The Role of Nutraceuticals and Probiotics in Addition to Lifestyle Intervention in the Management of Childhood Obesity-Part 2: Comorbidities.

Pediatric obesity is associated with a wide range of comorbidities beyond metabolic changes, affecting cardiovascular, endocrine, reproductive, musculoskeletal systems, and also mental health. Hypertension, commonly observed in children with obesity, increases the risk of long-term cardiovascular disease. Polycystic ovary syndrome (PCOS) presents another significant endo-reproductive challenge that often develops during adolescence in females, leading to further comorbidities in adulthood. Additionally, excess adiposity can negatively impact bone health by modifying bone metabolism and increas…

Therapeutic effect of mesenchymal stem cells and their derived exosomes in diseases.

Mesenchymal stem cells (MSCs) are multipotent stem cells characterized by their robust proliferative capacity, homing ability, differentiation potential, and low immunogenicity in vitro. MSCs can be isolated from a variety of tissues, primarily including but not limited to bone marrow, adipose tissue, umbilical cord, placenta, and dental pulp. Although there have been a large number of clinical studies on the treatment of diseases by MSCs and MSCs-derived exosomes (MSCs-EXO), the large-scale clinical application of MSCs and MSCs-EXO have been limited due to the heterogeneity of the results amo

Phase-Specific Biomechanical Characterization of Upper Limb Movements in Stroke.

Stroke often leads to persistent upper limb dysfunction that impairs activities of daily living, yet objective biomechanical indicators for precise assessment remain limited. This study aimed to characterize phase-specific impairments in energy output, torque stability, and muscle coordination during the hand-to-mouth (HTM) task and to explore their potential for improving rehabilitation evaluation. Motion data from 20 stroke patients and 20 healthy controls were recorded using wearable surface electromyography and inertial measurement unit systems. A musculoskeletal model was applied to calcu

Androgen receptor pathway signaling inhibitors in development for prostate cancer therapy.

INTRODUCTION: Prostate cancer (PC), including castration-resistant disease (CRPC), remains largely driven by dysregulated androgen receptor (AR) signaling. While androgen deprivation therapy (ADT) combined with next-generation AR inhibitors improves survival, resistance inevitably arises through mechanisms such as AR amplification, mutations, and splice variants. Additionally, chronic AR suppression induces significant metabolic, musculoskeletal, and cardiovascular toxicities, highlighting the need for novel therapies that overcome resistance while minimizing systemic adverse effects. Pharmacological Aspects in the Management of Children and Adolescents with Prader-Willi Syndrome.

Prader-Willi syndrome is a rare neurodevelopmental disorder that impacts the musculoskeletal, endocrine, pulmonary, neurologic, ocular, and gastrointestinal systems. In addition, individuals with Prader-Willi syndrome have issues with cognitive development, characteristic behavioral problems, and perhaps most profoundly, appetite control. Currently, the only US Food and Drug Administration-approved therapy for Prader-Willi syndrome is growth hormone, which has been Food and Drug Administration approved for > 20 years for the treatment of growth failure in Prader-Willi syndrome. Growth hormone

Epidemiological and Histopathological Characteristics of Fetuses with Congenital Disorders: A Study in Greece.

Congenital malformations constitute a major public health issue. Nonetheless, Greece does not participate in the European network of congenital anomalies. To examine the epidemiology of congenital abnormalities and investigate possible factors associated with their development, autopsies were performed on 649 fetuses referred from three healthcare facilities in Thessaloniki during 1992-2008. Adequate statistical analysis methods were implemented. Birth defects were found in 256 fetuses (39.5%)-primarily related to the musculoskeletal (17.3%), nervous (14.5%), cardiovascular (12.5%) and urinary

Multidisciplinary Management of Pregnancy in Bladder Exstrophy: A Case Report.

