Pain in NMOSD and MOGAD: A Systematic Literature Review of Pathophysiology, Symptoms, and Current Treatment Strategies.

Neuromyelitis optica spectrum disorders (NMOSDs) and myelin oligodendrocyte glycoprotein-antibody-associated disease (MOGAD) are autoimmune inflammatory disorders of the central nervous system (CNS). Pain is highly prevalent and debilitating in NMOSD and MOGAD with a severe impact on quality of life, and there is a critical need for further studies to successfully treat and manage pain in these rare disorders. In NMOSD, pain has a prevalence of over 80%, and pain syndromes include neuropathic, nociceptive, and mixed pain, which can emerge in acute relapse or become chronic during the disease c

Potential indications for deep brain stimulation in neurological disorders: an evolving field.

Deep brain stimulation (DBS) is an established therapy for appropriately selected patients with movement disorders and neuropsychiatric conditions. Although the exact mechanisms and biology of DBS are not fully understood, it is a safe and well-tolerated therapy for many refractory cases of neuropsychiatric disease. Increasingly, DBS has been explored in other conditions with encouraging results. In this paper, available data is reviewed and new DBS targets, challenges and future directions in neurological disorders are explored. A detailed search of the medical literature discussing the poten…

When intracellular logistics fails--genetic defects in membrane trafficking.

The number of human genetic disorders shown to be due to defects in membrane trafficking has greatly increased during the past five years. Defects have been identified in components involved in sorting of cargo into transport carriers, vesicle budding and scission, movement of vesicles along cytoskeletal tracks, as well as in vesicle tethering, docking and fusion at the target membrane. The nervous system is extremely sensitive to such disturbances of the membrane trafficking machinery, and the majority of these disorders display neurological defects--particularly diseases affecting the motili

Upbeat nystagmus is a useful sign in the regional diagnosis of trigeminal nerve disorder with multiple sclerosis.

Trigeminal nerve disorder is an important neurological sign that is often seen with multiple sclerosis (MS). We investigated eye movements in three MS patients with trigeminal disorder due to pontine lesions near the trigeminal root entry zone (REZ). Upbeat nystagmus was observed in all MS patients with trigeminal REZ lesions. We conjecture that trigeminal nerve disorder and upbeat nystagmus appeared due to simultaneous damage to both the trigeminal nerve and the vestibulo-ocular reflex pathway. If upbeat nystagmus appears in MS patients exhibiting a trigeminal nerve disorder, such as trigemin

Neurophysiology of cerebellar ataxias and gait disorders.

There are numerous forms of cerebellar disorders from sporadic to genetic diseases. The aim of this chapter is to provide an overview of the advances and emerging techniques during these last 2 decades in the neurophysiological tests useful in cerebellar patients for clinical and research purposes. Clinically, patients exhibit various combinations of a vestibulocerebellar syndrome, a cerebellar cognitive affective syndrome and a cerebellar motor syndrome which will be discussed throughout this chapter. Cerebellar patients show abnormal Bereitschaftpotentials (BPs) and mismatch negativity. Cere

Rapidly Progressive Dementia Secondary to Vitamin B12 Deficiency: A Case Report.

INTRODUCTION: While vitamin B12 deficiency is classically associated with hematological abnormalities and subacute combined degeneration, its presentation as rapidly progressive cognitive decline with episodic movement disorders remains an underrecognized clinical phenomenon. This case highlights the diagnostic challenges posed by such non-classical neurological manifestations and underscores the need for increased clinical suspicion in cases of unexplained cognitive-motor decline. CASE PRESENTATION: A patient presented with rapidly progressive memory impairment and episodic limb tremors in t…

Corticotropin-releasing hormone neurons in the bed nucleus of the stria terminalis control wakefulness in physiological and stressful conditions in mice.

Stressful and emotionally salient stimuli often trigger heightened wakefulness that can predispose to sleep disorders such as insomnia. The bed nucleus of the stria terminalis (BNST), a center for emotional information processing and anxiety-related behaviors, contains diverse neuropeptide-enriched neuronal subpopulations that may respond to and drive opposing emotional states. Although recently implicated in arousal regulation, the mechanisms by which BNST neuronal subpopulations control sleep-wake states remain unclear. Here, we show that optogenetic activation of corticotropin-releasing hor

Subtalar joint kinematics defined by a rotational axis translating along the posterior talocalcaneal facet.

The subtalar joint is essential for the normal function of the human foot during bipedal walking, with its kinematics being pivotal for understanding foot biomechanics, disorders, and evolution. Traditionally, the helical axis representation has been used to assess subtalar joint movement, assuming translational motion along the rotational axis. However, recent observations challenge this assumption, revealing predominantly mediolateral translation during walking. To address this discrepancy, we propose a novel method that combines a rotational axis representation with a translational axis ali

METHOD: Twenty-eight adults with HVDs and 28 age- and sex-matched controls produced repeated utterances of /ifi/ at three different gesture rates (50, 65, and 80 beats per minute) and three self-induced vocal effort levels (mild, moderate, and maximum effort

Parkinsonism originates in a discrete secondary and dystonia in a primary motor cortical-basal ganglia subcircuit.

Although manifesting contrasting phenotypes, Parkinson's disease and dystonia, the two most common movement disorders, can originate from similar pathophysiology. Previously, we demonstrated that lesioning (silencing) of a discrete dorsal region in the globus pallidus (rodent equivalent to globus pallidus externa) in rats and produced parkinsonism, while lesioning a nearby ventral hotspot-induced dystonia. Presently, we injected fluorescent-tagged multi-synaptic tracers into these pallidal hotspots (n = 36 Long Evans rats) and permitted 4 days for the viruses to travel along restricted connect…
