Prognostic value of thumb pain sensation in birth brachial plexopathy.

OBJECTIVE: To evaluate the prognostic value of absent thumb pain sensation in newborns and young infants with birth brachial plexopathy. METHODS: We evaluated 131 patients with birth brachial plexopathy with less than two months of age. Pain sensation was evoked by thumb nail bed compression to evaluate sensory fibers of the upper trunk (C6). The patients were followed-up monthly. Patients with less than antigravity elbow flexion at six months of age were considered to have a poor outcome. RESULTS: Thirty patients had absent thumb pain sensation, from which 26 showed a poor outcome. Sensitiv

Total obstetric brachial plexus palsy in children with internal rotation contracture of the shoulder, flexion contracture of the elbow, and poor hand function: improving the cosmetic appearance of the limb with rotation osteotomy of the humerus.

Rotation osteotomy of the humerus has been described by several authors to treat the internal rotation contracture of the shoulder in Erb palsy. The main aim of the osteotomy in Erb patients is to bring the functioning hand to the face which greatly improves function. The author has performed the rotation humeral osteotomy in children with total obstetric brachial plexus palsy aiming for the improvement of the cosmetic appearance of the limb rather than improvement function. This article specifically reports on this group of patients.Over the last 15 years, the author has performed rotation hu

Osteochondroma associated with complete rupture of the distal biceps tendon: case report.

Osteochondromas are the most common benign bone tumors. However, osteochondromas rarely involve the proximal radius. We present the case of a solitary osteochondroma that occurred in a critical area of the proximal radius near the insertion of the biceps tendon and ultimately led to rupture of the tendon. The lesion was confirmed histologically and resected, followed by successful repair of the distal biceps tendon using a suture anchor.

Simple elbow dislocation among adults: a comparative study of two different methods of treatment.

Post-manipulation treatment of elbow dislocation includes plaster of Paris immobilisation for a mean of 2 weeks followed by physiotherapy, or sling support followed by early mobilisation. This study retrospectively reviewed 42 simple elbow dislocations. The management of 20 patients by the plaster of Paris method and 22 by the sling method was assessed after a minimum follow-up of 2 years using Mayo Elbow Performance Index (MEPI) scores, the Quick Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire and time off work. The final functional outcome in the plaster of Paris group showed

Shoulder and elbow function 2 years following long head triceps interposition flap transfer for massive rotator cuff tear reconstruction.

INTRODUCTION: Patient outcomes were determined at a minimum of 2 years following massive rotator cuff tear ( > or = 5 cm(2)) reconstruction using a triceps brachii long head interposition flap. A physiotherapist at an independent clinic performed all measurements. PATIENTS AND METHODS: Of 24 total eligible patients, 19 completed pre-operative and follow-up UCLA Shoulder Scores and ASES Function Scores. Of these 19 patients, 14 who did not have co-morbidities that negatively impacted their ability to perform intense exercise underwent a comprehensive clinical examination of involved and uninvo

Idiopathic or traumatic olecranon bursitis. Clinical features and bursal fluid analysis.

Thirty cases of idiopathic olecranon bursitis were studied. Most had previous local trauma. The process was unilateral and often associated with nontender pitting edema in cases of short duration. Ten patients exhibited a bony spur at the olecranon process, and amorphous calcific deposits were seen in 6. The bursal fluid was hemorrhagic with a xanthochromic supernatant, and the mucin clot test was poor or fair. Leukocyte count averaged 878/mm3, predominantly mononuclears. Many cells contained inclusion bodies. Glucose, total protein, and complement (C3) concentration averaged 80, 60, and 60% o

Humeral Fractures in Metabolically Compromised Bone: Outcomes of Structural Strut Onlay Allograft with Open Reduction and Internal Fixation - A Case Series of Three Patients.

INTRODUCTION: The "terrible tetrad" of the elbow - simultaneous posterior ulno-humeral dislocation, comminuted radial head fracture with dislocation, coronoid process fracture, and proximal ulnar fracture - is an injury pattern that simultaneously fulfills criteria for both the terrible triad and a Monteggia-type fracture-dislocation. Fewer than fifteen cases have been documented worldwide. We present two cases managed by different surgeons at different institutions and propose the formal adoption of the term "terrible tetrad." CASE REPORT: Case 1: A 50-year-old male painter sustained a right

Localizing the course of the radial nerve based on anatomical landmarks: A cadaveric study.

