The maintenance of normal hydration is an important function of the skin. The stratum corneum provides an antimicrobial, antioxidant, and UV barrier and plays an integral role in maintaining skin hydration. Environmental factors and disease states may compromise the barrier function of the stratum corneum, leading to excessively dry skin. Evidence supports the use of moisturizers in the treatment of various skin conditions, and a wide variety of these products are currently available. The presence of moisturizing agents in a compound, however, may not guarantee optimal moisturization effects.

Emollients are agents designed to make the stratum corneum softer and more plant by increasing its hydration. A large number of preparations are available today, many of which are marketed as cosmetic and therapeutic moisturizers. They are the most prescribed products in dermatology. Their structure and function are surprisingly complex and sophisticated, and many are equidistant between cosmetics and drugs. The use of the emollients corrects the problems in scaling disorders. It is well known that the electrical properties of the stratum corneum change after application of an emollient. It is

The physician's ability to successfully treat and at least control, if not cure, the skin diseases resulting in pruritus has come through better understanding of skin physiology and the fortuitous discovery of a number of pharmacologic agents that significantly influence the pathologic processes causing itching. While the therapeutic armamentarium is far from perfect, the experienced practitioner can use it to considerable clinical benefit for the pruritus patient.

A worker employed in a textile printing-shop showing a rebellious eczema on hands and forearms, was submitted to three series of patch-tests which disclosed a polysensitization to chromium, to formaldehyde, to the urea-formaldehyde resin and to some pigments (Helizarine BASF, Fluolac FMC). The chemical analysis of some of these pigments shows an interesting relation between allergenicity for a certain pigment and the presence of formaldehyde. Thus, it is an allergy to the pigment-support and not to the pigment itself.

BACKGROUND: Children with skin disorders usually receive care from a pediatrician, despite their limited training in this discipline. The advice of a dermatologist is frequently requested.
OBJECTIVES: To estimate the degree of concordance in the diagnosis, treatment, advice, and recommended follow-up of skin disorders between pediatricians (in private practice or a pediatric emergency department [PED]) and a dermatologist.
METHODS: This prospective study was carried out between June 25 and September 13, 2018. All patients younger than 18 years consulting at the PED of the University Children

Cutaneous inflammation is influenced by systemic signals beyond the skin, and gut microbial metabolites contribute to skin homeostasis and inflammatory responses. Major classes of gut-derived metabolites, including short-chain fatty acids, tryptophan-derived compounds, and secondary bile acids, may shape cutaneous inflammation through immune, neural, and endocrine pathways. However, current research remains fragmented across metabolite classes and pathways, and cross-pathway interactions remain unclear. As a result, the relationship between metabolite disturbances and distinct inflammatory phe

Skin diseases vary significantly across racial and ethnic groups, influenced by genetic, environmental, and socioeconomic factors. However, patients with skin of color (SOC) have historically been underrepresented in research, resulting in disparities in diagnosis, treatment, and access to care. In the April issue of the Journal, we dedicate attention to these key disparities. We focus on the differences in clinical symptoms and treatment challenges related to inflammatory skin diseases such as psoriasis, atopic dermatitis, and hidradenitis suppurativa in SOC patients.

The skin, covering our entire body as its largest organ, manifests enormous complexities and a profound interplay of systemic and local responses. In this heterogeneous domain, B cells were considered strangers. Yet, recent studies have highlighted their existence in the skin and their distinct role in modulating cutaneous immunity across various immune contexts. Accumulating evidence is progressively shedding light on the significance of B cells in maintaining skin health and in skin disorders. Herein, we integrate current insights on the systemic and local contributions of B cells in three p

Changes in the microcirculation of the skin are a frequently observed accompanying phenomenon of many diseases, far beyond the spectrum of dermatological diseases. Not all of these changes are pathological, many are transient and have no serious consequences. This is true for many inflammatory diseases such as psoriasis vulgaris or atopic eczema. However, there are also diseases in which functionally and morphologically recognizable microangiopathies lead to severe disease consequences. One of the most important diseases in this context is systemic sclerosis, an autoimmune systemic disease wit

The close interaction between skin and clothing has become an attractive cornerstone for the development of therapeutic textiles able to alleviate skin disorders, namely those correlated to microbiota dysregulation. Skin microbiota imbalance is known in several skin diseases, including atopic dermatitis (AD), psoriasis, seborrheic dermatitis, rosacea, acne and hidradenitis suppurative (HS). Such microbiota dysregulation is usually correlated with inflammation, discomfort and pruritus. Although conventional treatments, that is, the administration of steroids and antibiotics, have shown some eff

Since its introduction in 1952, topical glucocorticosteroids remain the initial and long-term treatment option for various forms of inflammatory dermatitis. A number of non-steroidal topicals for treating inflammatory dermatoses have been developed in the recent decades (such as topical calcineurin inhibitors, vitamin D analogues, and phophodiesterase-4 inhibitors), but none had the combination of broad therapeutic range, relatively rapid onset of action, high tolerability, and wide-spread clinical success; this allowed topical glucocorticosteroids to remain the mainstay of therapy. This situa

The skin presents a multifaceted microbiome, a balanced coexistence of bacteria, fungi, and viruses. These resident microorganisms are fundamental in upholding skin health by both countering detrimental pathogens and working in tandem with the skin's immunity. Disruptions in this balance, known as dysbiosis, can lead to disorders like psoriasis and atopic dermatitis. Central to the skin's defense system are mast cells. These are strategically positioned within the skin layers, primed for rapid response to any potential foreign threats. Recent investigations have started to unravel the complex

Hereditary alpha tryptasemia (HaT) is a recently identified disorder that is associated with dermatologic manifestations such as urticaria, flushing, pruritus, and atopic dermatitis (AD), as well as a broad range of other symptoms affecting multiple systems. Given the potential cutaneous manifestations and the fact that dermato-logic symptoms may be the initial presentation of HaT, awareness and recognition of this condition by dermatologists are essential for diagnosis and treatment. This review aims to summarize cutaneous presentations consistent with HaT and various conditions that share ov

Melatonin is the main hormone that regulates the sleep cycle, and it is mostly produced by the pineal gland from the amino acid tryptophan. It has cytoprotective, immunomodulatory, and anti-apoptotic effects. Melatonin is also one of the most powerful natural antioxidants, directly acting on free radicals and the intracellular antioxidant enzyme system. Furthermore, it participates in antitumor activity, hypopigmentation processes in hyperpigmentary disorders, anti-inflammatory, and immunomodulating activity in inflammatory dermatoses, maintaining the integrity of the epidermal barrier and the

Dupilumab was first approved for the treatment of atopic dermatitis (AD) and blocks the signaling of interleukin (IL)-4 and -13. Several other chronic skin conditions share mechanistic overlaps with AD in their pathophysiology, i.e., are linked to type 2 inflammation. Most recently, dupilumab was approved by the U.S. Food and Drug Administration for prurigo nodularis (PN). Given its relatively good safety profile, effective off-label use of dupilumab has been reported for a multitude of dermatologic diseases and several clinical trials for dermatologic skin conditions are currently ongoing. Identified in the late nineteenth century as a single species residing on human skin, Malassezia is now recognized as a diverse genus comprising 18 species inhabiting not only skin but human gut, hospital environments, and even deep-sea sponges. All cultivated Malassezia species are lipid dependent, having lost genes for lipid synthesis and carbohydrate metabolism. The surging interest in Malassezia results from development of tools to improve sampling, culture, identification, and genetic engineering, which has led to findings implicating it in numerous skin diseases, Crohn disease, and pancr
