Female BRCA mutation carriers with a preference for prophylactic mastectomy are more likely to participate an educational-support group and to proceed with the preferred intervention within 2 years.

Women with a BRCA mutation face a complex choice between breast cancer surveillance and prophylactic mastectomy. We determined risk management preferences shortly after genetic test disclosure and mastectomy status after a median observation period of 2 years. The effect of an educational-support group on the realisation of risk management preference was explored. We included 163 newly disclosed BRCA mutation carriers with no history of cancer, whose breast cancer risk management preferences were recorded. All carriers were offered the opportunity to participate an educational-support group. M

Transferring disease management and health promotion programs to other countries: critical success factors.

Governments and health service providers around the world are under pressure to improve health outcomes while containing rising healthcare costs. In response to such challenges, many regions have implemented services that have been successful in other countries-but 'importing' initiatives has many challenges. This article summarizes factors found to be critical to the success of adapting a US disease management and health promotion programme for use in Italy and the UK. Using three illustrative case studies, it describes how in each region the programme needed to adapt (i) the form and content

Understanding and treating panic disorder in the primary care setting.

According to studies, the median prevalence of panic disorder in the primary care setting is 4%. Rates are higher among certain patient popu … ommendations) is the second important contributor to inadequate treatment. Use of a chronic disease self-management approach would enhance treatment of panic disorder. This model requires that patients, in collaboration with the health care provider/system, take day-to-day responsibility for managing their illness by doing 3 things: adhering to recommended medica …

Design and baseline characteristics of the epidemiology and natural history of asthma: Outcomes and Treatment Regimens (TENOR) study: a large cohort of patients with severe or difficult-to-treat asthma.

BACKGROUND: Patients with severe and difficult-to-treat asthma represent a small percentage of asthma patients, yet they account for much of the morbidity, mortality, and cost of disease. The goal of The Epidemiology and Natural History of Asthma: Outcomes and Treatment Regimens (TENOR) study is to better understand the natural history of asthma in these patients. OBJECTIVE: To describe the methods and baseline characteristics of the TENOR study cohort. METHODS: The TENOR study is a 3-year, multicenter, observational study of patients with severe or difficult-to-treat asthma. From January th

Automated, patient-interactive, spinal cord stimulator adjustment: a randomized controlled trial.

OBJECTIVE: Programmable, multicontact, implanted stimulation devices represent an important advance in spinal cord stimulation for the management of pain. They facilitate the technical goal of covering areas of pain by stimulation-evoked paresthesiae. Adjustment after implantation requires major investments of time and effort, however, if the capabilities of these devices are to be used to full advantage. The objective of maximizing coverage should be met while using practitioners' time efficiently. METHODS: We have developed a patient-interactive, computerized system designed for greater eas

'We Didn't Really Learn How to Manage It': An In-Depth Mixed-Methods Exploration Into Temporomandibular Disorder Management Behaviours of First-Contact Healthcare Practitioners in Australia.

BACKGROUND: Temporomandibular disorders (TMDs) affect approximately 10% of the population. Management is often challenging, which can negati … erapists for initial care. Interventions perceived most important in first-line management included self-management strategies, advice and education and stress management (≥ 73%). Small but statistically significant associations were observed between practitioners' referral behaviours and their occupation, and patients' main presenting complaint (p < 0.05). Leadi …

Endoscopic Treatment of the Coloanal Anastomotic Dehiscence and Stricture: Stenting Via Rendezvous Technique.

An anastomotic leak is a serious complication of colorectal surgery. Leak management is often grueling, and a definitive stoma is often mandatory for rescue therapy. Herein, we present a patient who experienced coloanal anastomotic dehiscence and complete stricture at the proximal part of the anastomosis. This case was successfully treated with a fully covered self-expandable metallic stent placement via the combined endoscopic (per ileostomy) and manual (per anal channel) rendezvous technique.

Fulminant descending mediastinitis secondary to infectious mononucleosis.

Descending mediastinitis is a rare, life-threatening condition caused by contiguous spread of oropharyngeal or cervical infection into the mediastinum. Infectious mononucleosis generally results in a self-limited illness characterized by fever, pharyngitis and lymphadenopathy. We present an exceptional case of an 18-year-old with infectious mononucleosis complicated by progressive bacterial superinfection and fulminant descending mediastinitis. After resuscitation, broad spectrum antibiotics, critical care support and definitive surgical management, they made a full recovery.

Necrotizing sialometaplasia in a patient with an eating disorder: palatal ulcer accompanied by dental erosion due to binge-purging.

This report describes a case of necrotizing sialometaplasia (NS) accompanied by significant dental erosion of the maxillary teeth of the palatal surfaces owing to chronic self-induced vomiting. This observation contributed to the determination of an immediate and appropriate provisional diagnosis of NS in a patient with an eating disorder, which subsequently was confirmed histopathologically as NS. The diagnostic challenges presented by NS associated with eating disorders and its management are discussed.

Viral infections of the skin: clinical features and treatment options.

Viral infections of the skin can cause significant distress and embarrassment for people who are affected. This article explores four specific viral infections that manifest on the skin: herpes simplex, herpes zoster, viral warts and molluscum contagiosum. It describes the presentation and appearance of the virus as it affects the skin and what can be done to minimise the spread of the virus. Management options are considered, although many of these conditions are self-limiting.

