Objective Outcome Measures Continue to Improve from 6 to 12 Months after Conservatively Treated Distal Radius Fractures in the Elderly-A Prospective Evaluation of 50 Patients.

Distal radius fractures (DRF) in the elderly population above 65 years represent 18% of all fractures and are thereby the second most frequent fracture in the elderly. Fracture dislocation and comminution are often used to determine whether non-operative or operative treatment is indicated. The purpose of this prospective case series of minimally displaced DRF treated with a dorsal cast was to assess the complication rate and patient-reported outcome measures. This single-centre, single-blinded, prospective case series followed 50 conservatively treated DRF patients for one year. Primary outco…

Combined circular external fixation and open reduction internal fixation with pro-syndesmotic screws for repair of a diabetic ankle fracture.

The surgical management of ankle fractures among the diabetic population is associated with higher complication rates compared to the general population. Efforts toward development of better methods in prevention and treatment are continuously evolving for these injuries. The presence of peripheral neuropathy and the possible development of Charcot neuroarthropathy in this high risk patient population have stimulated much surgical interest to create more stable osseous constructs when open reduction of an ankle fracture/dislocation is required. The utilization of multiple syndesmotic screws (p

Agreement in the interpretation of extremity radiographs of injured children and adolescents.

OBJECTIVES: To measure agreement beyond chance (kappa) for comparison interpretations of extremity radiographs by pediatric radiologists and emergency physicians (EPs) and to identify factors associated with disagreement. METHODS: A random sample of 205 radiographs was selected from 1,016 patients having x-rays of their extremities in the emergency and radiology departments of a tertiary care pediatric hospital. Interpretations by the "official" reporting pediatric radiologist (ORPR), the treating EP, and a pediatric radiologist blinded to all clinical information (BPR) were compared for thre

[Mechanical efficacy of different internal fixation devices in the treatment of thoracolumbar fractures].

To know the efficacy of different internal fixation devices used in the treatment of thoracolumbar fractures, specimens of 15 adult cadaveric (T-8 to L-4) were prepared and were divided into 5 groups for comparative study. Except the control group, the vertebra of T-12 of every specimen in other 4 groups was artificially fractured, imitating flexion fracture dislocation, with standard technic. Then, they were fixed in each group, with Luque's rod, Dick's screw, pedicle screw plate or Dunn's device respectively. The stress distribution and yielding stress under load and dimensional motions in e…

[Distal radius fracture--indications for conservative, partial surgical and surgical therapy].

460 fractures of the distal radius including 91 which are treated surgically are investigated in a retrospective study. There were 260 fractures of the left and only 200 of the right radius. 66% of the patients were female, especially striking is the superior number of women older than 45 years. The evaluation of the final radiological results referring to anatomical joint angles and normal radius length shows a high correlation to the evaluation of the final functional results. 70% of the latter are valued as excellent or good, 19% as tolerable and 11% as poor. The classification according to

The spanning plate as an internal fixator in complex distal radius fractures: a prospective cohort study.

OBJECTIVE: Minimal invasive temporary spanning plate (SP) fixation of the wrist has been described as an alternative treatment method in complex distal radius fractures (DRFs). The purpose of this study is to conduct an outcome analysis of all consecutive DRFs treated by SP fixation representing the so far largest published patient cohort outside the United States. METHODS: Indication for SP fixation include DRFs with severe metaphyseal comminution, radiocarpal fracture dislocations with concomitant ligamentous injuries and very distal intra-articular fractures lacking the possibility of adeq…

[Surgical treatment of the medial epicondyle fracture of the humerus using the ''Inside-out'' technique. Description of the technique and results].

INTRODUCTION: the "Inside-out" technique is one of the options for medial epicondyle fracture fixation of the humerus. It involves preparing the fixation from the internal portion of the fracture to the external portion. Then, the fracture is fixed with a cannulated screw from the external to the internal portion. This study aims to describe our experience with this technique, its outcomes, and its complications. MATERIAL AND METHODS: this is a retrospective observational study describing a series of cases treated with the "Inside-out" technique for medial epicondyle fracture fixation from 20

Predicting redisplacement after manipulation of paediatric distal radius fractures: the importance of cast moulding.

INTRODUCTION: The majority of displaced distal radius fractures are managed by closed reduction and cast immobilisation. Redisplacement is associated with initial displacement, imperfect reduction and quality of cast. The aim of this study was to establish which factors predict the risk of redisplacement. MATERIALS AND METHODS: A retrospective analysis between September 2010 and April 2013 of children who underwent closed manipulation and cast immobilisation for a distal third radius fracture was performed. Open fractures, those treated with fixation, and cases with associated dislocations or

Management of distal humeral coronal shear fractures.

