Concurrent minimal change disease and retroperitoneal liposarcoma successfully treated by tumor resection and steroid therapy.

A 54-year-old Japanese woman developed simultaneous abdominal distension and bilateral leg edema. Her medical history and results of periodic medical check-up were unremarkable. Blood tests revealed severe hypoproteinemia and acute kidney injury, and urinalysis revealed 4+ proteinuria and 2+ hematuria. Abdominal computed tomography revealed a large intra-abdominal mass with fat tissue density. She underwent emergency tumor excision, splenectomy, and distal pancreatectomy. However, hypoproteinemia and acute kidney injury worsened. Therefore, she was transferred to the nephrology division for co

Preliminary Evaluation of Knee Kinetics in Female Athletes on Hormonal Contraceptives.

Recently, an emphasis has been placed on understanding how ovarian sex hormones and hormonal contraceptives affect risk for anterior cruciate ligament (ACL) injury. The literature presents large discrepancies in whether or not hormonal contraceptives affect ACL injury risk; therefore, the purpose of this study was to evaluate whether vertical ground reaction force (GRF) and knee valgus force are different between athletes who do and do not use hormonal contraceptives. Twenty-two female athletes volunteered to participate and were divided into two groups based on their answers to a health histo

Altered lower extremity joint mechanics occur during the star excursion balance test and single leg hop after ACL-reconstruction in a collegiate athlete.

The effects of ACL-reconstruction on lower extremity joint mechanics during performance of the Star Excursion Balance Test (SEBT) and Single Leg Hop (SLH) are limited. The purpose of this study was to determine if altered lower extremity mechanics occur during the SEBT and SLH after ACL-reconstruction. One female Division I collegiate athlete performed the SEBT and SLH tasks, bilaterally, both before ACL injury and 27 months after ACL-reconstruction. Maximal reach, hop distances, lower extremity joint kinematics and moments were compared between both time points. Musculoskeletal simulations we

Traumatic axonal injury despite clinical phenotype of mild traumatic brain injury: a case report.

OBJECTIVES: We report on a patient who suffered traumatic axonal injury (TAI) of various neural tracts despite airbag deployment following mild traumatic brain injury (TBI), which was demonstrated by diffusion tensor tractography (DTT). CASE DESCRIPTION: A 58-year-old female patient suffered from head trauma resulting from an in-car traffic accident. At the time of head trauma, her head and face hit the deployed airbag after flexion-hyperextension-rotation injury. The patient's Glasgow Coma Scale score was 15. Since the day of head trauma, she began to feel headache and upper back pain at the

A Finite Element Model of the Foot/Ankle to Evaluate Injury Risk in Various Postures.

The foot/ankle complex is frequently injured in many types of debilitating events, such as car crashes. Numerical models used to assess injury risk are typically minimally validated and do not account for ankle posture variations that frequently occur during these events. The purpose of this study was to evaluate a finite element model of the foot and ankle accounting for these positional changes. A model was constructed from computed tomography scans of a male cadaveric lower leg and was evaluated by comparing simulated bone positions and strain responses to experimental results at five postu

[Verapamil in conjunction with pressure therapy in the treatment of pathologic scar due burn injury].

BACKGROUND: Keloids and hypertrophic scars are dermal fibro-proliferative disorders unique to humans. Their treatment is a true challenge with multiple options, but not all the time with good results. Unfortunately this problem is not uncommon in patients with history of burn injury. The aim of this article is to evaluate the use of verapamil and pressure garments in patients with hypertrophic or keloid scar caused by burn injury. METHODS: We included patients with a hypertrophic or keloid scar caused by burn injury candidate to treatment with pressure garment. The pathologic scars were evalu

The effect of ankle posture on the load pathway through the hindfoot.

The foot-ankle complex is frequently injured in a wide array of debilitating events such as car crashes. Numerical models and experimental tests have been used to assess injury risk, but most do not account for the variations in ankle posture that frequently occur during these events. In this study, the positions of the bones of the foot-ankle complex (particularly, the hindfoot) were quantified over a range of postures. Computed tomography scans were taken of a male cadaveric leg under axial loading with the ankle in five postures in which fractures are commonly reported. The difference in th

What are the ethical issues in treating a patient with bilateral leg gangrene incapable of consenting to amputation secondary to psychiatric illness?

BACKGROUND: Bilateral and simultaneous lower extremity amputations unrelated to diabetes and peripheral vascular disease are uncommon, although they may be necessary in patients with severe blast injuries. Such amputations, however, usually are performed in patients who are able to understand and consent to the treatment. CASE DESCRIPTION: We present the case of a 29-year-old woman who experienced drug-induced psychosis and underwent substantial hypothermic injuries; most notably, irreversible frostbite injuries to both lower extremities, leading to dry and mummified gangrene. As a result of

Acute compartment syndrome.

