Limb lengthening and deformity correction with externally controlled motorized intramedullary nails: evaluation of 50 consecutive lengthenings.

Background and purpose - Limb lengthening with an intramedullary motorized nail is a relatively new method. We investigated if lengthening nails are reliable constructs for limb lengthening and deformity correction in the femur and the tibia. Patients and methods - 50 lengthenings (34 Precice and 16 Fitbone devices) in 47 patients (mean age 23 years [11-61]) with ≥12 months follow-up are included in this study. 30 lengthenings were done due to congenital and 20 because of posttraumatic deformity (21 antegrade femora, 23 retrograde femora, 6 tibiae). Initial deformities included a mean shorteni

Incarceration of the Posterior Tibial Tendon in an Isolated Comminuted Medial Malleolus Fracture.

Isolated medial malleolar fractures are a less common presentation of an ankle fracture. Treatment is not universally accepted, although many have agreed that any displacement warrants anatomic reduction and fixation. We present a case of an isolated, comminuted medial malleolar fracture that was displaced secondary to entrapment of the posterior tibial tendon between the fracture fragments requiring surgical intervention. The patient was treated with prompt open reduction and internal fixation and had an excellent functional outcome at 1 year. When open reduction and internal fixation of the

Proximal tibiofibular joint dislocation associated with tibial shaft fractures - 7 Cases.

OBJECTIVE: Lower leg fractures of the tibia with or without fracture of the fibula are very common. Proximal tibiofibular joint (PTFJ) dislocation is a very rare injury that can occur together with a tibia shaft fracture. As there is only scarce literature about this injury available, we would like to present our experience with the treatment of this entity. METHODS: We present a small case series of seven patients. In most cases, the tibia fracture was nailed in a closed technique. After distal locking the proximal fibula was exposed by a lateral approach exposing and preserving the peroneal

Triple fracture during rehabilitation after revision total knee arthroplasty.

PURPOSE: The incidence of periprosthetic fractures after knee-joint implant revisions is increasing in prevalence. We present a method of treatment for a patient who sustained a triple fracture--a periprosthetic femur fracture, a patella fracture, and a tibial shaft fracture. METHODS: The femoral fracture was treated with a specially designed intramedullary nail, the patella fracture with a figure-of-eight suture, and the tibial shaft fracture by a minimal-invasive plate osteosynthesis using a percutaneous plating technique. RESULTS: Osseous consolidation was confirmed, and the patient prese

Intramedullary nailing versus submuscular plating in adolescent femoral fracture.

BACKGROUND: Femoral fractures in adolescents usually need operative treatment, but the optimal method is unclear. The purpose of this study is to compare intramedullary nailing (IN) and submuscular plating (SP) in adolescent femoral fractures. MATERIALS AND METHODS: We performed the prospective, comparison study of IN and SP in adolescent femoral shaft fractures at a mean age of 13.9 years (11-17.4). Twenty-two cases of IN and 23 cases of SP were followed for a minimum of 1 year. We compared radiological and clinical results, surgical parameters, and complications of two techniques. RESULTS:

In-situ pinning for Slipped Capital Femoral Epiphysis in blacks: experience in a regional orthopaedic centre.

BACKGROUND: Slipped Capital Femoral Epiphysis (SCFE) is the commonest pathology affecting the adolescent hip with an incidence of approximately 2 per 100,000 population. Blacks are more commonly affected than Caucasians. Of many treatment options available, in-situ-pinning appears to be the most widely employed. AIMS AND OBJECTIVES: To determine the epidemiological pattern and the outcome of in-situ pinning for slipped capital femoral epiphysis in Nigerian adolescents. PATIENTS AND METHODS: This is a six-year retrospective review of all the cases of slipped capital femoral epiphysis treated

Comparison of intramedullary nailing and external fixation knee arthrodesis for the infected knee replacement.

UNLABELLED: We analyzed knee arthrodesis for the infected total knee replacement (TKR) using two different fixation techniques. Patients undergoing knee arthrodesis for infected TKR were identified and rates of successful fusion and recurrence of infection were compared using Cox proportional hazard models. Eighty-five consecutive patients who underwent knee arthrodesis were followed until union, nonunion, amputation, or death. External fixation achieved successful fusion in 41 of 61 patients and was associated with a 4.9% rate of deep infection. Fusion was successful in 23 of 24 patients with

Treatment of subtrochanteric fractures. A comparison of the Gamma nail and the dynamic hip screw: short-term outcome in 58 patients.

