Treatment of hypertrophic palatine tonsils using bipolar radiofrequency-induced thermotherapy (RFITT.).

CONCLUSION: Radiofrequency volume reduction of palatine tonsils is a gentle and safe treatment method in selected patients, which should carefully be considered as an alternative to tonsillectomy or tonsillotomy. OBJECTIVES: The aim of this study was the evaluation of bipolar radiofrequency-induced thermotherapy (RFITT) compared to standard blunt dissection tonsillectomy (TE) for the volume reduction of palatine tonsils in chronic tonsillar hypertrophy. PATIENTS AND METHODS: A total of 137 patients (98 children) were treated in two groups in a prospective controlled, randomized clinical tria

Analysis of functional results and quality of life following free jejunal flaps for reconstruction after upper aerodigestive neoplastic resection: the St James's experience.

Surgical treatment of hypopharyngeal cancers with extension to the postcricoid region generally requires a circumferential pharyngolaryngoesphagectomy followed by reconstruction of the upper aerodigestive tract. Many techniques have been described in order to achieve a safe and functional reconstruction. Interposition of the jejunal free flap (JFF) is a well-established technique and is the flap of choice in our unit. This is a retrospective review of all patients who required a JFF following pharyngolaryngoesphagectomy over an 9-year period. We studied medical charts, histological reports, an

Brain stem ischemia from intracranial dural arteriovenous fistula: case report.

BACKGROUND: Intracranial dural arteriovenous fistulas (AVFs) with spinal perimedullary venous drainage are rarely reported, but most of the patients initially have presented with myelopathy or subarachnoid hemorrhage. This is the first report of the intracranial dural AVF patient who presented with brain stem infarction. CASE DESCRIPTION: A 38-year-old woman experienced nausea and vomiting with an acute onset, followed by vertigo. Magnetic resonance imaging showed ischemic lesion in the medulla oblongata, and she was then sent to our hospital. On admission, she had nystagmus, swallowing diffi

[Cocaine smuggling in the gastrointestinal tract--the case report with the review of literature].

Body-packing is the way of psychoactive substances smuggling by swallowing of carefully prepared packages with drugs into the gastrointestinal tract or by insertion them into the vagina or the rectum, especially in order to avoid finding them by the custom service. Cocaine, as well as opiates, is the one of the most often smuggled drugs by so called body-packers. In the present study the first case of the body-packer from Malopolska region in Poland, who was observed in the Toxicology Department of the Collegium Medicum UJ, was described. The 29-year-old man swallowed 60 packages containing co

[Bilateral hypoxic-ischaemic thalamic lesions in newborns].

Bilateral hypoxic-ischaemic thalamic lesions are a rare type of neonatal encephalopathy with characteristic but often misinterpreted clinical features. This article describes history, clinical and diagnostic findings in a preterm and a term infant with hypoxic-ischaemic thalamic lesions after severe and abrupt intrauterine hypoxia. Both neonates presented with absent suckling and swallowing whereas other cranial nerves were unaffected. Characteristic findings in both newborns were profound muscular hypotonia and weak facial expressions together with feeding difficulties and frequent episodes o

Dissecting intramural haematoma of the oesophagus.

Dissecting intramural haematoma of the oesophagus is an under-recognized cause of sudden onset chest pain. Diagnosis is aided by the common co-existence of haematemesis, odynophagia, and dysphagia. Oesophagoscopy, barium swallow and cross-sectional radiology are all appropriate modalities of investigation. The latter is useful in excluding aortic dissection from the differential diagnosis. The condition usually follows a benign course with spontaneous healing requiring supportive treatment only. A systematic analysis of all the cases of dissecting intramural haematoma of the oesophagus (DIHO)

[Plastic reconstructive surgery of soft palate defects--functional and oncological aspects].

