Papillary carcinoma of the thyroid with manifestations resembling Graves' disease.

OBJECTIVE: To describe a case of papillary carcinoma of the thyroid that mimicked a diffuse toxic goiter. METHODS: We present a detailed ca … this nodule revealed papillary carcinoma of the thyroid. Total thyroidectomy and a modified radical neck dissection were performed. Microscopic examination showed that the entire gland was replaced by a papillary carcinoma with a predominantly follicular pattern of growth. Of 44 lymph nodes tested, 31 were positive for metastatic papillary thyroid carcinoma. The …

Superior laryngeal nerve preservation in peri-apical surgery by mobilization of the viscerovertebral angle.

Iatrogenic lesions of the superior laryngeal nerve (SLN) are much more common than is generally recognized. Since injury to this nerve may cause transient or even persistent changes either in quality of voice or in deglutition, an attempt should be made to localize and identify the nerve during surgery. This study included 74 patients who underwent surgical dissection near the thyroid apex in the neck. Effective prevention of SLN injury during surgery was achieved by anatomical localization of the nerve in the viscerovertebral angle and its functional identification with the nerve stimulator.

[The retro-auricular, transmastoid, infralabyrinth approach. A simple route for excision of tumors of the jugular foramen].

BACKGROUND: The jugular foramen is a complex area of the skull base. Its contents, the anatomical relationships in the region, and its locat … ferior to the labyrinth and medial to the third portion of the facial nerve, combined with a simple neck dissection, allows the surgeon to nicely expose the jugular foramen. The surgical procedure is described, following a brief anatomical reminder of the essential relationships in the area. A clinical observation is used to illustrate the purpose. RESULTS: Most …

Iodine concentration by the thymus in thyroid carcinoma.

A 14-yr-old boy underwent a total thyroidectomy with bilateral neck dissection for a papillary carcinoma with lymph node metastases. Total-body scanning with 3.7 GBq 131I revealed radioiodine accumulation in the anterior mediastinum. CT and MRI demonstrated a mediastinal mass which corresponded to the area of increased radioactivity. Five months later, another therapeutic dose of 131I was followed by a sternotomy and removal of the thymus because a hand-held radiodetecting surgical probe demonstrated that the thymus was the mediastinal structure which concentrated iodine. Thymus histology was …

Tumors of the paranasal sinuses: a therapeutic challenge.

Cancer involving the ethmoid and sphenoid frontal sinus complex can be successfully eradicated by a combined transcranial and transfacial surgical dissection. Survival rates of 44 to 58 percent with a 3 percent hospital mortality rate in patients whose previous surgery or radiotherapy was largely unsuccessful suggest that this cosmetically acceptable surgical endeavor should be used more often by the head and neck surgeon in treating paranasal sinus cancer. Utilizing the principles of antibiotic prophylaxis, strict attention to principles of tumor removal and surgical technique, and the talent

Prognostic factors for melanoma patients with lesions 0.76 - 1.69 mm in thickness. An appraisal of "thin" level IV lesions.

Fourteen variables were tested for their prognostic usefulness in 203 patients with clinical Stage I melanoma and primary tumor 0.76-169 mm thick. Only two variables, primary tumor location and level of invasion, were useful in predicting death from melanoma for these patients. Of the 12 deaths from melanoma, 11 occurred in patients with primary tumors located on the upper back, posterior arm, posterior neck, and posterior scalp (=BANS). There has been only one death from melanoma in 136 patients with melanoma located at other sites (11/67 vs 1/136, p less than 0.0001 Fisher's Exact Test). Of

Concurrent Reversible Cerebral Vasoconstriction Syndrome and Bilateral Vertebral Artery Dissection in the Postpartum Period: A Case Report.

Reports of postpartum women with concurrent reversible cerebral vasoconstriction syndrome (RCVS) and bilateral vertebral artery dissection (VAD) are rare. Guidelines on the management of future pregnancies in these patients are also limited. We present a case of a 33-year-old female who developed a severe, sudden-onset headache 2 h after a vaginal birth in a maternity hospital. The severe headaches recurred and were associated with neck pain, nausea, vomiting, and photophobia. Preliminary neuroimaging only included a noncontrast CT brain. This was normal and she was treated with triptans for p

Merkel cell carcinoma (MCC) is a rare, aggressive neuroendocrine skin malignancy. We report an exceptional case of a 71-year-old female presenting with a 5.7-cm MCC in the left parotid region, representing a rare case of true direct parotid parenchymal invasion-distinguished from the more common intraparotid nodal metastasis-alongside multiple regional nodal metastases. The patient underwent wide en bloc excision, superficial parotidectomy, and selective neck dissection. Despite tumor adherence, the main facial-nerve trunk was successfully preserved. The resulting 12×11 cm composite defect was…

Transoral robotic surgery for human papillomavirus-positive oropharyngeal squamous cell carcinoma: a retrospective observational study from a UK tertiary centre.

BACKGROUND: Human papillomavirus (HPV)-positive oropharyngeal squamous cell carcinoma (SCC) has favourable oncological outcomes, supporting the use of transoral robotic surgery with risk-adapted adjuvant therapy. While anticipated adjuvant treatment intensity is informed by pre-operative assessment, definitive post-operative histopathology determines final risk stratification. METHODS: A single-centre study was conducted at University College London Hospitals of 44 HPV-positive oropharyngeal SCC patients treated with primary transoral robotic surgery and selective neck dissection. Rare Bilateral Axillary Arch With Its Novel Insertion Into the Deltoid and Unilateral Supernumerary Head of the Sternocleidomastoid Muscle in a Single Cadaver.

Two rare and clinically significant anatomical variations were observed in a single cadaver during routine dissection at the Department of Cellular Biology and Anatomy of Louisiana State University (LSU) Health Shreveport in Shreveport, Louisiana. We report a rare case of a bilateral axillary arch muscle. On the right side, the muscle originated from the latissimus dorsi and merged with the short head of the biceps brachii, deltoid, and coracobrachialis muscles. On the left, it originated from the latissimus dorsi but was inserted into the fascia between the coracobrachialis and deltoid muscle

Simple modified silicone rubber injection technique in fresh cadaveric pelvis and extremities.

The silicone rubber injection technique was first described in 1999 and has been used in the vascular study of neurology and head and neck dissection. Silicone rubber is durable, flexible, and inexpensive. However, the original silicone rubber injection formula perfuses poorly into the pelvis and extremities. We present a simple modification to the silicone rubber injection technique, showcasing its effectiveness in studying the vascular structures in the pelvis and extremities. We used an ordinary mold-making silicone rubber. The new formula involves mixing the silicone rubber with silicone t…

A Rare Case of Submandibular Gland Squamous Cell Carcinoma: Diagnostic Challenges and Surgical Management.

Squamous cell carcinoma (SCC) in salivary glands is rare, often posing diagnostic challenges. This case report presents a 75-year-old male with progressively increasing swelling and pain in the right submandibular gland, eventually diagnosed as moderately differentiated SCC. The diagnostic journey involved fine needle aspiration cytology, imaging studies, and histopathological examination. The patient underwent surgical intervention, including submandibular gland excision and selective neck dissection, which successfully achieved local control. Subsequent postoperative follow-up indicated a fa…
