Disconnection of network hubs and cognitive impairment after traumatic brain injury.

Traumatic brain injury affects brain connectivity by producing traumatic axonal injury. This disrupts the function of large-scale networks that support cognition. The best way to describe this relationship is unclear, but one elegant approach is to view networks as graphs. Brain regions become nodes in the graph, and white matter tracts the connections. The overall effect of an injury can then be estimated by calculating graph metrics of network structure and function. Here we test which graph metrics best predict the presence of traumatic axonal injury, as well as which are most highly associ

Brain Age Modeling and Cognitive Outcomes in Young Adults With and Without Sickle Cell Anemia.

IMPORTANCE: Both sickle cell anemia (SCA) and socioeconomic status have been associated with altered brain structure and cognitive disability, yet precise mechanisms underlying these associations are unclear. OBJECTIVE: To determine whether brains of individuals with and without SCA appear older than chronological age and if brain age modeling using brain age gap (BAG) can estimate cognitive outcomes and mediate the association of socioeconomic status and disease with these outcomes. DESIGN, SETTING, AND PARTICIPANTS: In this cross-sectional study of 230 adults with and without SCA, individu

Depth or Breadth? Domain-Specific Expertise and Cross-Domain Engagement in the Development of Scholarly Excellence.

Scholarly excellence is often equated with deep specialization within a single domain, a premise embedded in doctoral training, peer review, and academic-evaluation systems. However, research from the emerging "science of science," drawing on large-scale analyses of publications, citations, and career trajectories, suggests a more complex relationship between expertise, cross-domain engagement, and scholarly impact. This conceptual article synthesizes insights from the science of science, cognitive research on expertise, and organizational studies to examine the roles of domain-specific depth

Classification of tau status with machine learning models in amyloid-positive cohorts.

INTRODUCTION: Although tau positron emission tomography (PET) imaging is effective for staging tau pathology, it is limited clinically by cost and availability. Machine learning models based on magnetic resonance imaging (MRI)- and amyloid PET-derived features may serve as useful screening tools for tau pathology. METHODS: Multiple machine learning models were developed to classify tau positivity in the Braak III/IV region using structural MRI, amyloid PET, and demographic features. Alzheimer's Disease Neuroimaging Initiative (ADNI) (n = 410) data were used for model training. Open Access Ser

The clinical implications of anosognosia in Alzheimer's disease.

INTRODUCTION: Assessing awareness of patients with Alzheimer's disease (AD) is essential for diagnosis and treatment. Anosognosia, defined as impaired awareness of cognitive and functional deficits, is associated with poorer prognosis, reduced decision-making capacity, increased risk of accidents, and earlier institutionalization. AREAS COVERED: This narrative review examines 53 studies. After outlining the main theoretical models of awareness in dementia, the multidimensional nature of anosognosia and its impact on patients' autonomy and decision-making abilities are discussed. This review a

[Focus on the Brain Under Anesthesia: The Safe Brain Initiative's EEG Bootcamp].

In current anaesthetic practice, monitoring remains largely focused on cardiorespiratory parameters, while the brain, the primary target organ of anaesthesia, is insufficiently monitored. EEG-based neuromonitoring enables direct, continuous, and non-invasive evaluation of the cerebral effects of anaesthetics, yet its clinical application is often limited to numerical indices derived from proprietary algorithms. This simplification of a complex biosignal is prone to artifacts, delays, and systematic bias, particularly in elderly and frail patients. We argue that index-guided anaesthesia alone ma

Effects of temporal lobe seizures on visual recognition memory in a non-human primate model.

Patients with temporal lobe epilepsy frequently report memory impairment, which significantly impacts their quality of life. Several studies have demonstrated an association between temporal lobe epilepsy and memory dysfunction, though the underlying mechanisms remain unclear. This study investigates whether a penicillin-induced temporal lobe seizure model in non-human primates (NHPs) replicates the recognition deficits observed in epileptic patients. We recorded hippocampal activity in three NHPs during a visual paired comparison (VPC) task before and during seizures. The penicillin model ind

Enhanced OSA Detection via Multi-Scale Modeling of EEG and ECG Signals.

Obstructive Sleep Apnea (OSA) is a prevalent sleep disorder that has been linked to various serious health conditions, including cardiovascular diseases, metabolic disorders, and cognitive impairments. Early detection of OSA is crucial for preventing these associated health risks. Although significant progress has been made in the automatic detection of OSA, existing methods predominantly rely on single signals, such as electrocardiogram (ECG), blood oxygen saturation, Airflow, and mainly use one-dimensional networks for feature extraction, which fail to fully leverage the time-frequency chara

The impact of musical expertise on disentangled and contextual neural encoding of music revealed by generative music models.

Music perception requires integrating individual notes into their broader musical context, yet how musical expertise shapes the neural encoding of this information across the auditory hierarchy remains unclear. Here, we address this by using the hierarchical representations from a generative music transformer model to predict human brain activity. We recorded scalp electroencephalography (EEG) from expert musicians and non-musicians, as well as intracranial EEG (iEEG) from six neurological patients, during listening to classical piano pieces. We found that deeper layers of the transformer, whi
