[Contraception and bone health in adolescence].

Adolescents' access to contraception has improved and indication for their use have been extended beyond pregnancy prevention. For adolescents, this implies the use of hormonal contraception before the accrual of peak bone mass. Sexual hormones, notably estrogens, play a major role in skeletal development, growth and acquisition of maximal bone mass density (BMD). This article will review different contraceptive methods and their impact on BMD, with particular attention to low-dose pills and medroxyprogesterone acetate, both of which seem to alter peak bone mass acquisition. Nevertheless, bene

[Bone density measurements on growing skeletons and the clinical consequences].

The measurement of bone mineral density (BMD) was established to judge the fracture risk in an individual. The most commonly used densitometric technique DXA is a two-dimensional method and reports BMD (bone mass/projection area), which increases during growth. Bone mineral density (in g/cm(3)), however, is almost stable and does not change with age or height. To analyze the data special pediatric references including data on age, sex and ethnicity are necessary as well as correction for height. Bone forms a unit with muscle. Bone responds to mechanical loading with increase in bone size and t

Metabolic effects of growth hormone in the child and adolescent.

During the year 2000, several original studies were published regarding the metabolic effects of growth hormone therapy in pediatric patients. Pharmacologic doses of growth hormone were rarely associated with abnormalities in glucose tolerance in children with intrauterine growth retardation and Turner syndrome; however, serum insulin levels were elevated. A report from the Pharmacia International Growth Study database suggested a possible increase in type 2 diabetes in growth hormone-treated patients, indicating the need for continued surveillance for this condition. Growth hormone therapy in

The influence of pharyngeal flap on facial growth. Long-term results in the patients with isolated cleft palate.

The facial growth of 22 children from 6 to 17 years of age with repaired isolated cleft palate was studied. Eleven patients had had posterior pharyngeal flap surgery at about 7 years of age and were examined one year before, and three and 10 years after, flap surgery. The other eleven patients served as matched references. The convexity of the facial profile decreased significantly in both groups. There were differences in growth between the two groups in the variables that concerned mandibular position and anterior facial height. The results indicated that the reference individuals had achiev

Effect of musculoskeletal development on the prediction of body density in females.

The purpose of this study was to determine if musculoskeletal development had a significant effect on the prediction of body density in females. Subjects consisted of 156 females aged 17 to 44 years. Subjects were divided into three groups on the basis of musculoskeletal development as determined by the Heath-Carter Anthropometric Somatotype mesomorphy rating. Anthropometric measurements included eight skinfold measures (tricep, subscapula, chest, midaxillary, abdomen, suprailiac, thigh, and calf), body diameters of the humerus and femur, and circumferences at five locations (waist, forearm, u

True hermaphroditism: sonographic demonstration of ovotestes.

A 15-year-old boy with normal external genitalia presented with severe bilateral gynecomastia, intermittent painful scrotal swelling, and a recent history of scrotal trauma. Ultrasound (US) revealed testicles of normal size with diffusely heterogeneous echotexture due to scattered cysts. Ovotestes were found at surgery and pathologic examination. Bilateral partial gonadectomies were performed. Four months later he had testicular pain and swelling. US revealed bilateral multiseptate cystic masses. After repeat surgery, he is now free of symptoms. The diagnosis of true hermaphroditism and ovotes

Severe yellow fever with 23-day survival.

In a severe case of jungle yellow fever, acquired in the Brazilian mid-west region, clinical and laboratory evidence of hepatic and renal failure, cardiovascular disturbance, coma and bleeding disorder developed. The patient was treated in an intensive care unit and hemodialysis was performed. In spite of severe liver dysfunction, most biochemical parameters returned to normal values but the patient finally died of respiratory failure on the 23rd day, due to secondary bacterial pneumonia. A post-mortem liver biopsy showed regeneration of normal liver architecture. Without the bacterial respira

[Morphologic features of the structure of the hand bones of young volley ball players].

By means of roentgenography method with subsequent roentgenogramometry and roentgenodensitometry, morphological peculiarities of structure and mineralization of the hand bones in sportsmen-volley-ball players and in schoolchildren who do not go in for sports, at the age of 14-19 years have been studied. Systematic training in volley-ball produces certain increase in dimentions of the hand bones, prolongation of their synostosis time, intensification in development of the sesamoid bones and increasing amount of mineral salts in their bones.

Management of developmental speech and language disorders: Part 1.

The identification of developmental problems in a child's acquisition of speech, language and/or communication is a core activity in child surveillance. These are common difficulties with up to 15% of toddlers being 'late talkers' and 7% of children entering school with persisting impairments of their language development. These delays can confer disadvantages in the long term, adversely affecting language, cognition, academic attainment, behaviour and mental health. All children presenting with significant speech and language delay should be investigated with a comprehensive hearing assessmen

7q11.23 Microduplication: a recognizable phenotype.

Williams-Beuren syndrome is a well-known microdeletion syndrome with a recognizable clinical phenotype. The subtle phenotype of the reciprocal microduplication of the Williams-Beuren critical region has been described recently. We report seven further patients, and a transmitting parent, with 7q11.23 microduplication. All our patients had speech delay, autistic features and facial dysmorphism consistent with the published literature. We conclude that the presence of specific dysmorphic features, including straight, neat eyebrows, thin lips and a short philtrum, in our patients with speech dela

Enteropathogenic escherichia coli infection in children.

PURPOSE OF REVIEW: Enteropathogenic Escherichia coli (EPEC) is an important diarrheal pathogen of young children. As the diagnosis of EPEC is now based mainly on molecular criteria, there has been an important change in its prevalence. The purpose of this study is to review the current epidemiology of EPEC infection and the new insights into its physiopathology. RECENT FINDINGS: Recent epidemiological studies indicate that atypical EPEC (aEPEC) is more prevalent than typical EPEC (tEPEC) in both developed and developing countries, and that aEPEC is important in both pediatric endemic diarrhea