BACKGROUND: acute compartment syndrome (ACS) is one of the few true emergencies in orthopedics and traumatology. It is a painful condition caused by the increase interstitial pressure (intracompart-mental pressure - ICP) within a closed osteofascial compartment which impair local circulation. It occurs most often in the legs, but it can affects also the arms, hands, feet, and buttocks. It usually develops after a severe injury such as fractures or crush injury, but it can also occurs after a relatively minor injury and it may be iatrogenic. Uncommon causes of ACS have been also described, that

Laparoscopic repair of a ureter damaged during inguinal herniorrhaphy.

BACKGROUND: Laparotomy has been used generally to deal with ureteral injury. Recently, a few papers have reported the repair of gynecologic ureteral injuries by laparoscopy, with encouraging results; however, successful repair of a ureter damaged during an open inguinal herniorrhaphy fully performed by laparoscopy has not been reported yet. PATIENT: A 49-year-old obese man (body mass index=35) in the 10 years before surgery began to note a protrusion in the scrotal region. The protrusion evolved asymptomatically for 8 years, and began to cause pain in the lumbar spine to the right and paresth

Knee joint laxity and neuromuscular characteristics of male and female soccer and basketball players.

Anterior cruciate ligament injuries are occurring at a higher rate in female athletes compared with their male counterparts. Research in the area of anterior cruciate ligament injury has increasingly focused on the role of joint proprioception and muscle activity in promoting knee joint stability. We measured knee joint laxity, joint kinesthesia, lower extremity balance, the amount of time required to generate peak torque of the knee flexor and extensor musculature, and electromyographically assessed muscle activity in 34 healthy, collegiate-level athletes (average age, 19.6 +/- 1.5 years) who

Females Have Earlier Muscle Strength and Functional Recovery After Bridge-Enhanced Anterior Cruciate Ligament Repair.

Background: While a sex effect on outcomes following anterior cruciate ligament (ACL) reconstruction surgery has been previously documented, less is known following bridge-enhanced ACL repair (BEAR). We hypothesized that female sex would have significantly worse early functional outcomes and higher retear rates following primary repair of the ACL enhanced with a tissue-engineered scaffold. Methods: Sixty-five patients (28 males and 37 females), age 14-35 with a complete ACL tear underwent primary repair of the ACL enhanced with a tissue-engineered scaffold (bridge-enhanced ACL repair) within 4

Study of aeroball injuries.

OBJECTIVE: To present the risks of aeroball, a new sport played by either two or four players on a trampoline court surrounded by specially constructed fabric walls, and to propose ways to increase awareness and reduce the incidence of injury, in particular, ankle injury. METHOD: A study was carried out to document the nature of aeroball related incidents, between 1991 and 1995, at Lancaster University Sports Centre. Lace-up ankle supports were introduced in April 1992, and their effect on the incidence of ankle injury was recorded. RESULTS: The lower limb received most injuries (90%), follo

Posterior tibial artery-based multilobar combined flap free transfer for repair of complex soft tissue defects.

In this report, the posterior tibial artery (PTA) based multilobar combined flap is introduced for the repair of complex soft tissue defects. The flap was designed based on the perforatoring branches of PTA in the anterior soleus muscle septum, which supply the skin over the medial side of the calf and the entire soleus muscle. The saphenous nerve was included in one perforator flap of the combined flap for reinnervation. The tibial artery was repaired with a vein graft after harvest of flap. From October 2005 to February 2007, eight patients (6 males, 2 females) underwent PTA-based multilobar

[Paragliding-associated bilateral partial rupture of the rectus femoris muscle].

Pain in the thigh or groin due to a rupture of the rectus femoris muscle is rather uncommon. We report on a patient with a bilateral rupture of the rectus femoris muscle that occurred due to a landing maneuver while para-gliding. The diagnosis was confirmed by ultrasound and MRI. Additionally, an old unilateral anterior cruciate ligament rupture was present. As a functional deficit of the quadriceps muscle could not be observed, a primarily conservative treatment seems to be appropriate.

Functional movement screen differences between male and female secondary school athletes.

The functional movement screen (FMS) is commonly used to assess movement capacity and determine injury risk. Evidence suggests that athletes who score 14 points or less on the FMS are at increased risk for injury, but differences between males and females have been minimally studied. The purpose of this study was to investigate sex differences in FMS scores of secondary school athletes. Using a cross-sectional study design, 60 healthy secondary school athletes performed the FMS, which is composed of 7 functional movement tasks (deep squat, hurdle step, inline lunge, shoulder mobility, active s

The efficacy of neuromuscular electrical stimulation with alternating currents in the kilohertz frequency to stimulate gait rhythm in rats following spinal cord injury.

BACKGROUND: Rehabilitation facilitates the reorganization of residual/regenerated neural pathways and is key in improving motor function following spinal cord injury. Neuromuscular electrical stimulation (NMES) has been reported as being clinically effective. Although it can be used after the acute phase post-injury, the optimal stimulation conditions to improve motor function remain unclear. In this paper, we examined the effectiveness of NMES with alternating currents in the kilohertz (kHz) frequency in gait rhythm stimulation therapy. METHODS: Tests were performed using 20 mature female Fi
