[Carpal Tunnel Syndrome and Lumbar Spinal Stenosis: When to Suspect Amyloidosis and How to Proceed with the Diagnostic Work-up].

Transthyretin (TTR) amyloidosis, particularly the wild-type form (ATTRwt), is an age-related systemic disease characterized by the deposition of misfolded TTR in tendons and ligaments. Patients therefore frequently present initially to orthopaedic or hand surgeons with carpal tunnel syndrome (CTS) or lumbar spinal stenosis (LSS) rather than to cardiologists or neurologists. These musculoskeletal manifestations may precede ATTR cardiomyopathy (ATTR-CM) by several years to more than a decade, serving as "red flags" that provide an opportunity for early diagnosis, particularly now that disease-mo

Evaluation of s-EMG Sensor Locations for Upper-Limb Compensatory Movement Detection.

Musculoskeletal rehabilitation is crucial for restoring upper limb function after elbow trauma or stroke. In unsupervised rehabilitation, patients may develop compensatory movements that hinder recovery. While wearable devices for home rehabilitation are a promising supplement to clinical therapy, they may overlook movement quality. Detecting compensatory motions in wearable systems is challenging, as placing sensors on all involved muscles reduces wearability, increases computational and power demands, and complicates sensor management. The objective of this study was to identify optimal loca

Astragalus and Cordyceps Derivatives in the Treatment of Aging-Related Chronic Diseases and Neurodegenerative Disorders.

Aging is associated with a rising burden of chronic metabolic, cardiovascular, musculoskeletal, and neurodegenerative diseases that share interconnected pathological mechanisms, including oxidative stress, chronic inflammation, mitochondrial dysfunction, metabolic imbalance, and immune dysregulation. Because these disorders arise from complex and overlapping biological disturbances, conventional single-target therapies often provide only limited benefit. In this context, traditional Chinese herbal medicines, characterized by multi-component and multi-target actions, are being re-evaluated usin

Unsegmented marine annelids as biomechanical models for soft robotics.

This work investigates marine worms as a source of bioinspiration for soft robotics, focusing on Phascolosoma stephensoni (Annelida), an unsegmented sipunculan species with a fully eversible introvert capable of remarkable elongations. High-resolution micro-computed tomography was used to resolve the internal musculoskeletal architecture across functional configurations. Morphometric analyses of live specimens revealed strong differentiation between body regions: trunk length remains nearly constant during motion (7.26 ± 3.40 mm retracted vs. 7.70 ± 3.47 mm extended), whereas total body length

Highly Mechanically Robust Human-Derived Hydrogels for Tissue Engineering Applications.

Hydrogels are widely used in tissue engineering (TE) for their ability to replicate the extracellular matrix, providing a supportive environment for cell proliferation and tissue regeneration. However, their inherently low mechanical strength often restricts their use in load-bearing biomedical applications. To address these limitations, this study aimed to develop the first human-derived double-network (DN) hydrogel by combining human methacryloyl platelet lysates (hPLMA) and the natural polysaccharide chitosan (CHT) through sequential covalent and ionic crosslinking. The resulting DN hydroge

Beyond the capsule: an integrated perspective on the wide world of frozen shoulder. A collaborative viewpoint.

BACKGROUND: Frozen Shoulder (FS) is a highly disabling glenohumeral condition marked by severe pain and restricted active and passive motion, typically without significant radiological changes. While conventionally viewed as a self-limiting and idiopathic disorder, recovery is often incomplete, suggesting that peripheral-only interventions are insufficient. OBJECTIVE: This manuscript reviews the contemporary understanding of FS etiopathogenesis, clarifying the underlying pain processing mechanisms, metabolic patterns, and psychological domains. Evidence about diagnostic tests and imaging for

Imaging of Anterior Chest Wall Inflammatory Disease: Expert Recommendations.

