Mosquito vectors of West Nile Fever in Israel.

West Nile fever (WNF) is endemic in Israel. In 1999, country-wide adult mosquito surveys were initiated and intensified after the 2000 country-wide outbreak of WNF in humans. In 8 consecutive yr, groups of male and female specimens of different species and from different locations were tested for infection with West Nile virus (WNV). Three species made up >87% of the total catch: Culex pipiens L. (52%), with an infection rate (IR) of 0.5; Cx. perexiguus Theobald (20%), with an IR of 2.7; and Aedes caspius Pallas (15%), with an IR of 0.6. The geographical and temporal distribution of WNV-infect…

Acute Q fever in children presenting with encephalitis.

We describe the cases of three children with encephalitis associated with Coxiella burnetii infection. Neurologic features ranged from status epilepticus and coma to headache and pseudotumor cerebri syndrome. Patients had good response to antibiotic treatment with doxycycline and recovered fully. Q fever should be included in the differential diagnosis of children with encephalitis, and routine serological testing should be considered, especially in endemic areas.

Suspected Crimean Congo Haemorrhagic Fever cases in Albania.

During 2003 to 2006 samples from 34 Albanian patients with suspected Crimean Congo Hemorrhagic Fever (CCHF) were tested by serology and PCR for CCHF virus; negative samples were further tested for hantaviruses, Leptospira spp. and Rickettsia spp. CCHF virus was detected in 38.2% of cases, hantaviruses in 11.7%, and leptospirosis and rickettsiosis were diagnosed in 29.4% and 2.9% of cases, respectively. There is a seasonal and clinical overlapping among the 4 diseases in Albania, suggesting that testing for these agents is necessary in cases with fever and haemorrhagic manifestations.

[Analysis of fatal outcomes issues of typhus fever].

The nature and frequency of complications in patients with typhus fever are of importance for its outcome. The causes of death were studied in 126 patients who had died from this disease. The immediate causes of death were as follows: infectious-toxic shock (31.7%), pneumonia (28.6%), and perforative peritonitis (26.2%). The pathomorphism of Payer's patches does not allow the stages of the disease to be identified. An immunohistochemical study established a drastic preponderance of granulomatous macrophages without proliferation markers with a simultaneous reduction in the count of T and B lym…

Immunological, cellular and molecular events in typhoid fever.

Salmonella, a facultative intracellular Gram-negative bacterium infects a wide range of hosts causing several gastrointestinal diseases and enteric fever in humans and certain animal species. Typhoid caused by Salmonella typhi remains a major health concern in India and worldwide. Also, with emergence of multidrug resistant strains, Salmonella has acquired increased virulence, communicability and survivability, resulting in increased morbidity and mortality. Though a number of vaccines for typhoid are available against S. typhi (or also against S. typhimurium), these have certain undesirable s…

Subclinical valvulitis in children with acute rheumatic Fever.

Doppler echocardiography facility is now available in most parts of the world and its routine use for the initial diagnosis of acute rheumatic fever (ARF) might enhance its early detection and, hence, prevention of rheumatic recurrences. To add to the existing evidence and to reemphasize the need of including echocardiography as a diagnostic criterion for ARF, we investigated the pattern and prevalence of subclinical valvulitis by Doppler echocardiography in patients with ARF manifesting as either pure chorea or isolated arthritis but without clinical signs of carditis. This prospective study …

Characterization and clinical management of clozapine-induced fever.

OBJECTIVE: To characterize clozapine-induced fever and suggest clinically relevant management recommendations. DATA SOURCES: Literature was accessed through MEDLINE (1966-June 2007) using the terms clozapine, fever, and adverse effects. In addition, reference citations from publications identified were reviewed. STUDY SELECTION AND DATA EXTRACTION: All English-language articles about human studies of fever associated with the use of clozapine were evaluated. DATA SYNTHESIS: Mild to high-grade fever frequently accompanies clozapine therapy. Fever usually occurs within 10-15 days after treatm…

[Neck pain and fever after peginterferon alpha-2a].

A 39-year-old woman presented to the emergency room with fever and tender swollen neck two days after the first injection of Peginterferon alpha-2a (Pegasys). She had been under Interferon therapy for melanoma over the last five months. Because of side effects Interferon alpha-2a (Roferon-A) was discontinued and she was given Peginterferon alpha-2a. On examination, she was unwell, febrile (38.8 degrees C) and her pulse was 110/min. Laboratory findings were as follow: erythrocyte sedimentation rate 85 mm/h, C-reactive protein 143 mg/L, white cell count of 11 x 10(9)/L (3.0-9.6), TSH 0.10 mU/L (…

Dengue hemorrhagic fever with special emphasis on immunopathogenesis.

Dengue virus infections are a serious cause of morbidity and mortality in most tropical and subtropical areas of the world; Southeast and South Asia, Central and South America, and the Caribbean. Dengue virus infection can be asymptomatic or causes two forms of illness, dengue fever (DF) and dengue hemorrhagic fever (DHF), which is the severe form of dengue illness and often fatal. Pathogenesis of DHF has been analyzed, and two mechanisms are considered to be responsible. These include dengue serotype cross-reactive immune responses and virulence of the virus. The immunopathological mechanisms…

Pulmonary haemorrhage syndrome associated with dengue haemorrhagic fever.

Dengue fever is a major public health problem in India. Dengue haemorrhagic fever (DHF), a more serious form of disease, occurs when a person previously infected with dengue is reinfected with a different serotype. Besides common manifestations pleural effusion, pneumonitis, haemoptysis and pulmonary haemorrhage have rarely been seen. We report a case of 30 years old male, who developed pulmonary haemorrhage, haemoptysis requiring blood transfusion. Serology was consistent with the diagnosis of dengue haemorrhagic fever.

Characterization of Rift Valley fever virus transcriptional terminations.

Rift Valley fever virus (RVFV) (genus Phlebovirus, family Bunyaviridae) has a tripartite negative-strand genome and causes a mosquito-borne disease among humans and livestock in sub-Saharan African and Arabian Peninsula countries. Phlebovirus L, M, and N mRNAs are synthesized from the virus-sense RNA segments, while NSs mRNA is transcribed from the anti-virus-sense S segment. The present study determined the 3' termini of all RVFV mRNAs. The 3' termini of N and NSs mRNAs were mapped within the S-segment intergenic region and were complementary to each other by 30 to 60 nucleotides. The termini…

Spotted fever rickettsioses in southern and eastern Europe.

Mediterranean spotted fever due to Rickettsia conorii conorii was thought, for many years, to be the only tick-borne rickettsial disease prevalent in southern and eastern Europe. However, in recent years, six more species or subspecies within the spotted fever group of the genus Rickettsia have been described as emerging pathogens in this part of the world. Tick-borne agents include Rickettsia conorii israelensis, Rickettsia conorii caspia, Rickettsia aeschlimannii, Rickettsia slovaca, Rickettsia sibirica mongolitimonae and Rickettsia massiliae. Many Rickettsia of unknown pathogenicity have al…

Evaluating fever of unidentifiable source in young children.

Most children will have been evaluated for a febrile illness by 36 months of age. Although the majority will have a self-limited viral illness, studies done before the use of Haemophilus influenzae type b and Streptococcus pneumoniae vaccines showed that approximately 10 percent of children younger than 36 months without evident sources of fever had occult bacteremia and serious bacterial infection. More recent studies have found lower rates of bacterial infection (1.6 to 1.8 percent). Any infant younger than 29 days and any child that appears toxic should undergo a complete sepsis work-up. Ho…
