The authors report a primary cutaneous cryptococcosis, caused by Cryptococcus neoformans in immunocompetent patient, a farmer who developed extensive lesions at the site of an injury caused by one of the chickens on his right forearm, while he was cleaning out his barn. Oral treatment with fluconazole was totally successful. A review of the literature showed the rarity of cutaneous cryptococcosis in immunocompetent patients and in contrast, that skin lesions frequently occur in immunocompromised patients.

Radial head arthroplasty has developed into a reliable procedure to address fractures of the radial head not amenable to reconstruction, particularly when associated with an unstable elbow or forearm. Good to excellent results can be anticipated when used for the correct indications and when care is taken to understand and address the concomitant injuries. This article will discuss the technique of radial head arthroplasty in the context of the injuries that often accompany radial head fractures.

Erythema nodosum (EN) is the most common form of panniculitis. It is characterized by the presence of rounded or oval, slightly raised, nonulcerative painful red nodules in the skin and subcutaneous fatty tissue, 1 to 6 cm in diameter, sometimes coalescing. They tend to be symmetrical in distribution and are usually located bilaterally on the lower extremities, particularly on the anterior tibial surface, although they may also involve the ankles, the lower parts of the thighs, and the forearms.

A 43-year-old woman presented with a recurrent and remitting eruption of reddish-brown papules in the sun-exposed areas of the arms and forearms that had been present for 2 years. Results of a histopathologic examination revealed syringoma. To the best of our knowledge, this is the first case report of syringomas confined to sun-exposed areas with a recurrent and remitting course over time. We also review the literature on acral syringomas.

Nocardia farcinica is an increasingly common cause of human infection in Europe. We observed an isolated nodular subcutaneous abscess due to N. farcinica in the forearm of a young patient with overlap syndrome between systemic sclerosis and polymyositis. Despite in vitro resistance to trimethoprim-sulfamethoxazole, this oral combination completely resolved the lesion within 3 weeks and was maintained for 6 months. The spectrum of cutaneous nocardiosis in the immunocompromised host is discussed.

Various patterns of transscaphoid, transcapitate fracture-dislocations have been described in the literature. There is little information on the method of management and the long-term results of such severe and rare injuries. The case described here involved a transscaphoid, transcapitate, palmar perilunate fracture-dislocation with ejection of the proximal pole of the scaphoid and lunate into the palmar aspect of the forearm. The functional result 32 months after delayed open reduction and internal fixation is reported.

We present a 44-year old man with an hemorrhagic dorsal skin wound of the left ring finger, which after angiography led to the diagnosis of an arteriovenous dysplasia with fistulas involving the whole left forearm and hand. The local problem was successfully treated by surgical hemostasis and skin flaps, but the underlying vascular pathology seems to be untreatable. Compressive gloves as used in burn treatment are a simple help in this particular case.

McArdle disease is reported in three generations of a consanguineous Druze family. The diagnosis was established on the basis of a failure of a rise in lactate in the ischemic forearm exercise test, glycogen accumulation in muscle fibers and the lack of myophosphorylase by histochemical and biochemical studies. The inheritance pattern is compatible with an autosomal recessive mode. Examination of family members revealed a marked variability in the clinical findings and functional status. This is the first reported case of the disorder in this ethnic group.

Forty patients undergoing hemodialysis were examined for bruits transmitted from the vascular access arm. In patients with forearm access, the bruits were localized to the clavicular and cervical regions on the same side. However, transmission to the sternal edge was noted in 8 of 10 patients with upper arm access. Compression of the fistula is a useful maneuver that allows the clinician to distinguish transmitted murmurs from those arising in cardiac valves.

Rotatory instability of scaphoid fractures was studied by a method not used previously. On two fresh specimens the scaphoid bone was fractured by osteotomy, and the fracture was fixed by a Hoffmann apparatus extending from the distal radius to the 1st, 2nd and 3rd metacarpals. This permits free rotation in the forearm. The scaphoid fragments were marked by Kirschner wires. No movement occurred between the fragments when supination/pronation was kept within the normal range of movement.

