Supraorbital craniotomy by fracture of the anterior orbital roof. Technical note.

The authors describe a new and rapid method to safely perform a supraorbital craniotomy. This technique can be used when tumor does not invade the orbital roof. Previous descriptions of the supraorbital craniotomy involved exposure of the frontal sinus by removing its anterior wall and using the Gigli saw to separate the orbital roof. This new approach avoids removal of the anterior sinus wall and separates the supraorbital bone flap from the calvaria by fracturing the anterior orbital roof forward. In addition, a method for harvesting a laterally based pericranium and muscle pedicle that cont…

Aneurysmal bone cyst: report of a case mimicking orbital tumor.

Aneurysmal bone cysts are rare bone tumors which normally affect the vertebral region or long bones. Involvement of facial bones is uncommon. Although nonneoplastic, these lesions are expansile and may cause local destruction of bone and soft tissues. When making the diagnosis, it is imperative to consider the clinical, radiologic, and histologic features of the entity to avoid confusion with other possibly malignant giant cell tumors. We report an unusual case involving the orbit in a 2 1/2-year-old girl.

Lack of complications of the hydroxyapatite orbital implant in 250 consecutive cases.

The coral-derived hydroxyapatite sphere is a popular, new integrated orbital implant designed to provide improved motility of the ocular prosthesis following enucleation. Although the implant has rapidly become widely used by ophthalmologists, there is little information available regarding the complications of this technique in a large series of cases. We report our results on our initial 250 consecutive cases of hydroxyapatite implantation for eyes enucleated primarily for intraocular neoplasms, with specific emphasis on the complication an their management. The reasons for enucleation inclu…

Spontaneous orbital hemorrhage in adult females. A report of three cases.

Three cases of spontaneous orbital hemorrhages in 3 adult females are reported. All of the patients had acute radiating pain in the orbit, vomiting and proptosis with a limitation of motility and ecchymosis of the eyelids. One was due to a large orbital varix with a preceding history of intermittent exophthalmos; the causes of the other cases could not be determined from their backgrounds. Within a few weeks, all of them had recovered from hematoma and had good prognoses without surgery. Orbital venous bleeding with 40 mm Hg pressure will cause more than 500 g tension on the four rectus muscle…

[Surgical treatment of endocrine orbital disease. Indications, methods, results].

Surgical removal of the thyroid gland in Graves-Basedow's disease has, beyond doubt, also a major effect on endocrine orbitopathy. After total or near-total removal of the thyroid gland after three months a slighter deterioration or even ameloriation of ocular protrusion is found, contrary to the effect of subtotal thyroidectomy. There exists a significant correlation between the volume of the residual gland evaluated according to 24 h-uptake of 131I, and between the regression of the protrusion (r = 0.385, p < 0.01). Total or near-total thyroidectomy alone even in the absence of other therape…

The medical orbital strut in the prevention of postdecompression dystopia in dysthyroid ophthalmopathy.

Ocular dystopia with inferomedial displacement of the globe is an unusual but well-recognized complication of bony orbital decompression. It is caused by displacement of the muscle cone and orbital connective tissue system into the maxillary and ethmoidal sinuses. We illustrate a surgical variation of bony orbital decompression leaving intact a strut of bone at the maxillary-ethmoid junction. This bony strut reduces inferomedial displacement of the muscle cone and provides a medial supporting "ledge" in cases requiring late orbital reconstruction.

Ophthalmic aspects of orbital injury. A comprehensive diagnostic and management approach.

Improved prognosis for eye injuries sustained during facial trauma depends on early assessment and accurate diagnosis. The surgeon responsible for management of the orbital fracture(s) must perform an ocular assessment at the time of initial examination. Abnormalities found on the five-point assessment outlined herein should lead to urgent ophthalmic consultation. Fracture repair should not be undertaken until the results of consultation have been discussed with the ophthalmologist. Vision should be carefully monitored in the preoperative period because delayed optic neuropathy may occur. Simi…

Orbital volume measured by a low-dose CT scanning technique.