BACKGROUND: Resection of the soft palate in tumor surgery can lead to significant swallowing disorders. Therefore rehabilitation needs a functional reconstruction of the remaining defects. MATERIALS AND METHODS: In four years, nine patients received a partial removal, and six patients a total removal of the soft palate including adjacent parts of the oropharynx in six cases due to oropharyngeal carcinoma. Partial defects were reconstructed with a neurovascularized infrahyoidal muscle flap, total defects with fasciocutaneous flaps of the lateral arm, radial forearm or scapula region. The funct

Clinical evaluation of an infusion of xylazine, guaifenesin and ketamine for maintenance of anaesthesia in horses.

General anaesthesia was maintained in 40 horses by the continuous intravenous infusion of a mixture of 500 ml 10% guaifenesin solution, 1 g ketamine and 500 mg xylazine. The median duration of infusion was 65 mins (interquartile range 51-95 mins). Anaesthesia was characterised by active palpebral reflexes, variable degrees of nystagmus and occasional swallowing. Swallowing was considered to be undesirable in horses undergoing laryngeal surgery, but the infusion produced satisfactory conditions for all other surgical procedures performed. Arterial blood pressure was well maintained in all cases

[Computerized manometry and deglutition after pharyngolaryngeal tumor resections].

33 patients were examined after laryngopharyngeal surgery by means of computer manometry using 4-channel-pressure probes. After local tumour resection in the region of tonsils and lateral oropharyngeal wall a slight cranial decrease in pressure results. Resections of the soft palate, glossotonsillar groove, base of the tongue, and of the vallecula lead to a cranial release of pressure reducing the driving force of the tongue. The swallowing action is therefore delayed and completely uncoordinated resulting in dyskinesia of the PE segment. Stenosis in the PE segment after hypopharyngeal resecti

[Rehabilitation of deglutition in patients with pharyngeal carcinoma].

Surgical treatment of pharyngeal carcinoma implies different reconstructive procedures. Despite primary healing, in some cases cicatricial shrinkage occurs, resulting in dysphagia. Of 113 patients with carcinoma of the oro- and hypopharynx in the last 4 years, 35 patients were studied in a specific follow-up examination. Deglutition was evaluated via subjective responses to a questionnaire. Reproducible information was gained by videofluoroscopy and manometry of the pharyngo-esophageal segment. To assess deglutition, an arbitrary number scale was established with 7 for excellent and 1 for poor

Oesophageal function in diabetes mellitus and its association with autonomic neuropathy.

We have evaluated oesophageal function in 34 diabetics and in 16 non-diabetic controls by a timed bolus transit method derived from dynamic oesophageal scintiscans (water transit time: WTT) and barium swallow. The diabetics were screened for autonomic neuropathy using standard cardiovascular responses and 10 patients were classified as abnormal. WTT was significantly prolonged in autonomic neuropaths compared with other diabetics (p less than 0.01) and controls (p less than 0.001). Abnormal peristalsis on barium swallow was seen more frequently in autonomic neuropaths (9/10) than in other diab

Malignant lymphoma of Waldeyer's ring following gastrointestinal lymphoma.

Gastrointestinal involvement occurs in a greater proportion of patients with malignant lymphoma of Waldeyer's ring than would be accounted for by change alone. In the absence of direct lymphatic connection between these two sites, the association of these tumors has prompted several hypotheses: that the gastrointestinal lymphoma is a concomitant primary tumor; or that it is related to the swallowing and implantation of tumor cells; or that it represents the homing tendency of the gut-associated lymphoid tissue. Five cases are described in which malignant lymphoma of Waldeyer's ring developed t

Effect of thoracic vagotomy and vagal stimulation on esophageal function.

The purpose of this study was to determine the effect of thoracic vagotomy and thoracic vagal stimulation upon esophageal peristalsis and lower esophageal sphincter (LES) function in the opossum. The thoracic portion of the vagus nerve was sectioned in the upper or lower thorax. Bilateral, but not unilateral, thoracic vagotomy above the level of the heart abolished peristalsis and LES relaxation in response to swallowing or cervical vagal electrical stimulation. Thoracic vagotomy at the level of the ventricle or below did not alter either peristalsis or LES relaxation during swallowing or cerv
