The authors report a patient with a history of angina pectoris who developed anaphylactic shock that was complicated by a heart failure due to focal heart ischaemia. Early coronary angiography confirmed the diagnosis of localised coronary hypoperfusion. Intra aortic balloon counter pulsation succeeded in the restoration of coronary blood flow and haemodynamic stability. The authors discuss the opportunity of such treatment when a focal coronary hypoperfusion is diagnosed.

Chlorhexidine is a bis-biguanide molecule widely used in many preparations as a disinfectant and an antiseptic. We describe four confirmed cases of allergy to chlorhexidine gluconate in urethral gel and review the existing published reports. Pertinent issues in the treatment and investigation of these patients are discussed.

We report on one young adult male patient who had severe anaphylactoid reaction after eating cheese made from goat's milk. This allergy to goat and sheep's milk, known by the patient, gave mainly rise to oral pruritus. Cow's milk and cheese were always eaten without any incident. Labial test and specific IgE antibodies confirm goat and sheep's milk allergy without cow's milk allergy.

A case report of an adverse reaction to a preparation of an amide local anesthetic, prilocaine with epinephrine, is presented.
