Impact of long-term nasopharyngeal airway on health-related quality of life of children with obstructive sleep apnea caused by syndromic craniosynostosis.

Obstructive sleep apnea in children with syndromic craniosynostosis is a well-documented entity, and airway management in these group of children is difficult, with well-documented cardiorespiratory and neurodevelopment consequences. Numerous well-documented techniques are available for airway management in this group of children. In our center, the nasopharyngeal airway (NPA) is the first-line treatment. This study aimed to evaluate the improvement in health-related quality of life in this group of children using the Glasgow Children's Benefit Inventory. We conducted a retrospective postal st

Tiredness and somnolence despite initial treatment of obstructive sleep apnea syndrome (what to do when an OSAS patient stays hypersomnolent despite treatment).

From a database of 4,129 patients with sleep-disordered breathing (SDB), we found 207 subjects (43 women) that still complained of daytime tiredness, fatigue, and/or sleepiness despite treatment. In 25 subjects the sleepiness developed 1 to 36 months following treatment and was related to noncompliance (8 subjects), significant weight increase and/or inappropriate treatment (10 subjects), or development of new medical problems (7 subjects). In the remaining 182 subjects, sleepiness was noted within 1 month after what was judged appropriate treatment for SDB. In this group, the reason for persi

Impact of obstructive sleep apnea on cardiometabolic health in a random sample of older adults in rural South Africa: building the case for the treatment of sleep disorders in underresourced settings.

STUDY OBJECTIVES: The association between obstructive sleep apnea (OSA) and increased cardiometabolic risk (CMR) has been well documented in higher-income countries. However, OSA and its association with CMR have not yet been investigated, based on objective measures, in southern Africa. We measured polysomnography-derived sleep characteristics, OSA prevalence, and its association with cardiometabolic diseases in a rural, low-income, African-ancestry sample of older adult participants in South Africa. METHODS: Seventy-five participants completed the study. Body mass index, hypertension, diabe

The prevalence of obstructive sleep apnea in sarcoidosis and its impact on sleepiness, fatigue, and sleep-associated quality of life: a cross-sectional study with matched controls (the OSASA study).

STUDY OBJECTIVES: Patients with sarcoidosis experience fatigue and excessive daytime sleepiness (EDS). However, the underlying pathomechanism is unclear. Studies suggested undiagnosed obstructive sleep apnea (OSA) to be an important contributor, but reliable data on prevalence and impact of OSA in sarcoidosis are scarce. METHODS: 71 adult patients with sarcoidosis, 1-to-1 matched to 71 adult controls according to sex, age, and body mass index were included. Participants underwent structured interviews (including Epworth Sleepiness Scale [ESS], Fatigue Assessment Scale [FAS], and Functional Ou

Re-Evaluating Positive Airway Pressure Indications in Obstructive Sleep Apnea: A Cross-Classification Analysis of Polysomnographic Severity and the Baveno Phenotypic Classification.

Background/Objectives: Obstructive sleep apnea (OSA) is traditionally diagnosed via polysomnography (PSG) using the apnea-hypopnea index (AHI). However, AHI alone may inadequately reflect the clinical heterogeneity, symptom burden, and cardiometabolic consequences of the disease. The Baveno classification incorporates symptom severity and comorbidity burden into a multidimensional framework. We aimed to evaluate the agreement between PSG-staging and the Baveno classification, to investigate the impact of reclassification on positive airway pressure (PAP) treatment recommendations. Methods: Thi

Impact of obstructive lung disease and sleep apnea symptoms on cardiovascular risk and all-cause mortality: insights from a community-dwelling cohort.

PURPOSE: Chronic obstructive pulmonary disease (COPD) and obstructive sleep apnea (OSA) are prevalent conditions with overlapping pathophysiological mechanisms. Their coexistence, termed overlap syndrome, is thought to amplify cardiometabolic risk. This study examined the 10-year risk of major adverse cardiovascular events (MACE) and all-cause mortality in individuals with COPD and OSA symptoms in a community-based cohort. METHODS: Baseline data (1998-1999) from the Hordaland Health Study were linked to national registries on mortality and cardiovascular events. Of 7,456 eligible adults born

Towards automatic home screening of obstructive sleep apnea using combined features from pulse wave amplitude, pulse-to-pulse interval and oxygen desaturation.

PURPOSE: Automatic home OSA screening with pulse oximeters often relies on oxygen saturation (SpO2), which may miss hypopneas associated with arousals. Photoplethysmography (PPG) from pulse oximeters can be processed to extract pulse wave amplitude (PWA) and pulse-to-pulse interval (PPI) which reflect autonomic activation during arousals. However, the role of PWA in automated OSA screening remains unexplored. This study evaluated the added value of PWA for detecting OSA segments, and estimating AHI compared with SpO2 and PPI. METHOD: From 90 PSG recordings, PWA and PPI were derived from finger

The impact of dexmedetomidine on postoperative delirium in obstructive sleep apnea patients (DPOD-OSA): study protocol for a prospective randomized controlled trial.

BACKGROUND: Recent studies suggest that patients at intermediate-high risk of obstructive sleep apnea (OSA) are at greater risk of developing postoperative delirium (POD). The administration of dexmedetomidine (DEX) is suggested to prevent POD in the normal population. However, few studies have investigated the impact of DEX on POD in OSA patients. METHODS/DESIGN: A prospective, randomized controlled trial will be conducted at a single center. Patients scheduled for elective laparoscopic surgery will be screened with the STOP-Bang questionnaire (SBQ). Patients who are intermediate-high risk f

Association of non-high-density lipoprotein cholesterol to high-density lipoprotein cholesterol ratio and atherogenic index of plasma with obstructive sleep apnea.

BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent sleep disorder associated with metabolic, cardiovascular disorders. The non-high-density lipoprotein cholesterol to high-density lipoprotein cholesterol ratio (NHHR) and atherogenic index of plasma (AIP) are thought to be better ways to detect cardiovascular diseases than single lipid parameters. The relationship between NHHR or AIP and OSA remains ambiguous. METHODS: 454 consecutive patients with suspected OSA were enrolled between June 2023 and February 2025. The OSA was defined as the occurrence of more than 5 apnea-hypopnea index (A
