Treatment of Atypical Central Cord Injury Without Fracture or Dislocation.

In general, spinal cord injury without radiological abnormality (SCIWORA) with spondylosis is considered to be a central cord injury. This article describes the clinical features and surgical outcomes of atypical central cord injury in patients with spondylotic SCIWORA (ACCISS). Fifty-two patients were enrolled in the study from January 2006 to December 2011. Diagnoses were made from imaging (computed tomography, magnetic resonance imaging) and clinical findings. The neurologic status of patients was assessed using the American Spinal Injury Association (ASIA) score and Japanese Orthopaedic As

Alcohol use disorders after bariatric surgery.

BACKGROUND: The increased incidence of alcohol use disorders (AUD) after bariatric surgery has been proposed despite limited empirical support. We sought to determine the prevalence of current and lifetime AUD and other Axis I diagnoses in patients who have undergone bariatric surgery, and to test the hypothesis that greater weight loss is associated with a higher incidence of AUD following surgery. METHODS: Individuals who underwent bariatric surgery between 2004 and 2007 were recruited for inclusion in the study. The diagnosis of current and lifetime AUD and other Axis I disorders was asses

Long-term endothelial cell loss following phacoemulsification: model for evaluating endothelial damage after intraocular surgery.

BACKGROUND: Newer concepts of phakic intraocular lens (IOL) surgery present concerns as to their long-term deleterious effect on the endothelium. We examine the behavior of the endothelium for up to 6 years following uneventful phacoemulsification surgery, to establish a baseline for what might be an acceptable level of endothelial cell loss due to intraocular surgical trauma. METHODS: Ninety-three eyes undergoing phacoemulsification surgery who had multiple endothelial specular microscopy examinations for up to 6 years were examined. Central endothelial specular photomicrographs were analyze

Liberal or Restrictive Postoperative Transfusion in Patients at High Cardiac Risk: The TOP Randomized Clinical Trial.

IMPORTANCE: Postoperative red blood cell transfusion guidelines recommend transfusion for hemoglobin levels less than 7 g/dL. However, the safety of this strategy in patients at high risk of cardiac events undergoing major operations remains unclear. OBJECTIVE: To evaluate the risk of death or major ischemic events within 90 days after a liberal transfusion strategy compared with a restrictive transfusion strategy in patients at high risk of cardiac events who had undergone major vascular or general surgery operations and developed postoperative anemia. DESIGN, SETTING, AND PARTICIPANTS: Thi…

Decision-making in primary hyperparathyroidism in older patients: surgical or conservative management?

Primary hyperparathyroidism (PHPT) is a frequent endocrine disorder, with prevalence increasing with age, affecting approximately 1.5% of adults aged over 65. While management is well codified before the age of 50, with surgery being preferred, recommendations for older individuals are less clear. In this population, however, surgery is still the only curative option, and several studies demonstrated improvement in cognitive and neuropsychiatric symptoms, following parathyroidectomy. However, this indication must be weighed against the patient's co-morbidities, frailty and wishes. Drug-based a

Addition of flexible laryngoscopy to anesthesiological parameters improves prediction of difficult intubation in laryngeal surgery.

INTRODUCTION: Among the numerous scores for difficult intubation assessment, there is no single score that is specific for laryngeal surgery. The parameters identified by this research will provide a foundation for developing a new score for preoperative airway assessment specific for laryngeal surgery. MATERIAL AND METHODS: This prospective pilot clinical study included 50 patients over 18 scheduled for microscopic laryngeal surgery. The following anesthesiological and surgical parameters were analyzed: flexible laryngoscopy, general and clinical data, inter-incisor gap (IIG), modified Malla

Age and number of lesions predict chalazion recurrence.

PURPOSE: Periocular lesions in pediatric patients usually require general anesthesia for surgical intervention. The US Food and Drug Administration (FDA) warns against multiple exposures to anesthesia in children younger than 3 years due to the increased risk of learning disabilities in this population. This study aimed to evaluate risk factors associated with chalazion recurrence after surgery. METHODS: A retrospective chart review over a five-year period identified 649 patients at our institution undergoing surgical intervention for chalazion. The primary outcomes examined were as follows:

Two Cases of Rectal Cancer with Retzius Shunt Treated with Robot-Assisted Surgery.

The retroperitoneal intestinal vein-general circulation anastomotic pathway is referred to as a Retzius shunt; however, it is not a well-recognized condition. Here, we describe two patients with a Retzius shunt who underwent robot-assisted surgery for rectal cancer. The first case was an 81-year-old woman who had tested positive for fecal occult blood. A type 0-Is tumor was found in the middle rectum, and we used robot-assisted surgery for resection. Intraoperative findings included a dilated vein between the inferior mesenteric artery (IMA) and inferior mesenteric vein (IMV); further, compute

CLPNet: Cleft Lip and Palate Surgery Support With Deep Learning.

Cleft lip and palate is the most common congenital malformation in oral and maxillofacial region. As a kind of facial plastic surgery, the most important factor for the success of cleft lip and palate repair surgery is the design of surgical markers and incisions. However, general hospitals especially in rural areas lack dependable medical resources, which makes the effect of the surgery hard to guarantee. To solve this problem, we propose a novel robotic surgery assistant technology based on deep learning to help reduce the technical threshold and improve the overall effect of cleft lip and p

Pulmonary complications after abdominal surgery in patients with mild-to-moderate chronic obstructive pulmonary disease.

Postoperative pulmonary complications (PPCs) are one of the most important causes of postoperative morbidity and mortality after abdominal surgery. Although chronic obstructive pulmonary disease (COPD) has been considered a risk factor for PPCs, it remains unclear whether mild-to-moderate COPD is a risk factor. This retrospective cohort study included 387 subjects who underwent abdominal surgery with general anesthesia in a tertiary referral hospital. PPCs included pneumonia, pulmonary edema, pulmonary thromboembolism, atelectasis, and acute exacerbation of COPD. Among the 387 subjects, PPCs d