Henoch-Schönlein purpura with intussusception in an adult.

Often seen in childhood, Henoch-Schönlein purpura (HSP) is a self-limiting illness, in which complications involving intussusception are rarely found in adults. This study reported a case of HSP initially presented as intussusception in a 60-year-old man. Gastrointestinal manifestations before the onset of rash may prevent an accurate diagnosis and appropriate management. Primary care physicians should be alert for this association between intussusception and HSP, especially in adult patients.

Thyroid associated orbitopathy.

Thyroid associated orbitopathy, also known as Graves' orbitopathy, is typically a self-limiting autoimmune process associated with dysthyroid states. The clinical presentation may vary from very mild disease to severe irreversible sight-threatening complications. Despite ongoing basic science and clinical research, the pathogenesis and highly effective therapeutic strategies remain elusive. The present article reviews the pathophysiology, clinical presentation, and management of this common, yet poorly understood disease, which remains a challenge to the ophthalmologist.

Gore-Tex Medialization Thyroplasty: objective and subjective evaluation.

Management of vocal fold paralysis and paresis continues to give rise to controversy and the use of numerous surgical techniques. The senior authors' (RTS, JRS) initial experience with fourteen patients using Gore-Tex Medialization Thyroplasty suggests that this technique has value. Evaluation included pre- and postoperative videostroboscopic assessment, aerodynamic measures, acoustic analysis, phonatory analysis, psychoacoustic evaluation, and patient self-evaluation. Stroboscopic and perceptual assessments were blinded. Statistical analysis supports the efficacy of this technique.

Infectious mononucleosis.

EBV-induced IM is a generally self-limited infection characterized by fever, pharyngitis, and adenopathy. Management consists of basic supportive measures and treatment of streptococcal pharyngitis when present. Corticosteroids may be considered for individuals who exhibit evidence of significant upper airway obstruction. To date there is little evidence to support the use of antiviral agents in immunocompetent patients. Complications of IM may arise, which can be life-threatening, but these are relatively rare.

Acute opium poisoning: a report of two hundred cases in Iran.

The diagnosis and management of 200 cases of self-poisoning due to orally ingested crude opium in Iran is reported and discussed. Gastric lavage supported when necessary by nalorphine was effective treatment in the majority of cases because of the slow absorption of opium. Mechanical ventilation was required in twelve cases (6%) only. There were three deaths; two of these were due to pulmonary oedema which was apparently not the result of hypoxia.

The complex relationship between treatment burden of multimorbidity and self-care in multimorbid patients with hypertension.

BACKGROUND: Multimorbid patients with hypertension experienced treatment burden from managing multiple chronic conditions. Although treatment burden can adversely affect self-care, several qualitative studies have suggested a complex relationship between the two factors. This study aimed to identify patient groups based on the level of multimorbidity treatment burden and self-care adherence and explore factors associated with these patient groups. We also examined if patients transitioned to a different group over six months and which factors were associated with either transitioning into or r

Snare-assisted bailout of an embolized Evolut FX transcatheter aortic valve replacement prosthesis using a valve-through-valve balloon-expandable implantation.

Transcatheter aortic valve replacement (TAVR) is now the standard of care for severe symptomatic aortic stenosis; however, rare procedural complications such as valve embolization can result in life-threatening consequences. We describe a case of intraprocedural embolization of a self-expanding Medtronic Evolut FX valve into the ascending aorta during deployment. The embolized prosthesis was stabilized with a snare and parked distal to the sinotubular junction. Because a second self-expanding valve could not be advanced across the displaced frame, a 23 mm balloon-expandable Edwards SAPIEN 3 Ul

The Sound of Silence: Engaging the Quiet Adolescent.

Engaging the silent adolescent is a major psychotherapeutic challenge. This article presents a comprehensive approach to this problem, illustrated with clinical material. This approach emphasizes a careful diagnostic assessment, including an assessment of the patient's level of personality organization and capacities to participate in psychodynamic psychotherapy. Three approaches to the silent teenager are explored in depth: a mostly supportive approach focused on containment of maladaptive behaviors; a psychodynamic approach with supportive elements focused on demonstrating safety through hum

Pneumomediastinum Associated with COVID-19: A Case Series.

Coronavirus disease 2019 (COVID-19), caused by the new coronavirus SARSCoV-2, is an infectious disease that has caused an increase in hospitalizations for pneumonia; the spectrum of clinical presentation is variable. One such presentation, pneumomediastinum (PM), is defined as the presence of air or some other gas in the mediastinum. It is a rare condition, usually benign and self-limited; it has been seen in patients with COVID-19. Although most cases are, as explained above, self limited and in addition, can be managed conservatively, close monitoring is recommended, as PM can cause life-thr

Psychosocial Consequences of Bariatric Surgery: Two Sides of a Coin: a Scoping Review.

This scoping review summarizes current evidence with regard to the impact of bariatric surgery on psychological health in adults with obesity. While a large body of evidence reports major metabolic benefit and improved quality of life, there is also ample evidence suggesting an increased incidence of self-harming behavior, a greater likelihood of developing an alcohol problem and higher rates of completed suicide among bariatric patients. Being able to identify the "at risk" patient population requires more longitudinal research into the risk factors for psychological complications after baria
