Hospital delivery volume, severe obstetrical morbidity, and failure to rescue.

BACKGROUND: In the setting of persistently high risk for maternal death and severe obstetric morbidity, little is known about the relationship between hospital delivery volume and maternal outcomes. OBJECTIVE: The objectives of this analysis were (1) to determine maternal risk for severe morbidity during delivery hospitalizations by hospital delivery volume in the United States and (2) to characterize, by hospital volume, the risk for death in the setting of severe obstetric morbidity, a concept known as failure to rescue. STUDY DESIGN: This cohort study evaluated 50,433,539 delivery hospita

Should We Postpone Elective Cardiovascular Procedures and Percutaneous Coronary Interventions During the COVID-19 Pandemic?

BACKGROUND: Prioritization among patients with coronary artery disease represents a difficult issue during the SARS-CoV-2 pandemic. We present our clinical practices and patients' outcomes after elective, emergent, and urgent cardiovascular surgery and percutaneous coronary interventions (PCI). We also investigated the rate of nosocomial infection of SARS-CoV-2 in health workers (HWs), including surgeons after cardiovascular procedures and percutaneous interventions (PCI). MATERIAL AND METHODS: We performed 186 cardiovascular operations and PCI between March 15 and October 15. According to th

Efficacy, patient-reported outcomes, and safety of the anti-granulocyte macrophage colony-stimulating factor antibody otilimab (GSK3196165) in patients with rheumatoid arthritis: a randomised, phase 2b, dose-ranging study.

BACKGROUND: The human monoclonal antibody otilimab inhibits granulocyte-macrophage colony-stimulating factor (GM-CSF), a key driver in immune-mediated inflammatory conditions. We aimed to evaluate the efficacy, safety, and key patient-reported outcomes related to pain in patients with active rheumatoid arthritis receiving otilimab. METHODS: This phase 2b, dose-ranging, multicentre, placebo-controlled study was done at 64 sites across 14 countries. Patients aged 18 years or older with rheumatoid arthritis who were receiving stable methotrexate were randomly assigned (1:1:1:1:1:1) to subcutaneo

Atrial fibrillation after cardiac surgery: a major morbid event?

OBJECTIVE: The purpose of the study was to investigate the incidence, predictors, morbidity, and mortality associated with postoperative atrial fibrillation (AF) and its impact on intensive care unit (ICU) and postoperative hospital stay in patients undergoing cardiac surgery in the Department of Veterans Affairs (VA). SUMMARY BACKGROUND DATA: Postoperative AF after open cardiac surgery is rather common. The etiology of this arrhythmia and factors responsible for its genesis are unclear, and its impact on postoperative surgical outcomes remains controversial. The purpose of this special subst

[Unclear acute respiratory failure in a 64-year-old woman after coronary intervention].

HISTORY AND ADMISSION FINDINGS: Due to a retroperitoneal hematoma after cardiac catheterization a 64 year-old woman received two concentrates of red blood cells. Within two hours after transfusion the patient developed acute dyspnoea, anxiety and shivering. INVESTIGATIONS: Computertomography (CT) of the chest revealed a new bilateral, basally accented pulmonary edema. Pulmonary embolism was ruled out. A cardiac cause in terms of cardiogenic shock had been excluded by echocardiography and other non-invasive measurements. Moreover, no circulatory overload (transfusion-associated circulatory ove

A multicentre randomised controlled trial of Transfusion Indication Threshold Reduction on transfusion rates, morbidity and health-care resource use following cardiac surgery (TITRe2).

BACKGROUND: Uncertainty about optimal red blood cell transfusion thresholds in cardiac surgery is reflected in widely varying transfusion rates between surgeons and cardiac centres. OBJECTIVE: To test the hypothesis that a restrictive compared with a liberal threshold for red blood cell transfusion after cardiac surgery reduces post-operative morbidity and health-care costs. DESIGN: Multicentre, parallel randomised controlled trial and within-trial cost-utility analysis from a UK NHS and Personal Social Services perspective. We could not blind health-care staff but tried to blind participant

Antihypertensive Treatment Escalation and Maternal Outcomes During Expectant Management of Early-Onset Preeclampsia With Severe Features.

BACKGROUND: Early-onset preeclampsia with severe features is associated with substantial maternal morbidity. During expectant management, oral antihypertensive medications are frequently escalated and intravenous agents are administered for severe hypertension, yet the clinical significance of escalating antihypertensive treatment has not been well characterized. OBJECTIVE: To evaluate the association between antihypertensive treatment escalation during expectant management of early-onset preeclampsia with severe features and adverse maternal outcomes. STUDY DESIGN: This retrospective cohort

Short-Term Safety and Healthcare Utilization Following Intragastric Balloon Placement in Patients With and Without Diabetes Mellitus: A Propensity-Matched Analysis.