Coronal shear fractures of the distal humerus are rare, complex fractures that can be technically challenging to manage. They usually result from a low-energy fall and direct compression of the distal humerus by the radial head in a hyper-extended or semi-flexed elbow or from spontaneous reduction of a posterolateral subluxation or dislocation. Due to the small number of soft tissue attachments at this site, almost all of these fractures are displaced. The incidence of distal humeral coronal shear fractures is higher among women because of the higher rate of osteoporosis in women and the diffe

Complex pediatric orbital fractures combined with traumatic brain injury: treatment and follow-up.

The treatment of orbital fractures aims at the restoration of orbital anatomy and prevention of posttraumatic sequels. The treatment of facial fractures in patients with traumatic brain injury may necessitate a postponement of fracture surgery to allow for brain recovery. However, such delay of reconstruction in complex orbital fractures may lead to inferior results. Fourteen pediatric patients with complex orbital fractures were retrospectively reviewed. Eleven patients (79%) had brain injury, with a mean Glasgow Coma Scale score of 8.6 at initial evaluation. Fracture patterns were highly com

Operative treatment of displaced proximal humeral fractures: two-year results in 99 cases.

The operative treatment of comminuted and displaced fractures of the proximal humerus has been evolving in recent years. Classical open reduction and internal fixation techniques have an increased risk of avascular necrosis. Minimal osteosynthesis procedures often result in a suboptimal fracture reduction and require postoperative immobilization of the arm in some cases. This study reviewed ninety-nine out of 142 patients (70%), an average of 30 months (range 12 to 72 months) after indirect reduction and internal fixation of two-, three- or four-part fractures of the proximal humerus. The surg

Adverse Events After Surgical Treatment of Adult Diaphyseal Forearm Fractures: a Retrospective Analysis of 470 Patients.

BACKGROUND: The incidence of and risk factors for adverse events after internal fixation of diaphyseal forearm fractures have not been well defined in the current literature. The objective of this study was to estimate the incidence of adverse events after diaphyseal forearm fracture surgery in adults and explore potential risk factors for adverse events. METHODS: We conducted a retrospective, multicenter, cohort study in which we evaluated all diaphyseal forearm fractures between 2009 and 2019 in patients presenting to 4 trauma hospitals in southern Finland. Patients <16 years of age and fra

From Missed Diagnosis to Optimal Outcomes: A Comprehensive Review of Lisfranc Injuries.

Lisfranc injuries are traumatic disruptions of the tarsometatarsal joint complex. Injuries range from subtle ligamentous injuries to significant fracture dislocations. These relatively uncommon injuries are frequently missed during the initial presentation in emergency departments. Delayed or inadequate treatment can lead to chronic pain, midfoot arthritis, and significant functional disability. Early recognition and appropriate management of Lisfranc injuries are crucial for achieving optimal outcomes. Treatment strategies range from conservative management for stable injuries to surgical int…

[Long fingers proximal interphalangeal joint trauma].

Sprains and dislocations of the proximal interphalangeal joint (PIP) are very common but are, nevertheless, often missed. They require an appropriate treatment to prevent stiffness and deformities. Initial assessment should include anteroposterior and true lateral radiographs. Intra-articular fractures are referred to the specialist. Clinical examination to detect laxity is essential and will guide the treatment. The treatment is based on the restoration of joint congruency and achieving early mobilization. Surgical indication is rare. The evolution is slow, pain and joint swelling can persist

Prevalence and current concepts of management of farmyard injuries.

Farmyard injuries in young adults and the elderly usually result from machinery accidents, whereas children tend to be injured by runovers and motor vehicle collisions. A variety of farmyard injuries secondary to environmental and human factors may cause permanent disability or even death. Common injuries include lacerations, hand injuries, amputations, farm animal bites, fractures and dislocations. Special considerations should be taken into account when treating these injuries, focusing on their noteworthy bacterial load in order to minimise the risk of chronic morbidity.

Proximal interphalangeal joint injuries of the hand. Part I: anatomy and diagnosis.

Injury to the proximal interphalangeal (PIP) joint is one of the most common hand injuries encountered by orthopedic surgeons. The injuries are often misdiagnosed, resulting in delayed treatment and permanent impairment. The different types of PIP-joint fractures and dislocations have been well described, and a variety of therapeutic options have been proposed. Early detection and aggressive rehabilitation are the cornerstones of successful treatment. This is the first of a two-part series reviewing PIP injuries. We describe the anatomy and biomechanics of the PIP joint and review the classifi
