Castleman's disease.

Castleman's disease (CD) is a rare atypical lymphoproliferative disorder whose morphology, soon after the original presentation of Castleman et al., has been definitely subdivided in a hyaline vascular (HV) and plasma cell (PC) histopathological pattern, with intermediate variants. The former occurs much more frequently than the latter and is usually localized to the mediastinum or pulmonary hilum. The latter involves lymph nodes separately or in aggregations and often displays multicentricity with systemic symptoms including autoimmune phenomena and aggressive course. Infections are the most

Dual lympho-myeloproliferative disorder in a patient with t(8;22) with BCR-FGFR1 gene fusion.

The case of a patient presenting with a myeloproliferative disorder (MPD) characterized by a t(8;22) (p12;q11) translocation was investigated. The rearrangement resulted in the production of BCR-FGFR1 and FGFR1-BCR chimeric transcripts after in-frame fusions of BCR exon 4 with FGFR1 exon 9 and FGFR1 exon 8 with BCR exon 5, respectively. The four previously reported patients with such translocation presented with an atypical chronic myeloid leukemia (CML) without Philadelphia chromosome. In addition to a myeloproliferation, the patient had a B cell proliferation. The phenotypic characterization

Benefits and risks of induction immunosuppression in pediatric autoimmune liver disease from SPLIT Registry.

The optimal induction regimen for pediatric liver transplantation (LT) in patients with Autoimmune Liver Disease (AILD) is not well-defined. This study compared outcomes between corticosteroids only, anti-thymocyte globulin (ATG), and IL-2 receptor antagonists (IL2RA; Basiliximab/Daclizumab). A retrospective review of 445 pediatric LT recipients with AILD (Autoimmune hepatitis, primary sclerosing cholangitis, or autoimmune sclerosing cholangitis) was conducted using the SPLIT database. Re-transplants were excluded. Primary outcomes included biopsy-confirmed rejection, culture-proven infection,

Hepatic pseudolymphoma with hepatobiliary-phase ring-like hyperintensity on gadoxetic acid-enhanced MRI: radiologic-pathologic correlation with intratumoral fibrosis.

Hepatic pseudolymphoma (HPL) is a rare benign lymphoproliferative disorder that can mimic malignant hepatic tumors on imaging. Hepatic pseudolymphoma is usually small and solitary; however, multiple lesions can develop in some cases. Herein, we report multiple HPLs with an unusual hepatobiliary finding on gadoxetic acid-enhanced MRI. A woman in her 50 s with non-alcoholic fatty liver disease was incidentally found to have 2 hepatic tumors: a 3-cm lesion in segment 5 (S5) and a 1-cm lesion in segment 8 (S8) during pre-operative cholelithiasis evaluation. The laboratory tests, including liver fu

Comparison of Mycophenolate Dosing Strategies in Pediatric Heart Transplant.

BACKGROUND: Mycophenolic acid (MPA) is a common immunosuppressant used in children post-orthotopic heart transplant (OHT). Multiple MPA dosing strategies exist, with limited reports of comparisons between dosing strategies and patient outcomes. METHODS: We performed a retrospective review of heart transplant recipients ≤ 21 years from 2014 to 2022, comparing dose concordance and clinical outcomes of weight-based mycophenolate dosing (15 mg/kg/dose every 12 h) to body surface area (BSA)-based dosing (600 mg/m2/dose every 12 h). RESULTS: We analyzed 109 patients total (56% in the BSA-based coh

Endoscopic-Assisted Excision of Parotid Castleman Disease in Pediatric Patients: A Case Series of Three Patients and a Literature Review.

BACKGROUND Castleman disease (CD) is a rare lymphoproliferative disorder, and parotid involvement in children is exceptionally uncommon. It is typically classified as unicentric (UCD) or multicentric; the hyaline vascular (HV) type is most prevalent. Surgical excision remains the standard curative treatment for UCD. CASE REPORT This study synthesizes recent advances in endoscopic-assisted excision of parotid CD among pediatric patients. We describe 3 pediatric patients (ages 2, 7, and 10 years) who presented with asymptomatic parotid masses, all initially misdiagnosed. Each patient underwent e

Hairy cell leukemia in a 51-year-old Syrian male: a case report.