BACKGROUND: Intragastric balloon (IGB) therapy is a minimally invasive endoscopic intervention for weight reduction in patients with Class I-II obesity and in lower BMI patients with obesity-related comorbidities such as diabetes mellitus. Diabetes mellitus is associated with vascular complications, increased susceptibility to infection and delayed gastric emptying, underscoring the need to evaluate procedural safety and device tolerance in this population. However, evidence regarding short-term safety in this higher-risk population remains limited. This study aims to compare 30-day safety out

[Diagnostic value of combined application of semaphorin 7A, calponin 1 and D-dimer in acute aortic dissection].

OBJECTIVES: Acute aortic dissection (AAD) is a life-threatening condition with difficulty in early differential diagnosis, and ideal biomarkers are still lacking. This study aims to investigate the diagnostic value of serum semaphorin 7A (Sema7A), calponin 1 (CNN1), and plasma D-dimer, alone and in combination, in patients with AAD. METHODS: A retrospective study was conducted. 90 patients with AAD who visited the Second Xiangya Hospital of Central South University from December 2022 to December 2023 were enrolled, along with 53 patients with acute myocardial infarction (AMI) and 18 patients

Incidence and risk factors associated with index hospitalization leg ischemia following endovascular abdominal aortic aneurysm repair.

OBJECTIVE: Although the incidence of index hospitalization leg ischemia after abdominal aortic aneurysm repair is low (1%-3%), it is a serious complication that merits investigation. Preoperative risk factors, such as demographics, medical history, and medication use (or a lack of) that may be associated with this complication are understudied. Our aim was to predict the factors that may be associated with the development of leg ischemia after endovascular repair in the index hospitalization period. METHODS: All deidentified patient records from the Vascular Quality Initiative's abdominal aor

Preoperative COVID-19 infection increases risk for 60-day complications following total shoulder arthroplasty: a propensity-matched analysis.

BACKGROUND: Total shoulder arthroplasty (TSA) is an effective treatment for a wide range of shoulder pathologies. Literature investigating the impact of COVID-19 diagnosis on outcomes following TSA is limited. The objective of this study was to perform a retrospective multi-institutional database analysis to investigate the association between preoperative COVID-19 diagnosis and 60-day complications following TSA. METHODS: We queried the TriNetX database using Current Procedural Terminology and the International Classification of Diseases, Tenth Revision codes for patients who underwent a TSA

An Evaluation of the Safety of Half-Dose Direct Oral Anticoagulants Following Total Joint Arthroplasty: A Pilot Study.

Background Total joint arthroplasty (TJA) patients on preoperative anticoagulation therapy present a challenge for adult reconstruction surgeons. The goal in managing such patients is to limit bleeding complications associated with administering the medications while preventing medical complications from withholding them. At our institution, we began a protocol in 2017 that utilizes a half-dose direct oral anticoagulant (DOAC) regimen for one week followed by resuming of the full-dose regimen in select patients who underwent TJA. This study investigated the 90-day safety profile associated wit

Characteristics and in-hospital outcomes of female patients presenting with ST-segment-elevation myocardial infarction without standard modifiable cardiovascular risk factors.

BACKGROUND: Standard Modifiable Cardiovascular Risk Factors (SMuRF) such as hypertension, diabetes mellitus, hyperlipidemia, and smoking have long been established in the etiology of atherosclerotic disease. We evaluate in-hospital outcomes of female STEMI patients without these risk factors. METHODS: The National Inpatient Sample databases (2016 to 2021) were queried to identify STEMI admissions as a principal diagnosis using ICD 10 codes. Patients with a history of coronary artery disease, myocardial infarction, coronary bypass graft, percutaneous coronary intervention, takotsubo cardiomyop

Lower Intraoperative and Immediate Postoperative Complications in Robotic Versus Conventional Primary Total Hip Arthroplasty: A Retrospective Cohort Analysis of Over 360,000 Patients.

PURPOSE: Although a trend of an improved alignment with robotic total hip arthroplasty (THA) over conventional methods has surfaced from recent series, it is unknown whether these results translate into meaningful enhancements in postoperative outcomes. To address this lack in the literature, we compared the perioperative morbidity and mortality with robotic and conventional THA in a large national cohort of 367,894 patients. We hypothesized that no significant differences would exist in the outcomes between the two groups. METHODS: Records were extracted from 2016-2019 from the National Inpa