BACKGROUND Classic bladder exstrophy (CBE) is a rare congenital condition affecting multiple organs, primarily the urinary and musculoskeletal systems. Management involves multiple reconstructive surgical procedures and aims to maintain renal function, achieve urinary continence, and support normal reproductive health. Pregnancies in women with CBE can be complicated by recurrent urinary tract infections, pelvic prolapse, and a higher risk of preterm birth. CASE REPORT We report the case of a 33-year-old woman with a history of CBE, admitted at 32 weeks of pregnancy with fever and symptoms of …

IL-34 as a Novel Mediator Linking Vitamin D Deficiency with Osteoporosis and Knee Osteoarthritis.

Vitamin D deficiency (VDD) is a widespread human condition closely associated with musculoskeletal disorders, involving alterations in body composition and systemic inflammation. In this cross-sectional study, 1075 adults were classified into a VDD (n = 304) group and a VD sufficient (VDS) group (n = 771). Body composition measurements, including the fat mass-to-weight ratio (FM/WT), were assessed using bioelectrical impedance analysis. Plasma levels of IL-9, IL-34, and MCP-1 were also measured. Data on osteoporosis (OP) and knee osteoarthritis (KOA) were collected. Regression analyses indicat

Advanced Gastric Adenocarcinoma Presenting With Shoulder Pain and Diplopia.

Gastric adenocarcinoma is a leading cause of cancer-related mortality worldwide, with many cases diagnosed at an advanced stage. While liver, peritoneal, and lymphatic involvement are common, metastases to soft tissue and orbital structures are exceedingly rare and may present with non-specific symptoms that mimic benign conditions. We report the case of a 48-year-old woman who presented with persistent musculoskeletal pain and diplopia. Initial symptoms were attributed to sciatica, but further evaluation revealed a right orbital mass, gastric antral thickening, peritoneal carcinomatosis, and

A unique case of late-onset CIPO caused by a missense mutation in the long isoform of FLNA.

Mutations in the filamin A (FLNA) gene cause a broad range of disorders, affecting musculoskeletal, nervous, vascular, and gastrointestinal systems, collectively known as filaminopathies. In contrast to previously described mutations in the long isoform of FLNA, which alter the reading frame and lead to loss of Filamin A expression resulting in congenital short bowel syndrome or chronic intestinal pseudo-obstruction in pediatric patients, here we present the clinical and genetic features of an adult patient with chronic intestinal pseudo-obstruction in whom whole exome sequencing revealed a no

Regional variation in low-value musculoskeletal surgery: a nationwide study from the Finnish Care Register.

BACKGROUND AND PURPOSE: Healthcare systems globally are grappling with resource constraints and rising costs. Concerns have been raised about "low-value" care, which consumes healthcare resources without benefiting patients. We aimed to examine regional differences in common low-value musculoskeletal surgeries in Finland and explore explanatory factors behind the variation. METHODS: Using data from the Finnish Care Register for Health Care, surgeries conducted from 2006-2007 compared with 2020-2021 were analyzed across 20 hospital districts. Selected surgeries (acromioplasty, rotator cuff rep

Evidence for therapeutic use of cannabidiol for nail-patella syndrome-induced pain in a real-world pilot study.

Nail-patella syndrome (NPS) is a rare genetic disease characterized by dysplastic nails, patella abnormalities, skeletal malformation, and chronic pain. Although chronic pain in NPS is mainly due to bone and musculoskeletal symptoms, it can also result from neurological dysfunction. Conventional analgesics are often insufficient to relieve NPS-associated chronic pain. Cannabinoids, which act on the serotonergic and/or noradrenergic pain systems, may therefore represent valuable non-psychoactive alternatives for managing pain in these patients. The effectiveness and safety of synthetic cannabid

Leaky gut, circulating immune complexes, arthralgia, and arthritis in IBD: coincidence or inevitability?

Most host-microbiota interactions occur within the intestinal barrier, which is essential for separating the intestinal epithelium from toxins, microorganisms, and antigens in the gut lumen. Gut inflammation allows pathogenic bacteria to enter the blood stream, forming immune complexes which may deposit on organs. Despite increased circulating immune complexes (CICs) in patients with inflammatory bowel disease (IBD) and discussions among IBD experts regarding their potential pathogenic role in extra-intestinal manifestations, this phenomenon is overlooked because definitive evidence demonstrat