The aim of this non-randomised prospective study was to evaluate the short-term outcome of Gamma nail and dynamic hip screw (DHS) fixation in the treatment of subtrochanteric hip fractures due to low-energy trauma in the elderly. All of the 1,624 femoral hip fractures in 1,511 patients of the Oulu Hospital, aged over 49 years, were prospectively registered from 1991-1999 using special forms. Seventy-three (4.5%) of the fractures were subtrochanteric. After exclusions, 58 patients constituted the final study group. Of these, 43 were treated with Gamma nails and 15 with DHS. Ipsilateral femoral neck and shaft fractures.

Ipsilateral, concomitant femoral neck and shaft fracture is a rare fracture pattern encountered by orthopaedic surgeons. We review 11 cases of this fracture pattern. Treatment was by reduction and stabilization of the femoral neck fracture with one or more cannulated screws, followed by fixation of the femoral shaft using a Russell-Taylor reconstruction nail or a Russell-Taylor Delta reconstruction nail. When properly sequenced, this has proven to be a successful treatment method in these dual fractures, resulting in a low complication rate. No avascular necrosis of the femoral head or nonunio

Valgus deformity following supracondylar elbow fractures in children.

We reviewed the cases of 10 patients with cubitus valgus deformity who had a supracondylar elbow fracture during childhood, in order to identify the causes of valgus deformity. Cubitus valgus deformity occurred in 3 cases because of failure to recognize the deformity at the initial treatment. In 7 patients the deformity was recognized but was insufficiently treated. In this series there was no evidence of progressive valgus deformity after healing of the fracture. Incomplete correction is the main cause of valgus deformity. It is suggested that, in order to reduce the incidence of valgus defor

In vitro laxity-testers for knee joints of mice.

The knee joints of mice can be used as a model for studying the effects of interventions on knee laxity. The goal of this study was to quantify knee joint laxity in vitro. Three devices were developed: a positioning- and cementing device, an anterior-posterior (AP) laxity tester and a varus-valgus (VV) laxity tester. The positioning and cementing device was used to position the joint in a reproducible way and to attach clamping pins to the proximal femur and distal tibia using PM MA. The clamping pins were used to fix the joint to the AP- and VV-testers. In both testers the load was applied by

External fixation of severe open tibial fractures with the Hoffmann frame.

Extensive experience with stable external fixation of open tibial fractures has confirmed the observation that this method promotes healing of skin and soft tissue damage, reduces the risk of infection, and facilitates the treatment of patients with multiple injuries. In cases of very severe open fractures the method seems to be the treatment of choice and can reduce the frequency of amputation. Recently, complete long-term follow-up results showed that healing times can be reduced if the period of rigid external fixation is reduced. Functional end results after stable external fixation compar

Lag screw cutout risk factors in geriatric intertrochanteric femur fractures.

BACKGROUND: Lag screw cutout remains the most frequent mechanical failure after proximal femoral nailing (PFN) for geriatric intertrochanteric femur fractures and is strongly influenced by bone quality, reduction, and implant positioning. This study aimed to evaluate these domains comprehensively and to identify independent predictors of cutout. METHODS: In this retrospective cohort study, patients aged ≥ 65 years who underwent PFN (single implant design, APFN®) for AO/OTA 31.A1-A3 intertrochanteric fractures between January 2017 and June 2025 were reviewed. Patients with follow-up < 6 months

Clinical study of 3D printed personalized prosthesis in the treatment of bone defect after pelvic tumor resection.

BACKGROUND: /Objective: In recent years, prostheses have been widely used for limb reconstruction after pelvic tumour resection. However, prostheses are associated with problems leading to tumour recurrence, poor implant matching, defects after tumour resection, and easy implant looseness or failure. To achieve a precise preoperative design, complete tumour resection, and better anatomical structure matching and prosthesis stability, this study used three-dimensionally (3D)-printed osteotomy guides and personalised prostheses for reconstruction after pelvic tumour resection. This study aimed t

Role of autologous non-vascularised intramedullary fibular strut graft in humeral shaft nonunions following failed plating.

BACKGROUND: Non-union humeral shaft fractures are seen frequently in clinical practice at about 2-10% in conservative management and 30% in surgically operated patients. Osteosynthesis using dynamic compression plate (DCP), intramedullary nailing, locking compression plate (LCP), Ilizarov technique along with bone grafting have been reported previously. In cases of prior failed plate-screw osteosynthesis the resultant osteopenia, cortical defect, bone loss, scalloping around screws and metallosis, make the management of non-union more complicated. Fibular graft as an intramedullary strut is us