INTRODUCTION: Hairy cell leukemia (HCL), a rare B-cell lymphoproliferative disorder, originates from the splenic marginal zone B cell. However, diagnosis can be particularly challenging in resource-limited settings where advanced tests are not readily available. Misclassification may result in non-selective chemotherapy exposure and increased toxicity. Reporting such cases is important to highlight diagnostic challenges in resource-limited countries. CASE PRESENTATION: A 51-year-old male presented with abdominal discomfort, weight loss, and fatigue. Examination revealed splenomegaly. Initial

Living Donor Liver Transplantation as Salvage Therapy for Steroid-Refractory Chronic Hepatic GVHD Following Allogeneic Hematopoietic Stem Cell Transplantation.

BACKGROUND: Hepatic graft-versus-host disease (GVHD) is a rare but life-threatening complication following allogeneic hematopoietic stem cell transplantation (HSCT). Steroid-refractory hepatic GVHD carries a high risk of mortality with limited salvage options. Liver transplantation after HSCT poses unique challenges due to dual alloimmunity and is rarely pursued, particularly in pediatric patients. We report a pediatric patient with steroid-refractory chronic hepatic GVHD successfully treated with living donor liver transplantation (LDLT) from a parental donor. CASE PRESENTATION: A 13-year-ol

Case Report: CD19 CAR-T therapy induces dual remission in AML-M2b patient with CNS-PTLD and relapse.

BACKGROUND: Acute myeloid leukemia (AML)-M2b with t(8;21)(q22;q22)/RUNX1::RUNX1T1 (AML1-ETO) is associated with a high risk of relapse after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Post-transplant lymphoproliferative disorder (PTLD), particularly involving the central nervous system (CNS), confers a poor prognosis. Although CD19 chimeric antigen receptor T-cell (CAR-T) therapy is established in B-cell malignancies, its application in acute myeloid leukemia (AML) or CNS-PTLD has rarely been reported. CASE: A 24-year-old male with AML-M2b showed persistent RUNX1::RUNX1T1

Classification of NK-large granular lymphocytic leukemia by CD56 expression.

NK-large granular lymphocytic leukemia (NK-LGLL) is a rare chronic lymphoproliferative disorder and displays heterogeneity that remains insufficiently defined. CD56 plays a pivotal role in NK-cell maturation linked to cytotoxicity. However, whether CD56 might be associated with distinctive characteristics in NK-LGLL has not been determined. Hence, this study aims to explore potential associations between CD56 and clinical and biological features in 47 patients with NK-LGLL. Above all, anemia (57.4%) was the most prevalent symptom. Patients treated with immunosuppressive therapy showed a favora

Combined Thymoglobulin and Interleukin-2 Receptor Antagonist Induction Therapy in Kidney Transplantation Is Associated With Worse Delayed Graft Function, Acute Rejection, Graft Loss, and Mortality.

INTRODUCTION: Single-agent induction with either Thymoglobulin (ATG), alemtuzumab, or interleukin-2 receptor antibodies (IL2Ra) is a standard of immunosuppression during kidney transplantation (KTX). Dual-induction therapy (DIT) with ATG + IL2Ra is occasionally administered as part of a center-specific protocol or salvage following failed ATG or IL2Ra induction. We report the outcomes of single-agent regimens versus DIT. METHODS: Analysis of the UNOS database from January 1, 2000, to June 30, 2024, focused on KTX recipients induced with ATG, alemtuzumab, IL2Ra, or DIT. Recipient and donor cha

Thrombotic Thrombocytopenic Purpura Triggered by Chronic Lymphocytic Leukemia: A Case Report.

INTRODUCTION: Chronic lymphocytic leukemia (CLL) is an incurable lymphoproliferative disorder characterized by the accumulation of mature malignant B cells in the blood, bone marrow, and secondary lymphoid tissues. While it has a heterogenous clinical course, individuals with CLL are at increased risk for autoimmune complications, infections, and secondary non-hematologic secondary malignancies. Peripheral autoimmune cytopenias are a well-known phenomenon in CLL. However, other autoimmune complications, including immune thrombotic thrombocytopenic purpura (TTP), are rare and less investigated.

An overview of prognostic markers in patients with CLL.

Chronic lymphocytic leukemia (CLL) is a low-grade B-cell lymphoproliferative disorder. It is the most prevalent type of leukemia in the western countries, with a median age at diagnosis of 70 years. In 2023, it is estimated that there will be 18,740 new cases of CLL, and an estimated 4,490 people will die of this disease. It represents 1.0% of all new cancer cases in the U.S. The rate of new cases was 4.6 per 100,000 men and women per year based on 2016-2020 cases, age-adjusted. Death rates from CLL are higher among older adults, or those 75 and older. The death rate was 1.1 per 100,000 men an