A method for measuring orbital volume using low-dose CT with contiguous 3 mm transaxial sections is described. The accuracy of the method is 1.6%, as demonstrated by comparing CT volume measurements with those derived directly from alginate impressions and on repeat scanning the precision of the measurement was judged as 1.3%. Within the same individual, the right and left orbital volumes were observed to be within 0.6 cm3 (s.d. +/- 0.33 cm3) of each other. This study demonstrates that low-dose CT scanning is a practical method of determining orbital volume and could be used to advantage in th…

Orbital metastases from transitional-cell cancer of the urinary bladder.

Transitional-cell cancer accounts for 90% of the histological types of cancer of the urinary bladder. This tumor most often spreads towards the lymph nodes, lung and bones, but other much more infrequent metastatic sites such as the orbit, have also been described. We report on 2 patients with transitional-cell bladder tumors that developed metastases to the orbit.

Advancement-onlay: an improved technique of fronto-orbital remodeling in craniosynostosis.

Eighteen patients with nonsyndromic craniosynostosis underwent fronto-orbital remodeling with an advancement-onlay technique. The mean age of the infants was 5 months (range = 2-11 months) when the procedure was performed for the following indications: unilateral coronal synostosis (n = 10); bilateral coronal synostosis (n = 3); metopic synostosis (n = 2); and multiple craniosynostoses (n = 3). The technique consists of (1) unilateral or bifrontal craniotomy, (2) superior orbital rim recontouring and advancement, and (3) frontal bone graft rotation and onlay. Posteriorly, the frontal bone graf…

[Fractures of the orbital floor. Apropos of a homogeneous series of 70 patients].

The authors report their experience based on a homogeneous series of 70 fractures of the orbital floor. Different anatomo-clinical forms were defined in particular fractures of the orbital floor, accompanying an "internal pivoting" of the cheek bone, which by their incarceration mechanism resemble the trap-door fractures. The blow-out fracture associated with the lower orbital margin also raises therapeutic problems. After a clinical and CT study, the authors recommend treatment via the lower orbital approach using silastic implants. The different sequelae are described clinically and are appr…

Management of patients with facial trauma and associated ocular/orbital injuries.

Ocular injury can be associated with facial fractures in a significant percentage of cases. It is mandatory for surgeons treating such fractures to be familiar with the types of orbital injuries, the appropriate physical examination and diagnostic tests, and the management techniques involved in treating traumatic visual loss. This article reviews the diagnostic and therapeutic approaches to this important problem.

Early treatment of orbital floor fractures with catheter balloon in children.

Fifteen pediatric patients with orbital floor fractures were successfully treated early, following injury by a transantrally introduced catheter-balloon technique. The latter stabilizes and supports the torn, but still vascularized periosteum at the edges of the fracture and also serves as a guide for its further proliferation and new bone formation. Apart from a transient anesthesia in the dermatome of the infraorbital nerve in certain patients no complication or sequela was noted in this series. Accordingly early repair with the use of catheter-balloon appears to be a viable alternative in t…

Orbital emphysema causing vision loss after a dental extraction.

The use of high-speed air-cooled rotary cutting instruments is common in dental practices. When used near open wounds, the forced air can lead to subcutaneous emphysema and involvement of vital structures. This case highlights the unfortunate episode of orbital emphysema with optic nerve damage after a pneumatically cooled drill was used in the removal of a mandibular second molar.

Naso-orbital-ethmoid injury: report of a case and review of the literature.

Trauma to the midface severe enough to cause nasal fractures will often result in fractures of the bony naso-orbital-ethmoid (NOE) complex. Extensive damage may be encountered in the interorbital area because of the fragility of the bony framework. The medial orbital walls, ethmoid sinuses, and cribriform area of the anterior cranial fossa can resist a maximum compressive force of only 30 g. NOE injuries are usually the result of either a direct blow sustained during an altercation or of a high-velocity motor vehicle accident. Quite often multiple systemic injuries serve to complicate treatmen…
