Altered Cerebellar Spontaneous Activity and Its Association with Arousal Index in Comorbid Insomnia and Obstructive Sleep Apnea: A Resting-State fMRI Study.

Background: Frequent nocturnal arousals are a core feature of comorbid insomnia and obstructive sleep apnea (COMISA), yet the underlying central mechanisms remain unclear. Identifying brain functional correlates of nocturnal awakenings may help clarify arousal-related mechanisms and inform potential interventional targets. Methods: A total of 99 participants (COMISA, insomnia alone, OSA alone, and healthy controls) underwent clinical assessments, polysomnography, and brain magnetic resonance imaging (MRI). MRI metrics were compared across groups, followed by correlation and regression analyses

Association of peripheral NLRP3 inflammasome activation with prevalent and incident chronic kidney disease in patients with obstructive sleep apnea: a prospective study.

BACKGROUND: Obstructive sleep apnea (OSA) has been increasingly associated with chronic kidney disease (CKD). NLRP3, a central component of the inflammasome, is involved in hypoxia- and inflammation-related pathways. We therefore investigated whether circulating NLRP3 was associated with prevalent CKD and incident CKD risk in patients with OSA. METHODS: This single-center observational study included 648 OSA patients. Multivariable linear and logistic regression models were used to evaluate the associations of circulating NLRP3 with renal function indices and prevalent CKD. Cox proportional h

Treatment preferences for comorbid obesity and obstructive sleep apnea (PRO-CON OSA) survey: Patient and provider preferences for CPAP and/or tirzepatide.

PURPOSE: Approximately one of ten US adults has comorbid obesity and obstructive sleep apnea (COBOSA). Traditionally, sleep medicine management of COBOSA focused on continuous positive airway pressure (CPAP). Recently, tirzepatide (a once-weekly injection) was approved for COBOSA after demonstrating substantial reduction of weight and OSA severity in efficacy trials. We assessed patient and provider attitudes towards these therapies for COBOSA. METHODS: We conducted an online survey (November 2024–August 2025) targeting US adults with OSA and/or obesity (“patients”) and sleep medicine provider

Effects of CPAP on endothelial activation and fibrinolytic balance in coronary artery disease with obstructive sleep apnea: The RICCADSA randomized controlled trial.

BACKGROUND: Obstructive sleep apnea (OSA) promotes endothelial activation and a prothrombotic milieu through intermittent hypoxia, oxidative stress, and systemic inflammation, mechanisms closely linked to atherosclerosis progression. The vascular effects of continuous positive airway pressure (CPAP) therapy in patients with established coronary artery disease (CAD) remain incompletely understood. OBJECTIVE: To evaluate the longitudinal effects of CPAP treatment on endothelial adhesion molecules and fibrinolytic balance in patients with CAD and OSA. METHODS: In this randomized controlled anal

Effect of Continuous Positive Airway Pressure on Left Atrial Volume Index in Patients With Stroke and Obstructive Sleep Apnea: A Randomized Trial.

BACKGROUND: Obstructive sleep apnea is associated with left atrial enlargement, a predictor of stroke. We hypothesized that continuous positive airway pressure (CPAP) would reduce the left atrial volume index (LAVI) and modify its longitudinal trajectory in patients with stroke and obstructive sleep apnea. METHODS: Patients with stroke and obstructive sleep apnea were randomized to standard care plus CPAP or standard care alone. Coprimary outcomes were absolute 12-month change and longitudinal trajectories of LAVI and N-terminal pro-B-type natriuretic peptide (NT-proBNP), assessed at baseline

MFR, SVR and ASVR as promising biomarkers in overweight and obese population with obstructive sleep apnea syndrome: insights from an observational study.