BACKGROUND: Surgeons treating humeral shaft fractures must be able to reliably locate the radial nerve in the posterior arm to limit iatrogenic injury. This study aims to improve the understanding of radial nerve anatomy by increasing the sample size, comparing bilateral upper extremities, and analyzing the impact of sex on its location. METHODS: Twenty-five pairs (n = 50) of fresh cadaveric upper extremities were studied with the cadaver in the lateral position. The shoulder was abducted and internally rotated and the elbow flexed to 90° and supported, as done intraoperatively. A triceps-spl

Motivational-addictive profiles of nonsuicidal self-injury in Chinese youth: a cluster analysis with validation using external correlates.

BACKGROUND: Nonsuicidal self-injury (NSSI) is heterogeneous, yet clinically useful exploratory subgrouping remains limited. We examined whether motivational and addiction-like features could delineate meaningful data-driven NSSI profiles and relate to clinically relevant outcomes. METHODS: We clustered six motive dimensions of the Ottawa Self-Injury Inventory (OSI; F1-F6) and an addiction-like score in adolescents (N = 311; age range: 12-21 years; mean age = 15.15 years, SD = 2.60). A three-cluster solution was retained based on a multi-criterion evaluation of internal fit indices, cluster-si

Restoring musculocutaneous nerve function in 146 brachial plexus operations - A retrospective analysis.

INTRODUCTION: A brachial plexus lesion is a devastating injury often affecting young, male adults after traffic accidents. Therefore, surgical restoration of elbow flexion is critical for establishing antigravity movement of the upper extremity. We analyzed different methods for musculocutaneous reconstruction regarding outcome. METHODS: We conducted a retrospective analysis of 146 brachial plexus surgeries with musculocutaneous reconstruction performed at our department from 2013 to 2017. Demographic data, surgical method, donor and graft nerve characteristics, body mass index (BMI) as well

Conservative management of distal bicipital tendinopathy with lateral antebrachial nerve entrapment: A case report.

INTRODUCTION: Kettlebell snatches are an efficient and effective exercise. If the kettlebell being utilized is too heavy or too many repetitions are executed, this can lead to an overuse injury such as a tendinopathy. Multiple orthopedic tests exist to evaluate for a distal biceps tendon rupture. At present, there are no publications utilizing shockwave and active rehabilitation to treat distal bicipital tendinopathy with Lateral Antebrachial Cutaneous Nerve (LABCN) entrapment. Currently, no published manuscripts are reporting distal bicipital tendinopathy with LABCN nerve entrapment being tre

Results of Locking Compression Plate (LCP) Fixation in Humeral Shaft Nonunion.

Although nonoperative treatment is indicated & successful for the majority of diaphyseal humeral fractures, nonunion is not rare condition. The prevalence of nonunion as a complication of conservative treatment has been reported to be as high as 15%. Locking compression plate combined with autogenous cancellous bone grafting can result in reliable healing of these humeral nonunion with excellent functional outcome. This prospective observational study was conducted September, 2016 to October, 2018 at National Institute of Traumatulogy & Orthopaedic Rehablitation, Dhaka & Mymensingh Medical Col

Electrophysiological study of the dominant motor innervation to the extensor digitorum communis muscle and long head of triceps brachii at posterior divisions of brachial plexus.

BACKGROUND: Restoration of elbow and finger extension function is still challenging in management of complete brachial plexus avulsion injury, mainly because of fewer available donor nerves for transfer to the radial nerve. Selective neurotization could be a potentially alternative for overcoming this dilemma. This study was designed to identify the innervation dominance of the extensor digitorum communis muscle (EDCM) and long head of the triceps brachii (LTB) at the level of division of brachial plexus. METHODS: From February 2008 to October 2009, 17 patients with complete brachial plexus a

Effects of velocity on upper to lower extremity muscular work and power output ratios of intercollegiate athletes.

OBJECTIVES: Peak torque expresses a point output which may, but does not always, correlate well with full range output measures such as work or power, particularly in a rehabilitating muscle. This study evaluates isokinetic performance variables, particularly (a) flexor to extensor work and power output ratios of upper and lower extremities and (b) overall upper to lower extremity work and power ratios, in intercollegiate athletes. The purpose was to ascertain how speeds of 30 and 180 degrees/s influence agonist to antagonist ratios for torque, work, and power and to determine the effects of t