A 61-year-old male receiving treatment for acute myelomonocytic leukaemia (AMML) developed weakness and sensory loss in the thumb, index and middle finger and half of the ring finger of the left hand as a result of a spontaneous haemorrhage into the carpal tunnel and forearm, an unusual cause of median nerve compression. These findings were confirmed by CT scanning and post-mortem examination. This is the first report of spontaneous haemorrhage occurring in a patient with AMML.

Traumatic recurrent isolated posterior dislocation of the radial head occurred in a 56-year-old woman. The differential diagnosis included congenital dislocation, neurovascular disorders, tumors of the forearm, and Monteggia-type fracture-dislocation. The etiology of the injury is obscure. The treatment of choice depends on the age of the patient and the status of the elbow. The characteristics distinguishing this case from others previously presented in the literature include the recurrence, posterior displacement, and age of the patient.

It is reported on a 44-year-old patient who has a peculiar form of the disturbance of the distribution of fatty tissue. Apart from clear superficial hypertrophies at upper arms, shoulders, thorax and hypogastrium as well as plum-sized nodes at the left femor and retroauricularly discrete atrophies are found in the face, at forearms and lower legs. In very inconspicuous paraclinical findings special forms of the progressive lipodystrophy and of the indolent symmetrical lipomatosis are discussed.

The medial antebrachial cutaneous nerve is a sensory nerve that provides sensation to the forearm and elbow. Neuropathy of this nerve can result from various causes, including iatrogenic injury. We report a case of medial antebrachial cutaneous neuroma following a skin punch biopsy and discuss the surgical management and outcome. Although such complications are uncommon, this case emphasizes the importance of anatomical awareness and postoperative evaluation when patients present with altered sensation.

Malignant glomus tumors (MGTs) are exceptional but pose diagnostic and therapeutic challenges. Wide resection is the recommended treatment method, however, no data are available concerning adjuvant therapies. We present an exceptional case of extradigital deep-seated MGT of the forearm, with an exceptional bone infiltration. Despite being treated with wide resection, the patient had an optimal functional outcome, no functional loss, no motor or sensitive deficits and has returned to his full daily activity.

A 22-year-old man was admitted to our hospital because of polytrauma following a car accident. The instrumental investigations detected parenchymatous and bone lesions, including a floating elbow (articular distal humerus fracture associated with ipsilateral forearm fracture). After initial temporary stabilization with external fixator and restoration of hemodynamics, we proceeded to the fixation of the upper limb. To reduce local and general risks of complications, a two-stage reconstruction technique has been applied.

A 2½ year old girl attended our facility following attack by a tapir at a city zoo. She sustained multiple injuries including a forearm laceration and multiple perforating wounds to her abdominal wall. She had several procedures, including bowel resection, performed under the care of the General Paediatric Surgery and Plastic Surgery teams and was treated with a course of IV antibiotics. She recovered well and to date has suffered no long-term adverse outcome.

We present a case of recurrent anterior instability of the radial head presenting 4 weeks after injury. A 16-year-old girl injured the right upper extremity while playing volleyball. Thereafter, she felt a recurrent click associated with pain in the elbow when rotating the forearm. Image intensifier findings implied that the radial head would anteriorly dislocate with contraction of the biceps brachii. Annular ligament reconstruction using the palmaris longus tendon graft relieved the instability.

Fibular Hemimelia is a rare congenital disorder of deficiency to complete absence of fibula. It has been associated with post axial skeletal anomalies like shortening of femur, absence of lateral rays of foot and syndactyle, polydactyle etc. We report this case because of associated amniotic constriction bands over both the forearm, suggesting amniotic bands may be one of the causal factors of Fibular Hemimelia due to insult to the growing limb bud.

INTRODUCTION: Systemic vasculitis presenting as a tumorlike lesion are unusual and affect mainly the uro-genital system and breast. Muscle-localized tumorlike periarteritis nodosa are scarce.
EXEGESIS: We report localized tumorlike polyarteritis nodosa affecting the forearm muscles in a 90-year-old woman.
CONCLUSION: Other cases of polyarteritis nodosa localized to muscles are reviewed. This limited form has a benign clinical course and responds promptly to treatment with corticosteroids alone but relapses may occur.