BACKGROUND: Obesity is an important risk factor of obstructive sleep apnea syndrome (OSAS), but not all obese people can develop OSAS. The role of novel human body composition indices-muscle-to-fat ratio (MFR), skeletal muscle mass-to-visceral fat area ratio (SVR) and appendicular skeletal muscle mass-to-visceral fat area ratio (ASVR)-in OSAS remains largely unknown. The aim of this study is to investigate the associations of these indices to OSAS in overweight and obese adults. METHODS: Overweight or obese patients with complete human body composition and polysomnography data were included i

The genetic overlap and causal relationship between attention deficit hyperactivity disorder and obstructive sleep apnea: a large-scale genomewide cross-trait analysis.

BACKGROUND: Attention deficit hyperactivity disorder (ADHD) and Obstructive sleep apnea (OSA) are highly clinically co-occurring, but the mechanisms behind this remain unclear, so this article analyzes the reasons for the co-morbidities from a genetic perspective. METHODS: We examined the genetic architecture of ADHD and OSA based on the large genome-wide association studies (GWAS). The global genetic relationship between OSA and ADHD was explored. Cross-trait analysis from single nucleotide polymorphism (SNP) and gene level was performed subsequently to detect the crucial genomic regions. Fi

Health-related quality of life in obstructive sleep apnea patients treated with maxillomandibular advancement surgery: A cross-sectional pilot study (PILOT-QOMAS).

BACKGROUND: Patients with Obstructive Sleep Apnea(OSA) have a lower Health-Related Quality of Life(HRQoL) than individuals without OSA. Maxillomandibular Advancement(MMA) surgery effectively reduces OSA severity, which is often the focus of research. However, evaluating HRQoL seems essential to understand the treatments overall impact. STUDY DESIGN: The primary objective of this cross-sectional pilot study is to evaluate the post-operative HRQoL of patients who had MMA surgery for OSA, compared to a control group of patients who had bimaxillary surgery for facial skeletal deformity. The prima

App-Based Myofunctional Therapy to Improve Adherence and Outcomes of Mandibular Advancement Devices in Moderate Obstructive Sleep Apnea: Multicenter Randomized Trial Protocol.

BACKGROUND: Mandibular advancement devices (MADs) are a valid alternative for patients with moderate obstructive sleep apnea (OSA) who do not tolerate continuous positive airway pressure. Despite their efficacy, both adherence and long-term results remain a challenge. Myofunctional therapy (MT) has shown potential to improve upper airway muscle tone and reduce OSA severity, but its role as an adjunct to MAD treatment is still unclear. This protocol evaluates whether app-based MT (Airway Gym) can improve adherence and clinical outcomes in patients with moderate OSA treated with MADs. We describ

Cerebral oxygenation responses to obstructive sleep apnea in cognitively normal older adults: a study using simultaneous polysomnography and functional near-infrared spectroscopy.

Obstructive sleep apnea (OSA) can cause intermittent cerebral hypoxia that may impair brain health. However, widely used clinical indicators such as the Apnea-Hypopnea Index (AHI) and peripheral oxygen saturation (SpO₂) may not accurately capture cerebral oxygen dynamics. In this study, we recorded frontal lobe cerebral oxygen saturation (rSO₂) using functional near-infrared spectroscopy (fNIRS) while concurrently acquiring overnight polysomnography (PSG) in 23 cognitively normal older adults (13 with OSA and 10 controls). Our results demonstrated that rSO₂ was negatively associated with apnea

Navigating ambulatory perioperative care in patients with obesity and obstructive sleep apnea: a narrative review of ambulatory surgery outcomes and opioid safety.

Obesity and sleep apnea contribute to significant challenges in ambulatory surgical centers which include airway management and perioperative recovery. This narrative review was written following a literature review of articles available on ScienceDirect, PubMed, and Google Scholar from 2009 to 2024. This article highlights that obesity-related anatomical changes, such as increased neck circumference (NC) and reduced cervical extension, may complicate airway management. Successful ambulatory surgery for these high-risk patients depends on detailed attention throughout the perioperative period.
