propofol, isoflurane and nitrous oxide. Slow recovery could be achieved under treatment with ursodeoxycholic acid, spironolactone and substitution of fat-soluble vitamins A, D, E and K. Four months after admission the ascites had disappeared, liver synthesis had in

[Psychiatric disorders in hospitalized internal medicine and surgical patients. Prevalence and need for treatment].

The prevalence of psychiatric disorders in general hospital inpatients can be estimated at 30-50%. In Germany, hardly any studies have provided an assessment of prevalence rates and specific treatment necessities. In this study, 400 patients from medical and surgical departments were interviewed with a structured clinical interview. The point prevalence of all psychiatric disorders (clinical diagnoses) was 46.5%. The most prevalent groups of disorders were organic brain syndromes, depressive disorders, and alcoholism. With regard to disease entities, no significant differences between the samp

Comparison of percutaneous endoscopic gastrostomy with Stamm gastrostomy.

In a review of 125 percutaneous endoscopic gastrostomies (PEG) and 88 Stamm gastrostomies performed at Duke University Medical Center since 1978, the average operating room time for PEG (50 +/- 20 min) was shorter than for Stamm (96 +/- 26 min) (p less than 0.0001). General anesthesia was administered in only 13% of PEG placements compared with 64% of Stamm gastrostomies. The cost of PEG was about $1000 less than for Stamm gastrostomies. The average time after surgery until use of the feeding tube was 1.8 days for PEG compared with 3.4 days for Stamm (p less than 0.0001). The overall complicat

Absence of recall after general anesthesia: implications for theory and practice.

To investigate the possibility that recall of auditory information occurs following general anesthesia, letter-work pairs were presented to 36 patients receiving light planes of general anesthesia for short operations. Postoperative testing which tapped auditory and visual memory demonstrated no evidence of recall. In a subsequent study, a single work was repeated during brief obstetric procedures in 12 patients. Postoperative testing was designed to maximize apparent recall. The only patient able to remember the experimental word had awakened during anesthesia. Even without the use of more so

[Routine diagnostics prior to ear surgery-results of a survey among otorhinolaryngology departments in German-speaking countries. German version].

BACKGROUND: Diagnostic testing prior to ear surgery is an important part of the work of ear specialists in hospitals. To date, there is no national or international diagnostic standard. Rather, the test batteries used are dependent on hospitals' own doctrinal opinions. OBJECTIVE: The aim of this study was to conduct a survey in German-speaking countries to determine the routine diagnostic methods used prior to various ear surgery procedures. METHODS: An online questionnaire with 18 general questions on the hospital's equipment and availability of tests and 6 questions on specific ear operati

The Application of Sleep Deprivation Combined With Chloral Hydrate on Pediatric Dermatological Surgery.

BACKGROUND: In small dermatological surgeries for infant and child patients, it is required that the child be in a quiet state. However, general anesthesia not only requires the participation of professional anesthesiologists for surgery but also has a high cost and anesthesia risks. Parents have a low acceptance rate. At present, chloral hydrate combined with local anesthesia for sedation has been widely used in various pediatric examination and treatment projects. OBJECTIVE: To retrospectively analyze the impact of sleep deprivation on pediatric dermatological surgery performed under local

Splenic Artery Steal Syndrome in Patients with Liver Cirrhosis: A Retrospective Clinical Study.

BACKGROUND Splenic artery steal syndrome (SASS) can aggravate liver damage in patients with cirrhosis. This study explored whether SASS could be an effective therapeutic target for improving hepatic artery perfusion and liver function in patients with decompensated cirrhosis. MATERIAL AND METHODS Based on inclusion and exclusion criteria, 87 patients with hepatitis B cirrhosis and portal hypertension hypersplenism admitted to our General Surgery Department for splenectomy and pericardial devascularization surgery were selected. A total of 35 cases met the diagnostic criteria of SASS and were a…

The impact of warfarin on operative delay and 1-year mortality in elderly patients with hip fracture: a retrospective observational study.

BACKGROUND: Guidelines underline the importance of early surgery in elderly patients with proximal femoral fractures. However, most of these patients present a high number of comorbidities, some of which require the use of warfarin. Waiting for INR decrease is a cause of surgical delay, and this influences negatively their outcome. METHODS: We retrospectively reviewed all patients with proximal femoral fracture admitted to our unit from March 2013 to March 2017 to determine whether warfarin therapy is associated with reduction of survival, delay of surgery, and increased blood loss. From 1706