ABSTRACT: Inflammatory conditions of the anterior chest wall are clinically relevant but remain underrecognized and inconsistently assessed in imaging protocols. The anterior chest wall may be affected by systemic and localized diseases, such as spondyloarthritis, chronic nonbacterial osteitis, rheumatoid arthritis, crystal arthropathies, and osteoarthritis. In spondyloarthritis, anterior chest wall involvement is common yet frequently subclinical. Magnetic resonance imaging enables early detection of enthesitis, synovitis, and osteitis, as well as structural lesions, such as erosions, fatty m

Does effective therapy of psoriasis prevent the development of psoriatic arthritis?

Psoriasis frequently precedes psoriatic arthritis (PsA), delineating a tractable window for disease interception. This narrative review integrates mechanistic, imaging, and clinical evidence, together with expert interpretation, to examine whether effective treatment of psoriasis may modify the risk of PsA development. Converging immunopathogenic insights link skin and joint inflammation through interleukin (IL)-23/IL-17-driven pathways, enthesis-synovium crosstalk, and tissue-resident memory T-cell biology. Clinically, progression from psoriasis with arthralgia to PsA occurs in ∼20% of patien

Co-Occurring Non-Cardiac Congenital Anomalies Among Cases With Congenital Heart Defects.

Cases with congenital heart defects (CHD) often have other associated anomalies. The aim of this investigation was to assess the prevalence and the types of co-occurring anomalies in CHD in a well-defined population. The anomalies co-occurring with CHD were ascertained in all live births, stillbirths and terminations of pregnancy for fetal anomaly during 29 years in 387,067 consecutive pregnancies of known outcome in the area covered by our population-based registry of congenital anomalies. Of the 4640 cases with CHD ascertained during this period (prevalence of 119.9 per 10,000), 1228 (26.5%)

Anti-Melanoma Differentiation-Associated Gene 5 Antibody-Positive Juvenile Dermatomyositis Presenting With Predominant Joint Contractures.

INTRODUCTION: Anti-melanoma differentiation-associated gene 5 (MDA5) positive juvenile dermatomyositis (JDM) (anti-MDA5+ JDM) is a distinct subtype of JDM characterized by marked clinical heterogeneity. Although cutaneous manifestations and interstitial lung disease (ILD) are well recognized, atypical musculoskeletal presentations may lead to diagnostic delay. CLINICAL CASE: We report a 16-year-old male with anti-MDA5+ JDM who presented with progressive joint contractures as the predominant manifestation over a 2-year period. The patient also exhibited restricted mouth opening, with a classic

Ochronotic arthropathy mimicking degenerative osteoarthritis in an older adult with alkaptonuria: diagnostic and perioperative lessons from a case report.

BACKGROUND: Alkaptonuria is a rare inherited metabolic disorder caused by impaired homogentisic acid metabolism. Ochronotic arthropathy is a late musculoskeletal manifestation and may mimic ordinary degenerative osteoarthritis in older adults. This diagnostic overlap may delay recognition of the underlying systemic metabolic disease. CASE PRESENTATION: A 67-year-old woman presented with progressive pain and restricted motion of the left knee and hip for four years. Preoperative imaging suggested advanced degenerative arthropathy. However, she had a long-standing history of urine darkening aft

Anatomopathological aspects of osteoarticular lesions associated with Rhodococcus equi infection in two foals.

Rhodococcus equi is an important bacterium that affects foals up to six months of age, especially those that are immunocompromised. The main manifestation is pyogranulomatous pneumonia; nevertheless, extrapulmonary lesions have not only been reported, but are not uncommon, including arthritis and osteomyelitis. Despite the widespread occurrence of rhodococcosis, the macro- and microscopic lesions in the musculoskeletal system are still poorly understood, and a detailed study is needed to better understand the pathogenic potential of the organism. The aim of this report is to present the anatom…

Exploring the role of glial cells and inflammatory cytokines in chronic muscle pain from rodent models.

Musculoskeletal disorders are among the leading causes of disability worldwide, yet their underlying mechanisms remain incompletely understood. Experimental models of gastrocnemius muscle injury have been widely used to investigate the pathophysiology of chronic muscle pain. This study aimed to evaluate histological alterations in muscle tissue, nociceptive sensitivity, locomotor activity, and the role of spinal glial cells and inflammatory mediators following the induction of chronic myositis. Histological analyses revealed marked inflammation in the muscle tissue of myositis animals. Behavio
